The young chronic client in mental health today.

Due to the nature of the work with this demanding population, who, in spite of all efforts, demonstrates only minimal changes at best, nurses need aggressive management of their own lives. Burnout is rampant. Work with this type of client is not for everyone. Programs need to offer staff the opportunity to work with these clients only if the staff is capable and desires the challenge. It is necessary to set realistic goals and expectations with clients and to learn to accept tiny changes. Nurses must help the client become more realistic in their expectations of what will be achieved. It is im

[Skewfoot ].

The deformity, congenital metatarsus adductus, has been given different names in various countries by different authors. In addition, there are both mild and severe forms, and some authors subdivide the deformity by degree. The severe form is also called skewfoot or serpentine foot, but to us these terms only appear to represent differences in degree. With regard to etiology, it has been found that a narrow uterus, for various reasons, seems to play a role, and that the foot and lower leg is rotated internally. If not treated early, the internal rotation of the lower leg increases and is frequ

[Prenatal diagnosis and postnatal course in abnormalities of the kidneys and urinary tract].

The authors' own experiences concerning the prenatal diagnosis and postnatal course in 26 foetuses with anomalies of the urinary tract are reported. The malformations observed can be separated into five groups which can also be distinguished sonographically: Urinary tract malformations of poor prognosis, Unilateral cystic kidneys. Hydronephroses, Urinary tract malformations with ascites, Urinary tract malformations in combination with other anomalies of poor prognosis. Sonographic differentiation of the multicystic kidney (Potter type IIA) from the hydronephrosis is usually possible. The recog

[Diabetes mellitus type I, thyroid gland autoimmunity, thyroid gland function and HLA status].

In a cross-sectional study sera of 130 children and adolescents with insulin dependent (type I) diabetes were tested for the presence of cytoplasmatic islet cell antibodies (ICA), thyroid microsomal antibodies (TMA) as well as thyroid function (T3, T4, thyrotrophin). 102 of the patients were examined for thyroid enlargement, all patients with TMA were within this group. TMA were found in 15.4%, ICA in 38.5%, the latter decreasing with duration of diabetes. Of the TMA positive sera, 45% were also ICA positive, but only one child out of these was diabetic for more than 5 years. Thyroid enlargeme

Analysis of a microcephalic child from the late period (ca. 1100-1700 A.D.) of central California.

Microcephaly is a severe developmental abnormality which is induced either genetically or environmentally. The archaeological evidence for the occurrence of this abnormality is minimal. Out of six possible cases, only three have been published; all are adult and represented solely by cranial remains. The focus of the current study is the nearly complete skeleton of a 3-year-old child recovered during archaeological excavations in San Jose, California. The individual is compared to a sample of 184 California Indians, ranging from 0.5 to 5 years, which were also recovered from archaeological exc

A congenital abnormality in the arrangement of muscle bundles in a segment of the distal ileum, producing obstruction: a variant of the so-called "giant Meckel's diverticulum".

This 4-year-old male child was diagnosed at birth as having several minor congenital anomalies. X-rays taken during the first year of his life showed a single, massively dilated loop of bowel in the upper abdomen, not appreciated at the time. In 1980 he was admitted to Jackson Memorial Hospital with the diagnosis of small bowel obstruction. An upper gastrointestinal series showed one tremendously dilated loop of distal ileum. At laparotomy, the patient was found to have extreme segmental dilatation of one loop of distal ileum which ended abruptly; there was no evident external cause for obstru

Repair of ultraviolet light damage in a variety of human fibroblast cell strains.

Postreplication repair of DNA damage after ultraviolet light irradiation has been examined in a wide variety of human fibroblast strains. The donors were patients with xeroderma pigmentosum (XP) of different complementation groups or other hereditary disorders with indications of radiosensitivity, or with light sensitivity or multiple cancers. The defect in postreplication repair previously found in XP variants (excision-proficient XP's) has now been observed in a total of five XP variants and a less severe defect in postreplication repair has been found in excision-defective XP's in Complemen

A population-based study of access to immunization among urban Virginia children served by public, private, and military health care systems.

BACKGROUND: Pediatric immunization rates have increased in the United States since 1990. Nevertheless, national survey data indicate that up to one third of 2-year-old children in some states and urban areas lack at least one recommended dose of diphtheria-tetanus-pertussis (DTP)-, polio-, or measles-containing vaccines. Immunization has become a key measure of preventive pediatric health care in the United States. To achieve and maintain the national immunization goal that 90% of children receive all recommended immunizations by 2 years of age, the role of the health care system in immunizati

Long-Term Clinical, Immunologic, and Viral Reservoir Outcomes in Children Treated With VRC01LS and 10-1074 Monoclonal Antibodies in the Tatelo Study.

BACKGROUND: Broadly neutralizing antibodies (bNAbs) are a promising treatment option for children with HIV-1, but their long-term impact on virologic, immunologic, and clinical outcomes is unknown. METHODS: The Tatelo Study investigated dual bNAbs as an alternative to standard antiretroviral treatment (ART) in children. Children received bNAb-only treatment for 24 weeks (or until detectable viremia ≥400 copies/mL occurred) and were followed initially through 24 weeks after restarting ART. We continued follow-up from 24 to 96 weeks post-bNAb intervention to identify the long-term clinical, imm

Toxicity of traditional and soluble film automatic dishwashing tablets.

INTRODUCTION: Detergents used in automatic dishwashing machines are of two main types: traditional tablets that require removal from an external wrapper and newer soluble film tablets. OBJECTIVE: To determine the toxicity of automatic dishwashing tablets. METHODS: Telephone enquiries to the UK National Poisons Information Service were analysed for the period January 2008 to June 2019. RESULTS: Ingestion: Ingestion was involved in 798 traditional tablet exposures and 725 soluble film exposures. Clinical features (Poisoning Severity Score ≥ 1) developed in 22.2% of patients ingesting traditio

Hepatic artery injection of ¹³¹I-labelled metuximab combined with chemoembolization for intermediate hepatocellular carcinoma: a prospective nonrandomized study.

