In order to examine the effects of an alcohol dehydrogenase inhibitor on the embryotoxic effects of ethanol, pregnant ICR mice were treated with 100 mg/kg pyrazole prior to ethanol injection. I.p. treatment with 2 or 4 g/kg ethanol on day 7 of gestation increased the prenatal mortality rate and produced external and skeletal malformations in the offspring, and the embryotoxic effects were potentiated by pyrazole pretreatment, suggesting that ethanol rather than its metabolites is mainly responsible for the embryotoxicity. In the second experiment, we housed pregnant mice in an ethanol-vapor bo

The increasing incidence of cesarean birth has become quite controversial. The practice of delivering virtually all women with a prior cesar … of whom 751 (62%) underwent a trial of labor and 614 (82%) achieved vaginal delivery. There was no maternal mortality or perinatal mortality attributed to the trial of labor process. Oxytocin usage occurred in 38% of this population. The incidence of uterine rupture and scar dehiscence was similar in the various study subgroups. The overall group of women undergoin …

An outbreak of infectious bursal disease (IBD) in two California layer flocks resulted in the isolation of two infectious bursal disease viruses designated rA and rB. Increased mortality plus gross and histopathology in the layer flocks suggested rA and rB could be very virulent infectious bursal disease virus (vvIBDV). Preliminary studies indicated that high mortality resulted when bursa homogenates from the layer farms were used to inoculate specific-pathogen-free (SPF) chicks. In addition, rA and rB contained VP2 amino acid sequences typically seen in vvIBDV. Molecular and in vivo studies w

Despite growing recognition of sex disparities in cardiovascular disease (CVD), contemporary, harmonized analyses of sex-specific burden in the European region that could inform targeted prevention strategies, remain limited. Sex-specific patterns in CVD burden and risk factor exposure across 55 European Society of Cardiology (ESC) member countries were examined using data from the 2023 Global Burden of Disease study. Risk factor exposure was quantified using age-sex-standardized summary exposure values for 10 established cardiovascular risk factors. Disease incidence and mortality for major C

Type A acute aortic dissection is a rare life-threatening event that occurs most commonly in the third trimester or early postpartum and in women with connective tissue disorders. However, this case describes a type A aortic dissection diagnosed on postpartum day 2 in a woman with preeclampsia without a history of a connective tissue disease. The case emphasizes the importance of considering dissection in any parturient complaining of chest pain, especially in the setting of hypertension and a new murmur. Emergent imaging must be considered to decrease delays in surgical repair and to minimize

Preeclampsia is a hypertensive disorder of pregnancy affecting up to 8% of pregnancies. It is associated with significant neonatal and maternal morbidities and mortality. Although its pathogenesis is not completely understood, abnormal placentation resulting in imbalance in angiogenic factors, increased inflammation, and endothelial dysfunction are thought to be key pathways in the development of the disease. Administration of low-dose aspirin is recommended by professional societies for the prevention of preeclampsia in high-risk individuals. In this review, we summarize the evidence behind t

Sepsis is a leading cause of maternal mortality and morbidity worldwide. Pyoperitoneum is a grave and life-threatening manifestation of puerperal sepsis. Administration of broad-spectrum antibiotics and drainage of pus by laparotomy has long been the cornerstone of treatment for pyoperitoneum in a parturient. In this series of six cases, the successful laparoscopic management of postpartum pyoperitoneum is discussed. The latter offers the advantage of a magnified view of the surgical field, thorough lavage and drainage, and avoidance of long incisions to explore the abdomen, resulting in faste…

Sigmoid volvulus is a rare condition seen during pregnancy with high maternal and fetal morbidity and mortality. We report a case of a young 26-year-old woman, primipara, in her third trimester who presented with recurrent sigmoid volvulus at both 30 and 32 weeks of gestation. She underwent successful repetitive endoscopic decompression on both admissions with uneventful recovery. Endoscopic evaluation is safe in pregnancy and uncomplicated volvulus. It allows for diagnostic confirmation and assessment of complications; and it has successful outcomes in the presence of a multidisciplinary team

Venous thromboembolism (VTE) remains among the leading causes of maternal mortality in the developed world, presenting variably as deep vein thrombosis (DVT), pulmonary embolism (PE) or cerebral vein thrombosis (CVT), among others. Obesity in particular has been recognized as the principal contributing factor to the risk of VTE in pregnancy and with the global increase in the rates of obesity affecting reproductive age women, heightened awareness of the risk and consequences of VTE in this population are vital. Thus, prophylaxis, diagnosis and treatment of VTE in the obese gravida are discusse…

Abdominal pregnancies constitute 1% of ectopic pregnancies, among which broad ligament pregnancy is a rare form. The maternal mortality rate has been reported to be as high as 20%. The diagnosis is seldom established before surgery. Laparoscopic management of broad ligament ectopic pregnancy is the ideal form of treatment in appropriately selected patients. We present the case report of successful laparoscopic treatment of a 3x3.5cm broad ligament pregnancy. A search of literature shows that ours is the 6(th) case report of such a rare ectopic pregnancy managed endoscopically successfully.

