Anastomosis versus rectal stump procedure in cytoreductive surgery with hyperthermic intraperitoneal chemotherapy for colorectal and appendiceal peritoneal metastases: A comparative study.

AIM: Cytoreductive surgery (CRS) with hyperthermic intraperitoneal chemotherapy (HIPEC) is the standard treatment for peritoneal metastases (PM) of colorectal or appendiceal origin. Rectal anastomotic leakage (RAL) or rectal stump blow-out is a serious complication following rectal resection after CRS-HIPEC. This study aimed to compare outcomes between ileo-/colorectal anastomosis and rectal stump procedure and to identify risk factors for RAL and blow-out. METHOD: Data were retrospectively collected between 2012 and 2024 from a prospectively maintained HIPEC Registry and supplemented with th

Faecal calprotectin as a non-invasive marker of Crohn's disease in anal fistulas.

AIM: Faecal calprotectin (FC) is a noninvasive marker that reflects intestinal inflammation with good sensitivity. A prior study indicated that FC values above 150 μg/g could distinguish between anal fistulas (AF) of cryptoglandular or Crohn's disease (CD) origin. It is hypothesized as a useful triage test to rule out CD in newly referred AF patients, thus reducing the number of ileocolonoscopies performed and optimizing treatment regimens in AF while minimizing patient discomfort as well as healthcare costs. The aim of the study was to determine the accuracy of FC in distinguishing between an

The effect of neuromuscular electrical stimulation on bowel management in people with chronic spinal cord injury-An IDEAL 2a pilot study.

INTRODUCTION: Spinal cord injury (SCI) disrupts autonomic function, often impairing bowel control. About 14% of individuals require over an hour for defaecation, limiting social participation. This trial evaluated the effect of 16 weeks of neuromuscular electrical stimulation (NMES) on defaecation time, bowel transit time, bowel and bladder function and satisfaction in individuals with chronic SCI. METHODS: Participants with SCI more than 1 year post injury (AIS A-D, C5-L3) applied home-based NMES to the abdominal wall for 30 min before scheduled defaecation. Stimulation intensity was set to

Transanal irrigation: Bridging the gap in treatment for chronic constipation and/or faecal incontinence-A systematic review and management guidance.

AIM: The aims of the study were to: (1) update a systematic review on the efficacy of transanal irrigation (TAI) in patients with chronic constipation (CC) and/or faecal incontinence (FI); (2) provide guidance on the position of TAI in the treatment pathway of CC and FI; and (3) discuss knowledge gaps and areas of future research. METHOD: A collaborative of 11 experts (gastroenterologists, colorectal surgeons, and clinical scientists) from eight European countries was established. The expert group was divided into three main groups, with each group leading a specific section (systematic revie

Long-term perineal wound healing and re-operation rates after proctectomy in patients with inflammatory bowel disease-A retrospective multicentre study.

BACKGROUND AND AIMS: Perineal wound complications occur frequently after proctectomy in patients with inflammatory bowel disease (IBD), with persistent perineal wounds appearing in some patients. This study examines the surgical outcomes after proctectomy in patients with IBD in a contemporary setting, with primary perineal wound healing as the primary end-point. METHODS: This is a multicentre retrospective cohort study with five Swedish hospitals participating. Patients with IBD who had undergone proctectomy during the period of 2000-2019 were included. Two exposures of interest were analyse

Clinical Frailty Scale predicts overall survival after colon cancer surgery in people aged 80 years and older: A prospective multicentre observational study.

AIM: An increasing number of elective colon cancer surgeries are performed on older patients. Frail patients are at a higher risk of postoperative adverse events and early mortality. This prospective, multicentre, observational study aimed to analyse preoperative screening tools and the long-term survival of older patients having elective colon cancer surgery. The focus was on patients who survived more than 3 months postoperatively, excluding the effect of early postoperative mortality. METHODS: Patients aged ≥80 years with electively operated stage I-III colon cancer were recruited. Prospec

Comparing robotic with laparoscopic beyond total mesorectal excision for advanced rectal cancer-a propensity-matched analysis.

AIM: Robotic surgery is increasingly being used for rectal resection, with short-term benefits such as reduced hospital stay, faster bowel recovery and fewer complications. However, its utility for advanced rectal cancers requiring beyond total mesorectal excision has not been adequately evaluated. The aim of this study was to compare robotic and laparoscopic approaches for extended rectal resection, with postoperative and short-term oncological outcomes as endpoints. METHOD: A retrospective, single-centre study of patients with advanced rectal cancer requiring extended rectal resection betwe

Indocyanine green fluorescence angiography could reduce the risk of anastomotic leakage in rectal cancer surgery: a systematic review and meta-analysis of randomized controlled trials.

AIM: Several papers have shown that use of indocyanine green (ICG) decreases incidence of anastomotic leakage (AL) during colonic surgery, but no clear evidence has been found for rectal cancer surgery. Therefore, with this systematic review and meta-analysis of randomized controlled trials (RCTs) we aimed to assess if ICG could also reduce risk of AL in rectal cancer surgery. METHOD: PubMed, Scopus, CINAHL and Cochrane databases were searched for RCTs assessing the effect of intraoperative ICG on the incidence of AL of the colorectal anastomosis. Pooled relative risk (RR) and pooled risk dif

Does complete pathological response increase perioperative morbidity risk in rectal cancer?

AIM: The optimal management of patients with clinical complete response after neoadjuvant treatment for rectal cancer is controversial. The aim of this study is to compare the morbidity between patients with locally advanced rectal cancer who have had a pathological complete response (pCR) or not after neoadjuvant chemoradiotherapy (NCRT) and total mesorectal excision (TME). The study hypothesis was that pCR may impact the surgical complication rate. METHOD: A retrospective cohort study was conducted of a prospectively maintained database in Australia and New Zealand, the Binational Colorecta

Ultrasound assessment of the distal resection margin during robotic rectal surgery.

AIM: Ensuring an adequate distal resection margin (DRM) is a key factor in achieving the gold standard in surgical treatment for rectal surgery. The aim of this article is to describe our surgical technique and the usefulness of intraoperative ultrasonography (IOUS) for evaluating the DRM during robotic rectal surgery (RRS). METHOD: Prospective data on five consecutive patients with rectal cancer who underwent RRS between January 2023 and December 2023 were collected. IOUS was utilized to evaluate the DRM in all patients. RESULTS: The mean time for the IOUS examination ranged from 5 to 10 mi

Surgeon perceptions of personality as an influencing factor on anastomotic decision-making: A qualitative analysis.

AIM: Surgeon personality is a factor influencing rectal anastomotic decision-making. However, it is unclear how or why this may be the case, or what aspects of personality are involved. The aim of this study was to investigate the views of colorectal surgeons on how their individual personality may influence variation in anastomotic decision-making. METHOD: Purposive sampling was used to invite certified UK-based colorectal surgeons to participate, with individual interviews used for data collection. Participants were recruited until ongoing data review indicated no new codes were generated (