PURPOSE: Hepatocellular carcinoma (HCC) is the fifth and seventh most common cause of cancer in men and women, respectively. Transcatheter arterial chemoembolization (TACE) is the standardized therapy for the intermediate stage of HCC. However, the 3-year overall survival remains low (<30 %) in these patients. Thus, there is a critical need for the development of treatment modalities to improve the survival rate. This study aimed to evaluate whether the combination of (131)I-metuximab with chemoembolization could improve treatment efficiency. METHODS: Between January 2009 and January 2010, a

When Children's Needs Do Not Fit the System: Street-Level Bureaucrats Facilitating Home Care for Children Living With Respiratory Support.

This study explores the meanings of acting as a street-level bureaucrat in facilitating home care for children living with respiratory support and their families. A phenomenological hermeneutic approach was used, based on interviews with professionals involved in assessment, decision-making and care provision across different parts of the welfare system. The findings show that this work is lived in tensions between compassion and regulation, closeness and distance, and enduring needs and finite resources. Acting as a bureaucrat in this context means being confronted by the vulnerability of the

Complex mosaic form of trisomy chromosome 18 and monosomy chromosome X in a girl with esophageal atresia and mild developmental delay: Diagnostic and therapeutic challenge and a review of literature.

The coexistence of mosaic trisomy 18 and Turner syndrome represents an extremely rare genetic constellation, with only a few cases reported in the literature to date. We present the case of a 7-year-old girl with esophageal atresia, full trisomy 18 in lymphocytes, and mosaic copy number changes involving trisomy 18 with coexisting monosomy X in fibroblasts, suggesting the presence of additional cell populations lacking trisomy 18. In addition, we provide a review of the relevant literature. This case is of particular clinical significance, as it describes long-term survival in a child with Edw

Cerebral cystic echinococcosis in Vietnam.

In Vietnam, only few cases of cystic echinococcosis have been reported up to now. We report on the first case of cystic echinococcosis involving the central nervous system reported in Vietnam. Our patient, a 14-year-old child, presented with symptoms of headache and progressive tremor in the right hand that had lasted for 1 month and had worsened in the last days before admission. Following the demonstration of a large space occupying brain cyst by computed tomography, a presumptive diagnosis of a brain tumour was made, that was later overruled. Magnetic resonance imaging, a biopsy and serolog

[Between ethics and practice : thoughts about the clinical management of an infant with trisomy 18 (Edwards syndrome)].

Trisomy 18, also known as Edwards syndrome, is the second most common autosomal trisomy. The phenotype includes dysmorphia, congenital malformations and severe neurological impairment. The most common malformation is congenital heart disease. With a median survival of 2 to 14.5 days, trisomy 18 has been historically considered as lethal and care has long been palliative. This approach was proposed to a little girl with postnatal diagnosis. Initially, the child's parents disagreed with the management strategy. After further discussions, the parents agreed on a therapeutic approach based on comf

A Novel Pathogenic Splicing Mutation of OFD1 is Responsible for a Boy with Joubert Syndrome Exhibiting Orofaciodigital Spectrum Anomalies, Polydactyly and Retinitis Pigmentosa.

Joubert syndrome (JS) is an infrequent congenital neurodevelopmental ciliopathy, typically identified in children around the average age of 33 months. This disorder is characterized by developmental delay, cognitive impairment, and infantile hypotonia that may evolve into ataxia. Mutations in OFD1 results in Joubert syndrome with a variety of phenotypes. Here, we identified a child who presented with Joubert syndrome exhibiting orofaciodigital spectrum anomalies, polydactyly, and retinitis pigmentosa. Whole exome sequencing and Sanger sequencing revealed a splicing mutation (NM_003611.2, c.238

Knockdown-resistant mutations in head lice (Pediculus humanus capitis) collected from schoolchildren in Riyadh, Saudi Arabia.

Head lice infestation remains one of the most common child problems. This problem is not only attributed to the ability of head lice to spread rapidly but also because of the head lice resistance that develops from incomplete or improper treatment. Pyrethroids are a group of medications that have been widely used for the treatment of head lice. However, in recent years, several countries reported knockdown resistance (kdr). The aim of this study is to evaluate the frequency of pyrethroid-resistant mutations in Riyadh City, Saudi Arabia. To do that, Sanger sequencing was employed to find the fr

Aerococcus Urinae Infection Causing Malodorous Urine in a Child: A Case Report.

Aerococcus urinae (A. urinae) infection, primarily observed in elderly patients, is a rare yet emerging occurrence in the pediatric population. Advances in laboratory techniques have facilitated the increased identification of these bacteria in human infections. There have been only a few recent cases reported among children. The scarcity of literature on the clinical presentation and management of such infections in children presents a challenge for pediatricians. Here, we present the case of a 15-month-old male child with Down syndrome who presented with malodorous urine but lacked other typ

Grisel's syndrome and Down syndrome: a case report.

Grisel's syndrome is a non-traumatic subluxation of the atlantoaxial joint following an inflammatory process in the upper respiratory tract. Patients with Down syndrome have higher risks of developing atlantoaxial instability. This issue is mainly due to low muscle tone, loose ligaments, and alterations to the bone in patients with Down syndrome. Accompaniment of Grisel's syndrome and Down syndrome was not perused in recent investigations. To our knowledge, only one case of Grisel's syndrome in an adult patient with Down syndrome has been reported. In this study, we present a case of Grisel sy
