Genetic, Immunological, and Public Health Perspectives on Podoconiosis.

Podoconiosis is a neglected tropical disease (NTD) that primarily affects poor communities in tropical regions, particularly in sub-Saharan African countries. The disease results from prolonged contact with red clay soils, leading to chronic lymphedema of the lower limbs. However, the pathogenesis is not yet fully clarified, which continues to hinder the development of targeted interventions and definitive diagnostic tools. This review synthesizes recent advancements in understanding the genetic, immunological, and tissue-level factors driving podoconiosis to help bridge this knowledge gap. It

Renal Leukocyte Chemotactic Factor 2 (ALECT2) Amyloidosis With Concurrent IgA Nephropathy: A Case Report and Literature Review.

Leukocyte chemotactic factor 2 (ALECT2) amyloidosis is a rare form of renal amyloidosis, typically presenting with chronic kidney disease and variable degrees of proteinuria. Cases of concurrent ALECT2 amyloidosis and IgA nephropathy are exceedingly rare, with limited descriptions of their clinical and pathological features in the literature. Here, we report a 61-year-old Chinese woman who presented with symmetrical lower limb edema and microscopic hematuria, without nephrotic syndrome or significant proteinuria. Renal biopsy revealed diffuse interstitial amyloid deposition and coexisting IgA

Advances in Targeted Therapy for Systemic Lupus Erythematosus: Current Treatments and Novel Approaches.

Systemic lupus erythematosus (SLE) is a chronic autoimmune disease with diverse clinical manifestations that can lead to severe organ damage. The complex pathophysiology of SLE makes treatment selection difficult. This review examines the current evidence for biological therapies in SLE, including the anti-B cell activating factor antibody belimumab; the type I interferon receptor antagonist anifrolumab; the novel calcineurin inhibitor voclosporin; and rituximab, which targets CD20 on B cells. We also describe emerging therapies, including novel agents in development and CD19-directed chimeric

[RECTUS ABDOMINIS MASS AS THE FIRST PRESENTATION OF LUNG ADENOCARCINOMA].

Lung cancer has been a global epidemic since 1950. It was estimated that during 2018, worldwide, there were about 2.07 million new cases and approximately 1.76 million cases of mortality. The mortality from lung cancer is higher than breast, prostate, colon cancer and leukemia combined. Cigarette smoking is the most preventable risk factor of lung cancer. Treatment modalities include surgery, chemotherapy, radiotherapy and immunological treatment. The tumor's genetics plays a major role in treatment. Desmoid is the most frequent abdominal wall tumor. Metastasis from distant origins to the abdo

Host-intrinsic and host-extrinsic factors modulate immunity to Mtb infection, reinfection, and noncanonical vaccination routes.

Tuberculosis (TB) disease states and outcomes are highly heterogeneous. While this makes TB difficult to diagnose, monitor, and treat, it also presents opportunities to identify correlates of protection or disease severity that can be used as biomarkers and help inform future interventions. Immunological priming due to primary Mycobacterium tuberculosis (Mtb) infection can protect against subsequent reinfection; thus, comparing primary infection with reinfection can provide insights into features associated with host control. Here, we examine paradigms of natural and vaccine-induced immunity a

The role of aryl hydrocarbon receptor in the occurrence and development of periodontitis.

Periodontitis is a condition characterized by dysbiosis of microbiota and compromised host immunological responses, resulting in the degradation of periodontal tissues. The aryl hydrocarbon receptor (AHR), a ligand-activated transcription factor, plays a crucial role in the pathogenesis of periodontitis. AHR serves as a pivotal mediator for the adverse impacts of exogenous pollutants on oral health. Research indicates elevated expression of AHR in individuals with periodontitis compared to those without the condition. However, subsequent to the identification of endogenous AHR ligands, researc

Increasing Trends in the Pancreatitis Risk With Tumor Necrosis Factor Inhibitor Use.

Tumor necrosis factor (TNF) inhibitors are a class of pharmacologic agents used to treat a wide range of immunologic diseases. We present a systematic study of the pancreatitis risk with TNF inhibitor use through a retrospective case-control design disproportionality analysis of the U.S. Food and Drug Administration Adverse Event Reporting System (FAERS) database, with data ranging from the fourth quarter of 2012 (2012 Q4) to the second quarter of 2024 (2024 Q2). Our analysis reveals an increasing trend in the pancreatitis risk with TNF inhibitors when stratified by year, with all of the TNF i

Targeting immunogenic cell stress and death for cancer therapy.

Immunogenic cell death (ICD), which results from insufficient cellular adaptation to specific stressors, occupies a central position in the development of novel anticancer treatments. Several therapeutic strategies to elicit ICD - either as standalone approaches or as means to convert immunologically cold tumours that are insensitive to immunotherapy into hot and immunotherapy-sensitive lesions - are being actively pursued. However, the development of ICD-inducing treatments is hindered by various obstacles. Some of these relate to the intrinsic complexity of cancer cell biology, whereas other

Immunobiology of primary sclerosing cholangitis.

Primary sclerosing cholangitis (PSC) is a chronic inflammatory progressive cholestatic liver disease. Genetic risk factors, the presence of autoantibodies, the strong clinical link with inflammatory bowel disease, and associations with other autoimmune disorders all suggest a pivotal role for the immune system in PSC pathogenesis. In this review, we provide a comprehensive overview of recent immunobiology insights in PSC. A particular emphasis is given to immunological concepts such as tissue residency and knowledge gained from novel technologies, including single-cell RNA sequencing and spati

Deciphering Childhood Rosacea: A Comprehensive Review.

