[Molecular heterogeneity of malignant melanomas].

Malignant melanomas make up a heterogeneous group of tumors characterized by particular genetic aberrations depending on their anatomic localization and UV exposure. Activation of the mitogen-activated protein kinase (MAPK) signaling pathway is found in the majority of melanomas, with either somatic missense mutations of BRAF or, considerably more rarely, mutations of N-RAS. The loss of both products of the CDKN2A gene, proteins p16(ARF) and p14(INK4a), or amplification of microphthalmia-associated transcriptional factor (MITF) are also predisposing factors in the development of melanoma. BRAF

Detection of FGFR Alterations in Urothelial Carcinoma: Current Diagnostic Methodologies, Technical Considerations and Future Perspectives.

Fibroblast growth factor receptor (FGFR) alterations have emerged as clinically actionable biomarkers in urothelial carcinoma (UC), particularly following the introduction of FGFR-targeted therapies for patients with advanced disease. Activating mutations, gene fusions, copy number alterations, and expression changes involving FGFR2 and, especially, FGFR3 contribute to urothelial tumorigenesis through persistent activation of oncogenic signaling pathways and are enriched in specific molecular subtypes. Because these alterations occur at different molecular levels, their accurate detection requ

Molecular classification of infiltrating breast cancer: toward personalized therapy.

Breast cancer is a heterogeneous disease, which comprises several molecular and genetic subtypes, each with characteristic clinicobiologic behavior and imaging patterns. Traditional classification of breast cancer is based on the histopathologic features but offers limited prognostic value. Novel molecular characterization of breast cancer with cellular markers has allowed a new classification that offers prognostic value, with predictive categories of disease aggressiveness. These molecular signatures also open the door to personalized therapeutic options, with new receptor-targeted therapies

Treatment of Lung Cancer in Medically Compromised Patients.

Outcomes for patients with lung cancer have been improved substantially through the integration of surgery, radiation, and systemic therapy for patients with early-stage disease. Meanwhile, advances in our understanding of molecular mechanisms have substantially advanced our treatment of patients with advanced lung cancer through the introduction of targeted therapies, immune approaches, improvements in chemotherapy, and better supportive care. However, the majority of these advances have occurred among patients with good functional status, normal organ function, and with the social and econom

Analysis of genetic mutation characteristics in 40 Chinese patients with hepatocellular carcinoma.

BACKGROUND: Hepatocellular carcinoma (HCC) is notable for its marked molecular heterogeneity and poor response to systemic therapy. Targeted next-generation sequencing (NGS) has facilitated the profiling of somatic alterations in HCC. However, the interpretation of panel-based genomic features such as tumor mutational burden (TMB), copy number alterations, and candidate gene fusions remains challenging, particularly in HBV-endemic Asian populations. OBJECTIVE: To explore the somatic mutation landscape of HCC using a targeted sequencing panel in a regional cohort and to descriptively assess it

Discontinuation of tyrosine kinase inhibitors and new approaches to target leukemic stem cells: treatment-free remission as a new goal in chronic myeloid leukemia.

Only a small fraction of chronic phase chronic myeloid leukemia patients (CP-CML) achieves a very deep reduction of residual disease with imatinib. Second-generation tyrosine kinase inhibitors administered as front-line therapy for CP-CML have improved the rates and degree of deeper molecular responses. Owing to this improvement, new standardized definition of complete molecular remission has been provided, which allowed plan of prospective strategies to definitively discontinue therapy in the long-term. In this review, we report the results of several published discontinuation studies and cri

Anthrax lethal factor as an immune target in humans and transgenic mice and the impact of HLA polymorphism on CD4+ T cell immunity.

Bacillus anthracis produces a binary toxin composed of protective antigen (PA) and one of two subunits, lethal factor (LF) or edema factor (EF). Most studies have concentrated on induction of toxin-specific antibodies as the correlate of protective immunity, in contrast to which understanding of cellular immunity to these toxins and its impact on infection is limited. We characterized CD4+ T cell immunity to LF in a panel of humanized HLA-DR and DQ transgenic mice and in naturally exposed patients. As the variation in antigen presentation governed by HLA polymorphism has a major impact on prot

Treatments in idiopathic pulmonary fibrosis: time for a more targeted approach?