Varicella (chicken pox) infection is associated with a significant risk of maternal and fetal morbidity and mortality. The choice of anesthetic technique, either neuraxial or general anesthesia, in such patients remains controversial. Anesthetic management depends not only on the extent of disease involvement and associated complications, but also on the indication for cesarean delivery. We present the anesthetic management of a 25-year-old parturient with acute varicella infection who underwent emergency cesarean delivery under spinal anesthesia. The risks and benefits of neuraxial anesthesia

Acute urinary retention in pregnancy secondary to an impacted uterus is a rare occurrence. It can have non-specific presentations, leading to delay in diagnosis, hence potentially increasing maternal and fetal morbidity and mortality. A number of risk factors for the condition have been identified. We describe the case of a 31-year-old woman presenting with urinary retention at 18 weeks' gestation. Clinical examination revealed features consistent with a gravid uterus impacted in the pelvis. Management with urinary catheterisation followed by gentle manual disimpaction of the uterus was succes

Although uncommon, abnormalities of the placenta are important to recognise owing to the potential for both maternal and fetal morbidity and mortality. The placenta is often overlooked in the routine evaluation of a normal gestation, receiving attention only when an abnormality is detected. During the formal scan to confirm a fetal death, the information gathering process to elucidate a possible cause of death begins, yet, even in this instance, the placenta is seldom examined. We aim to draw attention to the importance of placental assessment by presenting a case of a stillbirth where the ans

Complete puerperal uterine inversion is an uncommon but potentially life-threatening obstetric emergency. It generally occurs as an obstetrical complication in the postpartum period and can present in acute, subacute, and chronic forms depending on the time interval after delivery. Maternal mortality has been reported to be as high as 15%, mainly because of life associated threatening blood loss and shock. Early diagnosis and treatment are essential, but diagnosis of this is not simple. This is a report of unusual case of complete uterine inversion diagnosed by accurate ultrasound leading to p…

Polyhydramnios is an excessive amount of amniotic fluid within the amniotic cavity. The etiology of polyhydramnios may be idiopathic, the consequence of fetal structural anomalies, or the consequence of various fetal and maternal conditions. The clinical importance of polyhydramnios is found in its association with adverse pregnancy outcomes and the risk of perinatal mortality. The antenatal management of polyhydramnios can be challenging as there are no formalized guidelines on the topic. The purpose of this review is to provide a literature-based overview on the subject of polyhydramnios in

Preeclampsia is a pregnancy-induced disease associated with considerable maternal and fetal morbidity and mortality. While delivery usually results in a complete healing there is growing evidence that preeclampsia is a significant risk factor for cardiovascular and metabolic diseases in later life of these mothers. Especially early-onset, severity and number of deliveries with preeclampsia are relevant risk-factors. Preeclampsia itself could play a causative role or may just be a transient early symptom. An increasing understanding of these correlations is of major importance concerning health

Heart disease is a main cause of maternal mortality in the United States and the United Kingdom. Most deaths are from acquired conditions. However, due to the increased survival of children born with congenital heart disease (CHD) over the past 30 years, the population of adults with congenital heart disease in the U.S. now exceeds 1 million. Thus, there are now more adults with CHD than children with CHD. Many of these adult survivors of pediatric heart disease are of childbearing age and are considering pregnancy. This article reviews the literature concerning pregnancy and CHD.

Purpura fulminans is an acute and frequently fatal disorder characterized by sudden onset of progressive cutaneous hemorrhage and necrosis due to dermal vascular thrombosis and disseminated intravascular coagulation. The authors present a neonate with extensive purpura fulminans due to group B streptoccoccal septicemia and evaluated the attributable clinical mortality and morbidity of this potentially lethal syndrome. Clinicians especially neonatologists should be aware that early-onset sepsis of group B Streptococcus in the newborn infant with purpura fulminans could be a cause of maternal ca

Liver transplantation : official publication of the American Association for the Study of Liver Diseases and the International Liver Transplantation Society2008eng
As the treatment of cirrhosis improves, pregnancy in patients with cirrhosis is likely to become more common. Although maternal and fetal mortality is expected to similarly improve, pregnant patients with cirrhosis face unique risks. These include higher rates of spontaneous abortion and prematurity and a potential for life-threatening variceal hemorrhage, hepatic decompensation, splenic artery aneurysm rupture, and postpartum hemorrhage. Pregnancy outcome may be influenced by the underlying etiology of liver disease, as in viral and autoimmune hepatitis. Medications also impact the course of

Pregnancy in a rudimentary uterine horn is rare and is usually associated with fetal death and serious maternal morbidity and mortality. A case of pregnancy in a rudimentary uterine horn with rupture 14 weeks after last menstrual period and is complicated with placenta accreta is presented. The patient had signs and symptoms of massive hemoperitoneum. An emergency exploratory laparotomy revealed rupture of the gravid rudimentary horn of a bicornuate uterus. Histologic examination of the specimen showed that placenta was accreta. The relative literature is reviewed and the association of placen
