A case of brown tumor on the hard palate associated with primary hyperparathyroidism is reported. The diagnosis was suggested by the oral findings and clinical history and confirmed by biochemical determinations. Excision of a parathyroid adenoma normalized the metabolic situation and at a one-year follow-up the palatal tumor had diminished in size and lamina dura was partially regenerated.

Caution must be used in the elderly in making the diagnosis of complicated migraine, since transient ischemic attacks (TIAs), other cerebrovascular dysfunction, or structural lesions also can produce prolonged neurologic deficits with headache. Although hypertensive headache itself has no specific diagnostic features, patients who do complain of headache with episodes of hypertension usually localize the pain to the occipital region. This headache is most pronounced on awakening in the morning, usually diminishing 2 to 3 hours after arising.

Idiopathic, benign, recurrent or persistent headaches represent the overwhelming majority of headache problems presenting to the general practitioner. Many practitioners have an unrealistic fear of 'missing a tumour' which results in a tendency to pursue exhaustive investigation. With attention to the history detail, the clinician usually can categorise the headache accurately and reassure the patient and himself about its benign nature without the need for expensive and usually unhelpful tests.

In 22 patients with sarcoidosis, symptoms and signs of muscle involvement were scanty, but muscle biopsy showed epithelioid cell granulomas consistent with sarcoidosis in 11 patients (50%). Serial sectioning of the muscle biopsies increased the diagnostic yield. The incidence of muscle involvement was highest among patients with both acute sarcoidosis and erythema nodosum. This is the first reported case of fatal visceral Kaposi's sarcoma which presented with oral lesions and in which skin lesions were absent throughout the course of the disease. Both clinical and histologic recognition may be difficult because of the similarity of the lesions to pyogenic granuloma.

This paper has described an unusual case of osteoclastoma of the petrous temporal bone which presented with multiple cranial neuropathies. A radical course of radiotherapy reversed the neuropathies but it is too early to comment on possible cure.

Reports of polymyalgia rheumatica and temporal arteritis in blacks are rare. We analyzed five cases of polymyalgia rheumatica and one case of temporal arteritis appearing in blacks. Epidermal cysts are common skin lesions. They are usually seen in the chronic sun-exposed areas of the skin. They can get complicated by inflammation, infection, rupture, or malignancy. Epidermal cyst in the perineal region is rare. We report a case of an infected ruptured perineal epidermal cyst mimicking gluteal abscess. The patient was successfully treated by complete surgical excision. Histology revealed epidermal cyst with foreign body giant cell reaction and no evidence of malignancy.

States of chronic parathyroid hypersecretion, related to a primitive parathyroid abnormality (adenoma, hyperplasia), or to a cause of chronic calcipenia (renal failure, vitamin D deficiency…) have a major impact on bone remodeling, alveolodental structures. Thinning of the lamina dura, maxillary or mandibular brown tumors, giant cell epulis are the most emblematic signs of the primary hyperparathyroidism. Other expressions are related to genetic factors such as fibrous tumors of the jaw in conjunction with mutations in the gene coding for parafibromin.
