Electrocochleographic evaluation of the guinea pig model of endolymphatic hydrops.

Electrocochleography (ECochG) was used to evaluate cochlear function in guinea pigs with experimentally induced endolymphatic hydrops (ELH) before and after osmotic dehydration with either glycerol or urea. We surgically induced ELH in the right ears of 9 guinea pigs, while the right ears of 6 guinea pigs received a sham operation. The left ears of the 15 animals constituted the normal group. Eight weeks after surgery, summating potential (SP) and action potential (AP) amplitudes were measured prior to and following the administration of glycerol or urea. The SPs and SP/AP ratios were reduced

Neurological phenomena during emergence from enflurane or isoflurane anaesthesia.

During emergence from anaesthesia, transient neurological signs that would usually be considered pathological may appear. The objective of this randomized, patient (n = 30) and observer-blinded study was to compare prospectively the incidence and duration of post-anaesthetic neurological abnormalities in healthy patients undergoing minor elective procedures following thiopentone and succinylcholine induction, and enflurane-N2O or isoflurane-N2O anaesthesia. Patients were studied for 60 min after anaesthesia. Arousal state, muscle tone, deep tendon reflexes, plantar reflex, sustained clonus, sh

The role of the globus pallidus subregions in the schizophrenia spectrum continuum.

Within the cortico-basal ganglia network, the volume and functional connectivity (FC) of the globus pallidus (GP) are altered in schizophrenia-spectrum disorders, with GP enlargement being relatively specific to schizophrenia compared to other disorders. However, little is known about alterations in its two major segments, externus (GPe) and internus (GPi), in schizophrenia-spectrum disorders. In this study, we compared GPe/GPi volume and FC between patients with schizophrenia and controls (n > 100), and examined the associations of GPe/GPi volume and FC with subclinical psychotic experiences

Repeatable, low-drift recordings in behaving nonhuman primates using flexible microelectrodes.

Neurophysiological recordings from nonhuman primates rely on rigid microelectrode arrays whose mechanical mismatch with brain tissue causes neuronal drift and unstable single-unit recordings. Here we present a flexible microelectrode platform for repeated recordings in awake rhesus macaques. We fabricated a 32-channel, 7-μm-thick Parylene-C array and developed a telescopic insertion method using concentric guide tubes and a retractable microwire shuttle to repeatedly deliver free-floating arrays through intact dura using standard recording chambers. Across two animals, we optimized the electro

Astrocytic TCF7L2 Impacts Brain Osmoregulation and Restricts Neuronal Excitability.

Astrocytes differentiate and mature during postnatal development, but the molecular mechanisms linking their maturation to neuronal function remain unclear. We investigated the role of Wnt/β-catenin signaling and its effector, the transcription factor TCF7L2, in postnatal astrocytes using single-nucleus RNA sequencing, imaging, morphometric analysis, microdialysis, and electrophysiology in Tcf7l2 conditional knockout (cKO) mice. Loss of Tcf7l2 caused widespread transcriptional dysregulation in astrocytes, particularly in genes related to amino acid and ion transport, as well as membrane potent

The liver as a metabolic and immune hub in Alzheimer's disease: From mechanisms to therapeutic opportunities.

Research on Alzheimer's disease (AD) has traditionally focused on the brain. However, emerging evidence indicates that the liver acts as a silent partner in neurodegeneration. As a core hub for metabolic and immune regulation, the liver communicates bidirectionally with the brain via the liver-brain axis, participating in the regulation of various neurophysiological processes, including neurotransmitter regulation, feeding behavior, and cognition. This review summarizes how liver-derived hepatokines, inflammatory mediators, and metabolic products modulate brain function. We highlight that live

Ensemble Coding of Hidden Objects in Visual Cortex.