Childhood rosacea is a lesser known, yet significant, skin condition presenting diagnostic and treatment challenges. Although often underdiagnosed due to unclear diagnostic criteria, it manifests similarly to adult rosacea, with features such as papulopustular, telangiectasia, granulomatous, idiopathic facial aseptic granuloma, and ocular rosacea. The complex pathophysiology involves genetic, immunological, and environmental factors. Distinguishing childhood rosacea from conditions like acne, steroid rosacea, sarcoidosis, and lupus vulgaris is crucial but complicated by the lack of established

Immune imprinting: The persisting influence of the first antigenic encounter with rapidly evolving viruses.

Immune imprinting is a phenomenon that stems from the fundamentals of immunological memory. Upon recurrent exposures to an evolving pathogen, the immune system must weigh the benefits of rapidly recalling established antibody repertoires with greater affinity to the initial variant or invest additional time and energy in producing de novo responses specific to the emerging variant. In this review, we delve into the mechanistic complexities of immune imprinting and its role in shaping subsequent immune responses, both de novo and recall, against rapidly evolving respiratory viruses such as infl

Organizing pneumonia associated with Richter transformation in chronic lymphocytic leukemia: a case report and review of the literature.

Organizing pneumonia (OP) is defined histologically by the presence of granulation tissue within alveolar ducts and alveoli. Recently, several lymphoid neoplasms have been implicated as a risk factor for OP, however, OP as a primary manifestation of malignancy transformation has not been widely reported in the literature. Here, we report a case of a patient with a history of chronic lymphocytic leukemia (CLL) who presented with weight loss, low-grade fever, lymphadenopathy, and bilateral pulmonary infiltrates revealed in imaging studies. Video-assisted thoracoscopic lung biopsy showed CLL cell

Exploring Therapeutic Advances: A Comprehensive Review of Intestinal Microbiota Modulators.

The gut microbiota establishes a mutually beneficial relationship with the host starting from birth, impacting diverse metabolic and immunological processes. Dysbiosis, characterized by an imbalance of microorganisms, is linked to numerous medical conditions, including gastrointestinal disorders, cardiovascular diseases, and autoimmune disorders. This imbalance promotes the proliferation of toxin-producing bacteria, disrupts the host's equilibrium, and initiates inflammation. Genetic factors, dietary choices, and drug use can modify the gut microbiota. However, there is optimism. Several thera

Next-Generation Therapies for Multiple Myeloma.

Recent therapeutic advances have significantly improved the outcome for patients with multiple myeloma (MM). The backbone of successful standard therapy is the combination of Ikaros degraders, glucocorticoids, and proteasome inhibitors that interfere with the integrity of myeloma-specific superenhancers by directly or indirectly targeting enhancer-bound transcription factors and coactivators that control expression of MM dependency genes. T cell engagers and chimeric antigen receptor T cells redirect patients' own T cells onto defined tumor antigens to kill MM cells. They have induced complete

ASFV subunit vaccines: Strategies and prospects for future development.

African Swine Fever (ASF) is an acute, highly contagious, and lethal disease caused by the African Swine Fever Virus (ASFV), posing a severe threat to the global pig farming industry. Although live vaccines are currently available, preventing and controlling ASF remains a considerable challenge. Several factors have impeded vaccine development, including the complexity of ASFV particles and the suppressive effects of its gene-encoded proteins on the host's immune system. This article delves into the immunological responses elicited by ASFV, encompassing both innate and adaptive immunity, and e

Understanding the spectrum from preclinical psoriatic arthritis to early diagnosis of the disease.

Psoriasis is a chronic inflammatory skin disease that often precedes the development of psoriatic arthritis. Advances over the past 10 years have deepened our understanding of the transition from skin inflammation to joint inflammation, revealing various phases during which genetic, environmental, and immunological factors can affect this transition. In 2023, a European Alliance of Associations for Rheumatology task force outlined key considerations for identifying individuals with subclinical disease from those with clinically stabilised disease. Discerning between subclinical psoriatic arthr

Severe Liver Damage in an Obese Patient: Onset of Celiac Disease or Overlap Syndrome?

Celiac disease (CeD) is an enteropathy caused by the complex interaction between genetic, environmental, and individual immunological factors. Besides the hallmark of intestinal mucosal damage, CeD is a systemic disorder extending beyond the gastrointestinal tract and impacting various other organs, causing extraintestinal and atypical symptoms. The association between CeD and liver damage has been classified into three main categories: mild and asymptomatic liver injury, autoimmune liver injury, and liver failure. We present a case of severe liver damage with cirrhotic evolution in an obese 1

Rilzabrutinib-induced transition from pemphigus vulgaris to pemphigus foliaceous: a case report and literature review.

The discovery of the role of Bruton's Tyrosine Kinase (BTK) in inflammation and autoimmunity has recently led to the development of BTK inhibitors for the treatment of autoimmune diseases, including pemphigus vulgaris. We herein present the case of a patient affected by pemphigus vulgaris, refractory to conventional immunosuppressive therapies and to multiple courses of rituximab, who was treated with rilzabrutinib and achieved disease control, but whose immunological profile switched from pemphigus vulgaris to pemphigus foliaceus after drug discontinuation. Furthermore, we reviewed the litera
