Mincle Receptor Deficiency Protects Against LPS-induced Preterm Birth and Fetal Inflammatory Response Syndrome.

BACKGROUND: Spontaneous preterm birth (PTB) and fetal inflammatory response syndrome (FIRS) are severe consequences of intra-amniotic inflammation (IAI) that heavily contribute to neonatal morbidity and mortality. While the terminal pathways of labor are well characterized, the specific upstream innate immune sensors responsible for initiating the breakdown of maternal-fetal tolerance and triggering the inflammatory parturition cascade remain incompletely defined. OBJECTIVE: This study aimed to investigate the in vivo functional role of the C-type lectin receptor Mincle in mediating lipopolys

Late-Onset Neonatal Sepsis: Contemporary Epidemiology, Diagnosis, Prevention, and Precision Therapeutics.

OBJECTIVES: Late-onset neonatal sepsis (LOS) remains a major challenge in modern neonatal intensive care units (NICUs), disproportionately affecting very low birth weight (VLBW) and extremely preterm infants. Despite advances in perinatal care, LOS continues to contribute to high mortality, prolonged hospitalization, and long-term neurodevelopmental impairment. Emerging trends-including evolving pathogen profiles, increased antimicrobial resistance (AMR), and widespread microbiome disruption-highlight persistent clinical and public health concerns. This review synthesizes current evidence on t

Adverse birth outcome and associated factors among mothers with antepartum hemorrhage in public hospitals Tigray, northern Ethiopia, 2020.

BACKGROUND: Adverse birth outcome is a major public health problem in the world and in Ethiopia. Antepartum hemorrhage complicates 3-5% of pregnancies and is a primary cause of perinatal and maternal mortality worldwide. One in five of all preterm babies are born in association with antepartum hemorrhage. Preterm birth, low birth weight and asphyxiated babies are at high risk for mortality, morbidity and developmental problems Thus, the aim of this study was to assess adverse birth outcomes and associated factors among mothers with antepartum hemorrhage in Tigray public hospitals, northern Eth

Severe Maternal Morbidity and near Miss-Events in Women with Heart Disease: Insights from a Cohort Study.

Background/Objectives: The maternal mortality ratio is one of the global health indicators, and cardiopathies are the leading indirect causes of maternal deaths. Proper management of pregnant women with heart disease is crucial, as the severity of these conditions can lead to complications during the perinatal period. This study aimed to evaluate the rate of severe maternal morbidity and associated factors in pregnant women with heart disease. Methods: A retrospective cohort study was conducted at a referral hospital in São Paulo from 2008 to 2017, including pregnant women with heart disease w

Risk factors and maternal-neonatal outcomes of placenta accreta spectrum: A case control study.

Placenta accreta spectrum (PAS) is a rare yet life-threatening obstetric complication. The primary risk factors include placenta previa and … gher rates of postpartum hemorrhage, CS and hysterectomy (P < .001), and adverse neonatal outcomes (perinatal mortality, congenital anomalies, preterm birth, low birth weight, and asphyxia; P < .05). Subgroup analysis revealed grade 3A PAS was most prevalent (89.9%), while grades 3D/3E (2.2% each) showed higher cesarean rates, hysterectomy, newborn asphyxia, and cong …

Comparison of 75 mg versus 150 mg aspirin for the prevention of preterm preeclampsia in high-risk women at a tertiary level hospital: study protocol for a randomized double-blind clinical trial.

BACKGROUND: Hypertensive disorders of pregnancy (HDP) pose significant risks to maternal and fetal health, with substantial mortality and morbidity rates globally, particularly in developing countries. Pre-eclampsia (PE) accounts for a notable portion of maternal morbidity and mortality, with varied prevalence across regions within countries like India. Despite advancements, disparities in healthcare access persist, influencing outcomes. PE not only affects maternal health during pregnancy but also predisposes women to long-term cardiovascular complications, emphasizing the need for early scre

Neurodevelopmental Outcomes in Neonates Surviving Fetomaternal Hemorrhage Compared with a Matched Unexposed Group in a Large Integrated Health Care System.

This study aimed to assess short-term neurodevelopmental outcomes for neonates affected by fetomaternal hemorrhage (FMH) and compare them with an unexposed group.A retrospective cohort analysis was conducted within a large integrated medical system spanning from 2008 to 2018. Neurodevelopmental outcomes of neonatal survivors of FMH were compared with matched controls. Clinically significant FMH in survivors was defined by maternal flow cytometry for fetal hemoglobin result of >0.10% and neonatal transfusion requirement. One unexposed infant was identified for each surviving FMH-exposed infant,

Gestational age at birth, birth weight, and gestational age when intrauterine brain sparing occurs determines the neonatal outcome in growth-restricted infants born before 32 weeks of gestation: a retrospective cohort analysis.

BACKGROUND: Preterm birth and fetal growth restriction are the main determinants of perinatal mortality. In the absence of therapeutic interventions, management is restricted to the observation of fetal growth and fetoplacental perfusion to determine the timing of delivery. Fetal circulatory redistribution, known as "brain sparing," represents a sign of fetal hypoxia and has been implemented in algorithms for when to deliver. In the absence of any other option, the nitric oxide donor pentaerythrityl tetranitrate (PETN), which has been shown to improve fetoplacental flow and reduce preterm birt…

Clinical and epidemiological profile of diabetes mellitus in pregnancy, Isle of Youth, 2008.

INTRODUCTION: Diabetes in pregnancy threatens both maternal and neonatal health, with risks of lung immaturity, fetal macrosomia, dystocia and impaired maternal kidney function. Before insulin, diabetes resulted in maternal mortality rates of 30-50% and perinatal mortality rates of 50-60%. Global prevalence estimates vary by population, geographical area and diagnostic criteria. Diabetes accounts for some 90% of endocrine complications in pregnancy. OBJECTIVES: Determine diabetes mellitus prevalence in pregnancy and characterize the population of diabetic women who gave birth in the Isle of Y…

Maternal mortality, birthweight, and immunogenetics: an evolutionary framework for obstetric risk.

Human childbirth is hazardous compared with other primates, where both perinatal and maternal mortality remain a major global challenge despite advances in neonatal and obstetric care. The concept of the obstetric dilemma frames the evolutionary trade-off between the requirements of bipedal locomotion, which constrains pelvic dimensions, and the need to deliver a neonate with a relatively large cranium. The limits of the maternal pelvis increase the risk of obstructed labor for larger babies, while smaller babies are vulnerable to other causes of mortality such as growth restriction or failure