Many species exhibit the understanding that visual objects that become hidden by others nonetheless still exist, a property known as object permanence. Previous studies in human and nonhuman primates have provided evidence that neurons within visual cortex encode objects that are remembered but not seen. However, past neurophysiological studies have generally failed to find evidence of visual cortical representations of hidden objects. We measured the activity of large populations of neurons within dorsal extrastriate cortex of macaques trained to monitor the identity of visual objects that mo

Human neuronal networks on micro-electrode arrays as a tool to assess genotype-phenotype correlation in CACNA1A-related disorders.

CACNA1A-related disorders constitute a diverse group of neurological conditions, including ataxia, migraine, and epilepsy. Despite extensive genetic studies, clear genotype-phenotype correlations remain elusive. Moreover, next-generation sequencing has identified many variants of uncertain significance (VUS). Here, we leveraged patient-derived and CRISPR-Cas9-engineered human neuronal networks to explore relationships between CACNA1A variants and neurophysiological activity. CACNA1A haploinsufficiency induced subtle alterations in glutamatergic network activity, whereas missense variants had a

Robot- and navigation-assisted cervical fusion and decompression for complex vertebral artery anomalies in os odontoideum with Klippel-Feil syndrome: illustrative case.

BACKGROUND: Vertebral artery (VA) anomalies, particularly those associated with congenital atlantoaxial dislocations, are well documented in the literature. Typically, these anomalies are linked to persistent first intersegmental artery variants or occur in conjunction with occipitalization of the atlas. The case presented in this report is unique due to the presence of bilateral ectatic VAs with an anomalous course beneath the posterior arch of C1, in the absence of occipitalization of the atlas. This finding adds to the growing recognition of anatomical variability in patients with congenita

Slow wave canonical activity deviation concept: Toward a slow wave-based EEG-fMRI reference map for health-associated network function.

Functional network architecture associated with health varies across individuals, limiting neuroimaging sensitivity for detecting early network maladaptations and identifying at-risk individuals. To increase sensitivity, we use slow wave events (SWEs) as neurophysiological markers in EEG-informed fMRI analyses. Neural excitability and network state impact SWE spatiotemporal dynamics. Analyzing simultaneous EEG-fMRI from two healthy cohorts (N = 24), we generated individual SWE-related BOLD maps. We created cohort-level probabilistic maps and derived reference maps revealing consistently engage

Machine learning and molecular docking-driven identification of novel umami peptides from pea protein and electroencephalography (EEG) neural mechanisms analysis of their umami-enhancing effects.

This study developed an integrated strategy combining machine learning prediction, molecular docking, and EEG-based neural characterization to screen and validate novel umami peptides derived from pea protein, including QEGEK, QEEEEQSH, and ATTETVDALR. The results showed that the umami recognition thresholds of the three peptides ranged from 0.203 to 0.328 mM. Sensory evaluation and electronic tongue analysis further indicated that QH8 and AR10 exhibited significant umami-enhancing effects when combined with MSG. EEG analysis provided objective neurophysiological evidence for the umami-enhanci

Neural Circuit Taxonomy and Precision Psychiatry in Major Depression.

Patients with major depressive disorder (MDD) demonstrate a relatively higher relapse risk and exhibit considerable clinical and biological heterogeneity with possible distinct neurophysiological mechanisms. In this chapter, we adapt an approach of neural circuit taxonomy and will demonstrate brain network-based correlates of clinical symptom subtypes in MDD. First, anhedonia might encompass negative-affect circuit dysfunction and reward-processing dysfunctions. Structural covariance of brain gray matter morphologies within the salience and limbic networks, and among the salience limbic defaul

Exploring Pembrolizumab-Induced IM3OS in a Patient With Bladder Cancer.

Pembrolizumab, an immune checkpoint inhibitor (ICI), has transformed cancer treatment across various malignancies, including metastatic urothelial cancer. As the use of pembrolizumab becomes increasingly widespread, understanding its potential adverse effects is essential. Here, we present the case of a 79-year-old male with urothelial cancer of the bladder, who experienced local recurrence after surgery and conventional therapies. He subsequently began pembrolizumab treatment. Following the first infusion, the patient developed a series of complications over a span of 26 days, including acute

Lessons from a Severe Case of Fulminant Guillain-Barré Syndrome: A Case Report.

