High tibial osteotomy leads to medial-lateral redistribution of knee internal forces during walking and stair ambulation and improves patient-reported outcomes.

PURPOSE: This study aimed to evaluate changes in medial and lateral knee contact forces during walking, stair ascent and stair descent following open-wedge high tibial osteotomy, and to assess their association with changes in patient-reported symptoms and function. METHODS: Fifty-two patients with medial knee osteoarthritis and varus malalignment were recruited and subdivided into two groups according to a partially randomised patient-preference design. Three patients dropped out at follow-up, resulting in a cohort of 49 patients who underwent either high tibial osteotomy (n = 25) or conserv

Lateral-Conforming Dual-Pivot Versus Nonconforming Anterior-Stabilized Bearings in Valgus Knees Undergoing Primary Total Knee Arthroplasty.

BACKGROUND: Personalized alignment strategies in total knee arthroplasty (TKA) have gained in popularity to recreate native biomechanics and improve outcomes. Prior studies have reported better patient-reported outcome measures (PROMs) for preoperative valgus deformities (compared to varus) when medial-conforming bearings are used. This study evaluated PROMs in a lateral-conforming dual-pivot (DP) bearing compared to a nonconforming anterior-stabilized (AS) bearing after primary TKA. METHODS: A retrospective review was performed on 2,427 primary TKAs implanted with either a DP conforming bear

Alignment and early clinical outcomes following total knee arthroplasty using a handheld navigation system compared to conventional instruments.

INTRODUCTION: Proper lower extremity alignment is important for success and longevity of total knee arthroplasty (TKA). Conventional instrumentation has demonstrated higher rates of mechanical axis (MA) malalignment compared to robotic or computer navigated surgery. The purpose of this study is to compare the alignment and early clinical outcomes between a handheld implant agnostic navigation system and conventional instrumentation. METHODS: 117 patients who underwent single-stage bilateral TKA were prospectively studied. The distal femoral and proximal tibial resections were performed using

Exercise-induced pain changes associate with changes in muscle perfusion in knee osteoarthritis: exploratory outcome analyses of a randomised controlled trial.

BACKGROUND: Exercise therapy is recommended for knee osteoarthritis (OA), but the underlying mechanisms of pain relief are not fully understood. The purpose of this study was to explore the effects of exercise on muscle perfusion assessed by dynamic contrast enhanced MRI (DCE-MRI) and its association with changes in pain in patients with knee OA. METHODS: Exploratory outcome analyses of a randomised controlled study with per-protocol analyses ( ClinicalTrials.gov : NCT01545258) performed at an outpatient clinic at a public hospital in Denmark. We compared 12 weeks of supervised exercise thera

Eight-year trajectories of changes in health-related quality of life in knee osteoarthritis: Data from the Osteoarthritis Initiative (OAI).

BACKGROUND: Knee osteoarthritis (OA) worsens health-related quality of life (HRQoL) but the symptom pathway varies from person to person. We aimed to identify groups of people with knee OA or at its increased risk whose HRQoL changed similarly. Our secondary aim was to evaluate if patient-related characteristics, incidence of knee replacement (KR) and prevalence of pain medication use differed between the identified HRQoL trajectory groups. METHODS: Eight-year follow-up data of 3053 persons with mild knee OA or at increased risk were obtained from the public Osteoarthritis Initiative (OAI) da

Changes in biomechanical risk factors for knee osteoarthritis and their association with 5-year clinically important improvement after limb realignment surgery.

OBJECTIVE: To evaluate 5-year outcomes after lower limb realignment and test the hypothesis that surgery-induced changes in selected biomechanical risk factors for medial knee osteoarthritis (OA) are associated with clinically important improvements. DESIGN: We prospectively evaluated patient-reported outcomes, full-limb standing radiographs and gait biomechanics before, 6 months (surgery-induced change) and 5 years after medial opening wedge high tibial osteotomy (HTO) in 170 patients (46.4 ± 8.9 years, 135 males) with knee OA and varus alignment. Logistic regression tested the associations

[Isolated rupture of the distal tendon of the biceps femoris muscle in a sport climber: an uncommon injury].