Idiopathic pulmonary fibrosis (IPF) is a progressive age-related lung disease, the cause of which is not been fully understood. IPF is a devastating disease with mortality worse than many cancers, and treatment options are limited. IPF is thought to occur after recurrent injury to the alveolar epithelium followed by abnormal repair characterized by the formation of fibroblast and myofibroblast foci and excessive deposition of extracellular matrix. An updated classification of the idiopathic interstitial pneumonias has encouraged a large number of clinical trials. On the whole, these have disap

Exploring the Comorbidity Mechanism among Functional Diarrhea, Irritable Bowel Syndrome with Predominant Diarrhea, and Functional Dyspepsia through the Gut-brain Axis and the Pharmacological Mechanisms of Shen-Ling-Bai-Zhu-San Therapy.

BACKGROUND: Functional Diarrhea (FDr), Irritable Bowel Syndrome with predominant diarrhea (IBS-D), and Functional Dyspepsia (FD) are common functional gastrointestinal disorders (FGIDs) with significant impacts on quality of life. While the gut-brain axis and key regulators like 5-hydroxytryptamine (5-HT), dopamine (DA), and butyrate are known to play crucial roles in these conditions, the mechanisms underlying their comorbidities remain unclear. Shen-Ling-Bai-Zhu-San (SLBZS), a traditional herbal formula, is effective in treating FGIDs. Still, the specific components and mechanisms mediating

Discovery of Bis-Thiourea Derivatives as Human DNA Topoisomerase IIα Inhibitors with Potent Anticancer Effects.

DNA topoisomerase IIα (Topo IIα) is essential for maintaining genomic stability during DNA replication and mitosis and is highly expressed in cancer cells, making it a promising target for anticancer therapy. In this study, bis-thiourea derivatives were investigated for their Topo IIα inhibitory activity and anticancer potential using in silico and in vitro studies. Molecular modeling demonstrated that compound 8 exhibited favorable binding affinity and stability within the ATPase domain of Topo IIα. Biochemical assays revealed that compound 8 inhibited Topo IIα activity and showed potent cyto

HIV-1 virome profiling using HIV-PULSE to guide therapeutic and curative interventions.

BACKGROUND: The persistent HIV-1 reservoir remains a major barrier to curing HIV-1, underscoring the need for a detailed understanding of its composition to inform targeted interventions. METHODS: Here, we apply the HIV Proviral Unique molecular identifier-mediated Long-read Sequencing (HIV-PULSE) assay to characterise the viral reservoir in 90 individuals on suppressive antiretroviral therapy. By generating near full-length proviral sequences, HIV-PULSE enables multiple layers of in silico analyses across the HIV-1 genome. FINDINGS: These data support the construction of participant-specifi

Management of diarrhea in solid organ transplantation.

PURPOSE OF REVIEW: Diarrhea is a common complaint in solid organ transplant recipients. We review both infectious and noninfectious causes of diarrhea and their management. RECENT FINDINGS: Diagnostics for diarrhea have now commonly incorporated multiplex gastrointestinal panels that provide rapid testing and identification of pathogens. The rate of Clostridium difficile in the transplant population has increased and fidaxomicin is now recommended as the therapy of choice for first episode and recurrences where available. Oral vancomycin remains an alternative. Norovirus is important to rule

Targeted Gene Sequencing in a Male Adult Diagnosed With X-Linked Osteoporosis Due to a Novel p.(Arg398Profs*2) PLS3 Variant.

Pathogenic loss-of-function variants in the plastin-3 gene (PLS3), encoding plastin-3 protein, are associated with early-onset X-linked osteoporosis. We present the case of a young adult male patient, with a history of multiple fragility fractures and blue sclerae, who was clinically diagnosed with osteogenesis imperfecta (OI) type 1 in childhood. He has been managed with intravenous bisphosphonate therapy, leading to an increase in bone density at the spine, stable bone density at the femoral neck, and a period free of fractures while on antiresorptive therapy. Two decades later, with a focus
