Rheumatoid valve lesions have been described for a long time in the literature. The authors report a case of rheumatoid aortic incompetence presenting with complete heat failure and treated semi-urgently by mechanical valve replacement. The pathological lesions observed on the aortic valves were pathognomonic of rheumatoid arthritis. Transthoracic echocardiography should be systematically proposed in the context of severe rheumatoid arthritis looking for valvular heart disease.

The purpose of this study was to characterize the onset and progression of mineralization in porcine bioprosthetic valves implanted in sheep and to test the hypothesis that such valves simulate calcification that is observed clinically and in other experimental models. Hancock I porcine aortic bioprosthetic valves (Medtronic Heart Valve Division, Irvine, Calif.) were implanted as orthotopic mitral valve replacements in juvenile sheep, retrieved after 1 to 124 days, and analyzed as follows: gross inspection, radiography, light, transmission, and surface scanning electron microscopy, and calcium

A 51-year-old woman, who had undergone aortic valve replacement (SJM 19 mm) and open mitral commissurotomy for aortic valve stenosis with regurgitation, and mitral stenosis, was admitted for redo surgery. We found discrete subaortic ring just below the prosthetic valve. The fibrous ring was resected and the aortic valve was replaced (SJM 21 mm) after augmentation of the aortic annulus by Nicks procedure. Mitral and tricuspid valves were replaced. Discrete subaortic stenosis after aortic valve replacement are mostly in cases with small prosthetic valve. The turbulence caused by the small valve

A case of endocarditis caused by Coxiella burnetii in a patient with an aortic Starr-Edwards prosthesis is described. The diagnosis was made by detecting high titres of antibody against Coxiella burnetii phase I antigens. After 15 weeks of chemotherapy with tetracycline and cotrimoxazole, aortic prosthetic valve replacement was carried out because of haemodynamic deterioration and he died 4 days later. The histologic picture of the excised valve was consistent with endocarditis. Coxiella burnetii should be considered in the differential diagnosis of culture-negative endocarditis in our country

Ebstein anomaly of the tricuspid valve is a well known malformation of the heart, but this occurrence in the mitral valve is a very rare congenital heart disease. In this case report a two years and nine months old girl with symptoms of heart failure during the first year of life due to severe mitral regurgitation was studied. The echocardiographic and angiographic features were typical of the Ebstein malformation of the mitral valve. The surgical treatment was made with implantation of pericardial bovine bioprothesis in the mitral position, without ressection of the natural valve tissue. The

A case of proctitis and fatal septicemia caused by Plesiomonas shigelloides in a 42-year-old bisexual male is reported. The medical history of the patient was significant for an aortic valve replacement 3 years before but was otherwise unremarkable. A serum specimen obtained at autopsy was negative for antibody to human immunodeficiency virus by Western blot (immunoblot) analysis. P. shigelloides isolated from blood was susceptible to all antibiotics tested, agglutinated in Shigella group D antiserum, possessed a greater than 100-megadalton plasmid, and was noninvasive in a HeLa cell invasion

Idiopathic hypereosinophilic syndrome is a rare systemic disease that can cause one form of endocardial fibrosis. Endocardial fibrous tissue with overlying thrombus formation in idiopathic hypereosinophilic syndrome leads to a restrictive cardiomyopathy that may produce mitral and/or tricuspid regurgitation. This report describes a patient with idiopathic hypereosinophilic syndrome who underwent mitral valve replacement and successful steroid therapy for an usually localized form of endocardial fibrosis.

Prosthetic valve replacement of the left-sided atrioventricular (AV) valve was performed in a patient with AV discordance and Ebstein's anomaly of the left-sided AV valve without other associated anomalies. An anatomical study of 22 specimens with AV discordance did not show a case with similar anatomy; 5 hearts had mild degrees of downward displacement of the attachment of the left-sided AV valve leaflets, with marked dysplastic changes and variable degrees of hypoplasia. The surgical implications are different according to the degree of downward displacement of the leaflets. This feature and

Incompetence of the mitral valve may occur within days or weeks following myocardial infarction. The etiology most often encountered is rupture of a papillary muscle or one or more chordae tendineae. Since echocardiography is available, dysfunction of the papillary muscle with resulting incompetence of the valve is diagnosed more frequently. The mortality of the disease without surgical correction is reported as high as 95% within the first 3 months following the development of a murmur. The results of the surgical therapy are closely related to the time interval between myocardial infarction

In this review, specific aspects and implications of echocardiography in patients who had undergone recent cardiac surgery will be analysed. This imaging method, which is fully effective in clinical practice, actually possesses diagnostic characteristics, which during assessments of patients, are found to be noninvasive and easily repeatable. They are of great value amongst this particular group of patients for discovering any possible complications from the surgical procedures. Technical problems and methodology will be described regarding the specificity of the patient during the early days

Infective endocarditis (IE) is associated with severe complications and a high mortality rate. Identification of the causative pathogen is crucial to optimise treatment. We present a case of prosthetic valve endocarditis caused by Corynebacterium freneyi, a very rare cause of human infection and not previously reported as a cause of IE. Despite proper antibiotic therapy, the patient eventually needed surgery after progression of the infection. After surgery, he quickly recovered without evidence of relapse during an 8-month follow-up period. This report highlights critical decision making in a

Mechanical heart valve prostheses are based on older designs without changes during the last 40 years. Today, there is an unmet need for less thrombogenic mechanical prostheses. Analysis of the relationship between flow characteristics and thromboembolic complications is possible using numerical and biomolecular flow studies that have shown that the reverse rather than the forward flow is responsible for local platelet activation and thrombosis. After peak flow, leaflets experience flow deceleration and the leaflets are still widely open when the flow becomes zero. The closure of the valve sta

Polymeric heart valves (PHV) can be engineered to serve as alternatives for existing prosthetic valves due to higher durability and hemodynamics similar to bioprosthetic valves. The purpose of this study is to evaluate the effect of geometry on PHVs coaptation and hemodynamic performance. The two geometric factors considered are stent profile and leaflet arch length, which were varied across six valve configurations. Three models were created with height to diameter ratio of 0.6, 0.7, and 0.88. The other three models were designed by altering arch height to stent diameter ratio, to be 0, 0.081

It has long been known that mechanical heart valves, when tested for durability using non-physiologic conditions common in accelerated testers, would cavitate. Until recently, cavitation was never observed in vivo. The discovery that a small number of Edwards-Duromedics heart valve explants indicated signs of cavitation erosion prompted a reassessment of the cavitation erosion potential of pyrolytic carbon (PyC). Analyses of the explanted valves indicated that cavitation may be accentuated by porous regions in the pyrolytic carbon coating from which most mechanical heart valves are constructed
