Atypical Presentation of Ludwig Angina: Diagnostic and Therapeutic Complications in a Rare Case.

Finding the clinical significance of facial swelling was of utmost importance in this case. Ludwig angina is a life-threatening cellulitis of the floor of the mouth that is mainly due to infectious causes. This particular patient had an atypical presentation except for the facial swelling in which the patient's non-specific symptoms made it difficult to obtain a diagnosis. Severe edema can make it difficult to intubate the patient; however, our case did not present with any common symptoms such as this. This is why it is of utmost importance to maintain dental/mouth hygiene and maintenance bec…

[Secondary prevention of cardiovascular diseases : targeting all the cardiovascular risk factors].

This article focuses on the secondary prevention of cardiovascular (CV) diseases, namely the prevention of recurrence in subjects with a personal history of CV event, and the prevention of a first event in patients identified as at very high risk. For all these patients at very high risk, treatment is primarily based on the application of hygienic and dietary measures, including increasing the volume of physical activity, modifying the diet, and obtaining, if necessary, weight loss, as well as stopping smoking. This strategy has proven its benefits in terms of reducing morbidity and mortality.

Familial hypertriglyceridemia/polygenic hypertrigliceridemia.

For decades, familial hypertriglyceridemia (HTG) has been considered a specific entity characterized by an increase in VLDL particles and an autosomal dominant inheritance pattern. In the genomics era, it has been proven that familial HTG, although it could be grouped in families, had a polygenic inheritance in which the phenotype would be determined by concomitant environmental factors. Hence its inclusion in the group of polygenic HTGs. Clinically, they are characterized by moderate HTG, with the consequent increase in cardiovascular risk, and in rare cases, by severe HTG with risk of acute

Management of Shallow Vestibule with Reduced Attached Gingiva in Fixed Prosthetic Intervention.

Shallow vestibule has long been considered a deterring factor in the use of removable dental prosthetics. The need for management in fixed prosthetic replacement is not widely discussed. Adequate attached gingiva is essential for continued proper oral hygiene. Muscular and fibrous traction leads to gingival recession, which can cause marginal leakage in a fixed prosthetic restoration. In the long-term, this causes the suprastructure to fail and, ultimately, the restoration also fails. Therefore, shallow vestibule with reduced attached gingiva should be identified in the diagnostic stage and sh…

Fibrous Ankylosis of the Temporomandibular Joint in a Young Child.

Temporomandibular joint (TMJ) ankylosis is an intracapsular union of the disccondyle complex to the temporal articular surface that restricts mandibular movements, including fibrous adhesions or bony fusion between the condyle, disc, glenoid fossa, and articular eminence. The leading causes include trauma and infection. It can be a serious and disabling condition that leads to difficulties in mastication, swallowing, speaking, esthetics and oral hygiene. Disturbances of facial and mandibular growth and acute compromise of the airway invariably result in physical and psychological disability. T…

Mandibular exostosis in canine with single tooth recession - a rare case report.

Buccal exostoses occur along the buccal aspect of the maxilla or mandible, usually in the premolar and molar areas. It has been suggested that the bony outgrowth represents a reaction to increased or abnormal occlusal stress to the teeth in the involved areas. Gingival recessions may occur without any symptoms but may give rise to the patient concern about poor esthetics, dentine hypersensitivity, inability to perform oral hygiene procedures, and loss of the tooth. This article presents a rare case of exostosis in the mandibular right canine region and single tooth recession in the mandibular …

[Stump problems after a lower limb amputation].

Problems in the amputation stump occur frequently in lower limb amputees. These problems can range from skin defects to serious vascular insufficiency and have a major impact on the amputee's quality of life. We illustrate this with 2 patients aged 20 and 65. The first patient developed a Candida skin infection of the stump skin as a result of silicone liner use. She was successfully treated with miconazole/hydrocortisone and hygiene measures. The second patient developed progressive lower limb ischemia that resulted in a stump wound and claudication. He was treated with vascular surgery. Pati…

Intrusion and uprighting teeth for fixed prosthesis with temporary anchorage device.

The mandibular first permanent molar is usually first to erupt around the age of six. Therefore, if the oral hygiene is not impeccable, it is usually also the first permanent tooth decayed in a child's mouth. A plethora of circumstances may lead to pulpal involvement, periapical pathosis, and endodontic treatment and in unfortunate situations, removal of this tooth. If this tooth is not replaced, or the extraction space is not retained for a considerable amount of time, the posterior occlusion will collapse. The most common occurrence is tipping of teeth in juxtaposition to the extraction site…

Cysticercosis in the pig.

Taenia solium cysticercosis is still an important parasitosis in rural pigs in many developing countries, México among them. The main causes for the persistence of this condition are lack of hygiene in the rural communities, lack of education of the animal owners, lack of control in the trade of pigs and their meat and lack of conscientious meat inspection. The pig production systems in the marginated areas of Mexico are briefly mentioned and it is stressed that among the important reasons for the persistence of the reproductive cycle of Taenia solium is the fact that appropriate toilet facili…

Prophylactic immunisation against traveller's diarrhoea caused by enterotoxin-forming strains of Escherichia coli and against cholera: does it make sense and for whom?

Traveller's diarrhoea (TD) constitutes the most common disease relevant to travel medicine with ETEC as the leading causative pathogen. Cholera is the most serious, but very rare form of TD. ETEC and cholera share pathogenic mechanisms by producing a toxin that has an 80% amino acid homology. A consensus of German-speaking experts sees the indication to use the whole cell/B subunit oral cholera vaccine (WC--BS) if cholera is a risk for aid workers or travellers with an anticipated threat of cholera who stay under poor hygienic conditions. The use of the vaccine should be considered in the indi

Report of an infant with noma (cancrum oris).

Noma (cancrum oris) is an infectious disease that destroys the oro-facial tissues and other neighboring structures in its fulminating course. The starting point of the disease is acute ulcero-necrotic gingivitis, which results in an extensive gangrenous plaque destroying all of the soft tissues of the face. It predominantly affects children aged 2-16 years and is primarily seen in areas where the socioeconomic standards are low and there is poor hygiene, as in developing countries. We discuss possible predisposing factors in cancrum oris such as malnutrition, infectious diseases, HIV infection…

[Epidemiology of Helicobacter pylori infection in Japan].

The prevalence of H. pylori infection in Japan is extremely low at an early age, as in other developed countries, and subsequently shows a rapid increase until it reaches a plateau of about 70% at 50 years of age. This phenomenon is thought to occur because people about 50 years old were born soon after World War II and spent their childhood in conditions of poor hygiene. It is suggested that H. pylori infection leads to histological chronic gastritis regardless of the strain of this organism, and after that the course of the disease does depend on the environmental factors such as diet, durat…
