Spatiotemporal epidemiological characteristics of human clonorchiasis in China.

This study explores the spatiotemporal distribution of Clonorchis sinensis infection rates across China using multiple spatial analysis methods, including spatial autocorrelation, hot spot analysis, kernel density estimation, and standard deviation ellipse analysis. By systematically reviewing historical literature and incorporating data from three national parasitic disease surveys, we identified significant clustering patterns and high-risk regions associated with the infection. The results show that infection rates were predominantly concentrated along major river systems, including the Don

First bovine vaccine to prevent human schistosomiasis - a cluster randomised Phase 3 clinical trial.

OBJECTIVE: Schistosomiasis is a neglected tropical parasitic disease caused by blood flukes of the genus Schistosoma. Schistosoma japonicum is zoonotic in China, the Philippines, and Indonesia, with bovines acting as major reservoirs of human infection. The primary objective of the trial was to examine the impact of a combination of human mass chemotherapy, snail control through mollusciciding, and SjCTPI bovine vaccination on the rate of human infection. METHODS: A 5-year phase IIIa cluster randomized control trial was conducted among 18 schistosomiasis-endemic villages comprising 18,221 res

Detection of disease-associated microglia among various microglia phenotypes induced by West Nile virus infection in mice.

West Nile virus (WNV) has emerged as a significant cause of viral encephalitis in humans and horses. However, the pathogenesis of the West Nile encephalitis remains unclear. Microglia are activated by WNV infection, and the pathogenic involvement of their phenotypes is controversial. In this study, we examined the diversity of microglia phenotypes caused by WNV infection by assessing various microglia markers and identified disease-associated microglia in WNV-infected mouse brain tissue. Cells positive for general microglia markers such as Iba1, P2RY12, or TMEM119 were detected in the control

Animal Models of Cryptococcus neoformans in Identifying Immune Parameters Associated With Primary Infection and Reactivation of Latent Infection.

Cryptococcus species are environmental fungal pathogens and the causative agents of cryptococcosis. Infection occurs upon inhalation of infectious particles, which proliferate in the lung causing a primary infection. From this primary lung infection, fungal cells can eventually disseminate to other organs, particularly the brain, causing lethal meningoencephalitis. However, in most cases, the primary infection resolves with the formation of a lung granuloma. Upon severe immunodeficiency, dormant cryptococcal cells will start proliferating in the lung granuloma and eventually will disseminate t

Outbreaks of influenza A virus infection in neonatal intensive care units.

BACKGROUND: Outbreaks of nosocomial influenza virus infections have been described rarely during childhood and even less so in the neonatal period. METHODS: We report 30 neonates admitted to 2 neonatal intensive care units with nosocomial influenza A virus infection, which occurred in 2 outbreaks during 1999. Risk factors for infection were evaluated, and control measures were adopted. Virus was detected by indirect immunofluorescence antibody screen. Any infant with nasopharyngeal aspirate positive for influenza A virus was considered infected. RESULTS: Of 95 infants screened 30 were positi

Rapid control of an outbreak of carbapenemase-producing enterobacterales in a recently built, single-bedded, specialist cardiothoracic hospital in England.

BACKGROUND: Increasing global prevalence of antibiotic-resistant carbapenemase-producing Enterobacterales (CPE) is a major concern. An outbreak occurred in a recently built, single-bedded cardiothoracic hospital, where infection control is crucial to protect vulnerable patients. AIM: The aim of this study was to describe transmission dynamics of CPE in a modern hospital and subsequent control measures used to contain the outbreak. METHODS: A case was defined as a polymerase chain reaction-positive CPE (KPC-2) inpatient from June 2024 onwards. Admission and weekly rectal screening were conduc…

Surgery in the shadow of Nipah: Preparedness, precautions, and protocols.

Nipah virus (NiV) is a highly lethal zoonotic infection with a case fatality rate of 70-75% and is recognized by the World Health Organization as a priority pathogen with epidemic potential. Recent cases in India highlight the ongoing risk of re-emergence, particularly in resource-constrained health systems. While surveillance and outbreak containment are often emphasized, the implications of NiV for surgical and perioperative care remain less often discussed. Operating rooms are high-risk environments for nosocomial transmission owing to aerosol-generating procedures and exposure to body flui

Implementing a Sustainable, Phased Approach to Reducing CLABSI Rates in a Level IV NICU.

