Comparison of two beam angular optimization algorithms guided by automated multicriterial IMRT.

PURPOSE: To compare two beam angle optimization (BAO) algorithms for coplanar and non-coplanar geometries in a multicriterial optimization framework. METHODS: 40 nasopharynx patients were selected for this retrospective planning study. IMRT optimized plans were produced by Erasmus-iCycle multicriterial optimization platform. Two different algorithms, based on a discrete and on a continuous exploration of the space search, algorithm i and B respectively, were used to address BAO. Plan quality evaluation and comparison were performed with SPIDERplan. Statistically significant differences betwee

Malignant peripheral nerve sheath tumor of the nasal cavity and nasopharynx in a child: A case report.

RATIONALE: Malignant peripheral nerve sheath tumors (MPNSTs) are rare neoplasms with only a few reported cases affecting the nasal cavity, paranasal sinuses, and anterior skull base. PATIENT CONCERNS: A 12-year-old girl with a mass in her nose was admitted to the Department of Otorhinolaryngology of Shenzhen Children's Hospital. She had a 4-month history of progressive, unilateral right nasal obstruction, unilateral mucopurulent rhinorrhea, foul nasal odor, snoring, hyposmia, occasional epistaxis, and no headache, no facial numbness, without eye swelling and vision loss. DIAGNOSE: A computed

Hairy polyps: A new case presentation and a pathogenetic hypothesis.

Hairy polyps are benign lesions found in the oropharynx or nasopharynx that are thought to be present at birth and can lead to upper airway obstruction in infants. Also known as naso-oropharyngeal choristoma, they are increasingly viewed as aggregates of bigeminal tissue, likely from the first or second branchial arches, found in aberrant locations. They are benign lesions that are usually successfully treated by surgical excision. Here we present a rare case of a hairy polyp originating in the eustachian tube of a 7-week-old male, discuss our management of the patient, and put forth a new hyp

Masses of the Nose, Nasal Cavity, and Nasopharynx in Children.

A wide range of masses develop in the nose, nasal cavity, and nasopharynx in children. These lesions may arise from the nasal ala or other structures of the nose, including the mucosa covering any surface of the nasal cavity, the cartilaginous or osseous portion of the nasal septum, the nasal turbinates, and the nasal bones. Lesions may also arise from the nasopharynx or adjacent structures and involve the nose by way of direct extension. The causes of nasal masses in children include congenital and developmental disorders such as congenital nasolacrimal duct mucocele, dermoid cyst, cephalocel

[Endoscopic transnasal surgery for giant fibrous dysplasia of the skull base, spreading to the right orbital cavity and nasopharynx (a case report and literature review)].

BACKGROUND: An extended endoscopic endonasal approach is increasingly used in surgical treatment of space-occupying skull base lesions. The international literature reports only 20 cases of surgical treatment for fibrous dysplasia (PD) of the skull base using the endoscopic endonasal approach. We present our experience with the endoscopic endonasal approach in surgical treatment for giant fibrous dysplasia of the skull base, spreading to the right orbital cavity and nasopharynx. CLINICAL CASE: A 26-year-old male patient presented with cranial pain, Vth nerve dysfunction on the right, right ke

Post-radiotherapy bilateral external auditory canal cholesteatoma.

In this article, we report a 55-year-old female case was treated with radiotherapy due to nasopharynx carcinoma and diagnosed with bilateral external auditory canal cholesteatoma four years after radiotherapy. Persistent otorrhea was present in the patient and the diagnosis was established through noticing a soft tissue mass eroding external auditory canal anterior wall in temporal bone tomography as well as the otoscopic findings. Canaloplasty and cholesteatoma excision were administered as the surgical treatment.

The other side of the coin: Leveraging Epstein-Barr virus in research and therapy.

Epstein-Barr virus is (EBV) a ubiquitous virus prevalent in 90% of the human population. Transmitted through infected saliva, EBV is the causative agent of infectious mononucleosis (IM) and is further implicated in malignancies of lymphoid and epithelial origins. In the past few decades, research efforts primarily focused on dissecting the mechanism of EBV-induced oncogenesis. Here, we present an alternate facet of the oncovirus EBV, on its applications in research and therapy. Finally, discussions on the prospective utilization of EBV in nasopharyngeal carcinoma (NPC) diagnosis and therapy wi

Ectopic thyroid papillary carcinoma of nasopharynx associated with adenoid hypertrophy: an unusual presentation.

Ectopic thyroid tissue of nasopharynx is an uncommon phenomenon and papillary thyroid carcinoma arising from the tissue is extremely rare. The authors report a rare case of 16-year-old girl with papillary thyroid carcinoma of nasopharynx. Clinicians were ever confused by adenoid hypertrophy and solved the diagnostic dilemma by adequate examinations. In the case, we mainly emphasize that surgeons should be aware of and actively consider such a possibility of ectopic papillary thyroid carcinoma of nasopharynx in children and adolescents with long-term nasal obstruction, even if thyroid carcinoma

Transpalatal approach with pedicled palatal osteo-muco-periosteal flap.

BACKGROUND: In the conventional technique of transpalatal approach, the palatal mucoperiosteum is incised and widely elevated and hard palate bone is removed in piece meal and discarded. These technique lead to complications, namely, temporary or permanent palatal fistula, palatal scarring leading to palatal disfiguration and velopharyngeal incompetence. In this article we describe a new technique of doing palatotomy to avoid these complications. METHODS: A retrospective review of post-operative complications of 20 patients that underwent transpalatal approach by the conventional technique wa

[Ectopic Warthin's tumour in nasopharynx. Revision of one clinical case].

This is a case report of a 69-year-old male patient referred to the ENT out patients with right-sided epistaxis and hipoacusis. Fibroptic examination reveals a cystic lesion at the exit of the right eustachian. A CT scan is informed as a well delimited and rounded-shaped tumour compatible with lymphoma. Excisional biopsy by a nasal rigid endoscopy is practiced and the histology result is Warthin's tumour. The nasopharynx is a rather unusual location for this type of tumours, almost exclusive in origin of major salivary glands. It's highlighted the unforeseeable histological diagnosis in this p

[Extramedullary plasmacytoma of the head and neck].

The aim of this review was to present plasma cell neoplasms, their development, common features and classification. Extramedullary plasmacytoma (EMP) is a most often plasma cell neoplasm in the head and neck. 80-90% EMP cases concern to head and neck: 40% nasal cavity and paranasal sinuses, about 20% nasopharynx, 18% oropharynx. Tumour symptoms are similar like in other neoplasm. It is very important to remember about plasma cell neoplasm during differential diagnosis because of relatively common occurrence. Extramedullary plasmacytoma is radiosensitive neoplasm with good prognosis therefore r

Post-operative cerebral infarction and Takayasu's disease.

A case of post-operative cerebral infarction is presented in a 53-year-old man, in whom a clinical diagnosis of Takayasu's disease was made retrospectively. The cerebral infarction occurred in the absence of any identifiable surgical or anaesthetic cause. The possible role of sodium nitroprusside in preventing cerebrovascular accidents in this condition is discussed and the literature reviewed.

Hairy polyp of the eustachian tube.

The hairy polyp of the nasopharynx is a rare but potential cause of airway obstruction in the newborn. A case of hairy polyp arising off the Eustachian tube is reported. Only eight prior cases of Eustachian tube origin have been described in the literature. The terminology and embryology of these tumors are included in the discussion. The site of the tumor and histopathology of this case supports the embryologic origin of a congenital inclusion cyst.