An isolated rupture of the distal tendon of the biceps femoris muscle without a direct trauma is an extremely rare injury. To date only a few case reports have been described in the literature. For sport climbers such an injury has never been described before. We report about a 41-year-old male sport climber complaining of such an injury after indoor rock-climbing. Unlike the previously described reports, the rupture occured during a phase of maximum static tension, without acceleration or movement of the knee joint. History and clinical characteristics, sonography and magnetic resonance imagi

Rare cause of knee pain after martial arts demonstration: a case report.

BACKGROUND: Patellar dislocations are a commonly treated injury in the Emergency Department (ED), with a majority of cases involving lateral subluxation of the patella outside of the joint space. Intra-condylar dislocations of the patella are rare. Of the two types of axis rotation, vertical and horizontal, the vertical occurs five times less frequently. These injuries most often undergo open reduction or, at best, closed reduction under general anesthesia. OBJECTIVES: To remind Emergency Physicians to consider this injury in any patient with severe knee pain and limited mobility, even with a

Lower limb injuries to passenger car occupants.

A detailed examination was undertaken of hospitalized car occupants who sustained a lower limb injury in a frontal crash. The assessment included an analysis of the type, severity and causes of these injuries and mechanisms involved in lower limb fractures. The findings showed that fractures and dislocations occurred in 88% of lower limb injury cases, that more than half were from crashes < 48 km hour-1 and that the number of fractures was directly proportional with delta V. Ankle dislocations and foot fractures from the floor and toe pand were the most common injury-source combination overall

Morphological Risk Factors for Posterior Cruciate Ligament Tear and Tibial Avulsion Injuries of the Tibial Plateau and Femoral Condyle.

BACKGROUND: Identification of morphological risk factors associated with the knee that threaten ligaments is important for understanding injury mechanisms and prevention. However, the morphological risk factors for posterior cruciate ligament (PCL) lesions are not clearly understood. PURPOSE: To investigate whether the medial tibial depth (MTD), medial and lateral posterior tibial slope, asymmetry of the medial and lateral slopes, radius of the sagittal plane medial femoral condyle, coronal tibial slope, and notch width index (NWI) were risk factors for PCL intrasubstance tearing (PCLIT) and

The anterior cruciate ligament-deficient knee with varus alignment. An analysis of gait adaptations and dynamic joint loadings.

Thirty-two patients with an ACL-deficient knee and lower limb varus alignment and 16 healthy controls were analyzed during level walking using a force-plate and optoelectronic system. The forces and moments of the lower limb and knee joint were measured and knee joint loads and ligament tensile forces were calculated using a mathematical model. The majority of patients (20 of 32) had an abnormally high adduction moment at the affected knee. The adduction moment showed a statistically significant correlation to high medial tibiofemoral compartment loads and high lateral soft tissue forces, but

Primary Anterior Cruciate Ligament Reconstruction in Level 1 Athletes: Factors Associated With Return to Play, Reinjury, and Knee Function at 5 Years of Follow-up.

BACKGROUND: Favorable outcomes after anterior cruciate ligament (ACL) reconstruction (ACLR) are often gauged by successful return to play (RTP), a low incidence of subsequent ACL injury, and positive patient-reported outcomes. Level 1 sports place the highest demands on the knee by requiring frequent pivoting, changes in direction, and jumping. PURPOSE: To analyze the outcomes of primary ACLR in level 1 athletes and identify pre- and intraoperative factors associated with RTP, ipsilateral ACL reinjury, contralateral ACL injury, and International Knee Documentation Committee (IKDC) score at 5

Evaluation of postoperative orientation of the knee and ankle joint after open wedge high tibial osteotomy.

BACKGROUND: Cases with varus knee osteoarthritis (OA) often present concomitant ankle joint deformity and poor tibial plafond inclination (TPI) in the coronal plane. Although changes to the TPI with respect to the ground after open wedge high tibial osteotomy (OWHTO) have been described, reports on the relationship between the correction angle and severity of knee joint line obliquity (KJLO) are scarce. This study aimed to examine the correlation between the correction angle after OWHTO and the change in knee and ankle inclination with respect to the floor. METHODS: Between January 2016 and D