BACKGROUND: Central line-associated bloodstream infections (CLABSIs) are preventable hospital acquired infections that lead to an increased mortality rate and extended hospital length of stay. Reducing CLABSIs in neonates in a Level IV neonatal intensive care unit (NICU) is challenging due to the complexities of patient diagnoses requiring extended line days and variability in nursing line care. PURPOSE: The purpose of this quality improvement project is to decrease the rate of CLABSIs from 2 infections per 1000-line days (baseline rate from 2019 to 2022) to less than 1 infection per 1000-lin

Silica nanoparticles with encapsulated DNA (SPED) to trace the spread of pathogens in healthcare.

BACKGROUND: To establish effective infection control protocols, understanding pathogen transmission pathways is essential. Non-infectious surrogate tracers may safely explore these pathways and challenge pre-existing assumptions. We used silica nanoparticles with encapsulated DNA (SPED) for the first time in a real-life hospital setting to investigate potential transmission routes of vancomycin-resistant enterococci in the context of a prolonged outbreak. METHODS: The two study experiments took place in the 900-bed University Hospital Zurich, Switzerland. A three-run 'Patient experiment' inve…

Use of T Cell Mediated Immune Functional Assays for Adjustment of Immunosuppressive or Anti-infective Agents in Solid Organ Transplant Recipients: A Systematic Review.

Background: Defining the optimal dosage of the immunosuppressive or duration of anti-infective agents is a challenge in solid organ transplant (SOT) recipients. We aimed to systematically review the literature regarding the use of T cell mediated immune functional assays (IFAs) for adjustment of the immunosuppressive or anti-infective agents in SOT recipients. Methods: We systematically searched PubMed, Scopus, EMBASE, Web of Science (WOS), Cochrane Central Register of Controlled Trials (CENTRAL), and ClinicalTrials.gov to find human interventional studies or study protocols that used either i

Infection Prevention in Shoulder Surgery.

The microbiome of the shoulder demonstrates distinctive differences to other orthopaedic surgical sites. Recent studies have demonstrated that the most common organisms found in deep shoulder infections are coagulase-negative staph lococcal species and Propionibacterium acnes . Many studies support diligent hand washing, decreasing operative time, routine glove changing, minimizing operating room traffic, and covering instruments as means for decreasing the risk of deep infection. On the other hand, hair clipping and the use of adhesive drapes may have little effect on decreasing the incidence

Antibiotic prophylaxis versus no prophylaxis for preventing infection after cesarean section.

BACKGROUND: The single most important risk factor for postpartum maternal infection is cesarean section. Although guidelines endorse the use of prophylactic antibiotics for women undergoing cesarean section, there is not uniform implementation of this recommendation. This is an update of a Cochrane review first published in 1995 and last updated in 2010. OBJECTIVES: To assess the effects of prophylactic antibiotics compared with no prophylactic antibiotics on infectious complications in women undergoing cesarean section. SEARCH METHODS: We searched the Cochrane Pregnancy and Childbirth Group

[A nosocomial outbreak of epidemic keratoconjunctivitis].

We describe a nosocomial outbreak of keratoconjunctivitis at the Ophthalmology Clinic in Hospital Carlos Van Buren, that affected 150 patients, during August and September of 2006. This outbreak was concomitant with a community outbreak produced by the same agent in the city of Valparaiso that affected 681 other patients. Although adenovirus was not isolated in the nosocomial cases, positive cultures were obtained from community cases, so the diagnosis was based on clinical and epidemiological criteria. The infection rate within the Clinic patients was 5.2% considering the population that atte

The possibility of human adenovirus detection from the conjunctiva in asymptomatic cases during nosocomial infection.

PURPOSE: To prevent outbreaks of nosocomial adenoviral conjunctivitis, proper management for transmission control must be performed. We collected conjunctival samples from asymptomatic inpatients and an ophthalmologist in an ophthalmology ward and attempted to detect the human adenovirus (HAdV) pathogen for infection control. METHODS: One inpatient was diagnosed with adenoviral conjunctivitis on the basis of typical, acute, and severe symptoms and virologic testing by using an immunochromatography (IC) kit. To survey nosocomial infection, conjunctival swabs from 17 other inpatients and 1 opht…