Background and Clinical Significance: Guillain-Barré syndrome (GBS) can rarely progress to fulminant paralysis with loss of brainstem reflexes, mimicking coma or brain death despite preserved cortical function. Case Presentation: A 38-year-old man developed rapidly progressive weakness following a diarrheal illness, culminating in quadriplegia, areflexia, respiratory failure, and complete loss of brainstem reflexes within 72 h. Neuroimaging was unrevealing. EEG demonstrated preserved cerebral activity with an alpha coma pattern. Despite initial intravenous immunoglobulin therapy, neurological

Epileptic activity in Alzheimer's disease: emerging insights and therapeutic implications.

An estimated 60% of patients with Alzheimer's disease develop epilepsy or subclinical epileptiform activity over the course of the disease. New-onset seizures in cognitively healthy adults also increase the risk of developing dementia. Epileptic activity, including both seizures and subclinical epileptiform discharges, can hasten the onset of Alzheimer's disease and accelerate cognitive decline. Studies are investigating whether antiseizure medications could improve cognitive outcomes, particularly in patients with Alzheimer's disease with epileptic activity. Detection of epileptic activity in

Persistent Post-Concussion Syndrome Following Motor Vehicle Collision: A 27-Month Longitudinal Case Study of Multimodal Rehabilitation.

Persistent post-concussion syndrome (PPCS) develops in 10 to 30 percent of patients following mild traumatic brain injury (mTBI), yet the most effective management approach remains an open question. This report describes a 37-year-old female who sustained a motor vehicle collision and was followed prospectively over 27 months. Structured evaluation at baseline identified dysfunction across oculomotor, vestibular, autonomic, cognitive, and neurophysiological domains. An intensive multimodal rehabilitation programme was associated with an 86 percent reduction in symptom burden (Post-Concussion S

Osmotexia and the somatosensory dimension of smell: toward a fourth axis in chemosensory evaluation.

BACKGROUND: The evaluation of olfaction is classically based on three psychophysical axes: threshold, discrimination, and identification. However, these do not fully reflect the multisensory nature of odour perception. It is well established that numerous odours can activate the trigeminal system, thereby generating somatosensory sensations such as texture, density, or temperature. This is a dimension that is currently absent from clinical evaluation models. AIMS/OBJECTIVES: The present study introduces and conceptualises osmotexia as the ability to experience odours in terms of internal text

Posterior Approach for Microsurgical Resection of a Ventrolateral Cervical Ganglioneuroma: an Illustrative Case and Technical Report.

Ganglioneuromas of the cervical spine are exceptionally rare, well-differentiated neuroblastic tumors. We present the case of a 4-year-old female with a several-month history of progressive fatigue, right upper extremity weakness, and gait instability, in whom MRI demonstrated a homogeneously enhancing, cervical intradural extramedullary mass causing significant ventral spinal cord compression. A posterior midline approach with cervical two to cervical six laminoplasty was performed under continuous neurophysiological monitoring, providing wide dorsal access to the lesion. Fidelity of dorsal column axon activity in response to clinical spinal cord stimulation paradigms.

Many novel spinal cord stimulation (SCS) paradigms have been developed during the last decade. However, understanding of underlying neurophysiological mechanisms is well behind clinical application. Therefore, this study aims to investigate differences between two clinically applied low-frequency SCS paradigms applied at 40Hz and 90Hz. A SCS paddle lead was implanted under the T1 vertebra of seven urethane anesthetized rats. Antidromic action potentials were recorded from the dorsal columns at T13/L1 using a 20MΩ bipolar recording electrode. Spiking fidelity and spike amplitude were investigat
