Forensic medicine part I. Child abuse: management of physical abuse (children 0-13 years of age).

Child abuse is the intentional maltreatment of a child with the purpose of inflicting injury or harm. The doctor's role in the management of child abuse is to: 1. Recognise or identify the problem. 2. Mobilise the child abuse management team. 3. Get a complete medical history. 4. Do a thorough clinical examination for treatment and medico-legal purposes. 5. Do special investigations as indicated. 6. Treat and refer child appropriately. 7. Provide medico-legal evidence if required (note taking and J88 forms). 8. Notify the Department of Social Services about abuse. 9. Ensure the safety of the c

Patient education for informed decision making about prostate cancer screening: a randomized controlled trial with 1-year follow-up.

PURPOSE: The efficacy of prostate cancer screening is uncertain, and professional organizations recommend educating patients about potential harms and benefits. We evaluated the effect of a videotape decision aid on promoting informed decision making about prostate cancer screening among primary care patients. METHODS: A group of 160 men, 45 to 70 years of age, with no history of prostate cancer, were randomized to view or not to view a 20-minute educational videotape before a routine office visit at a university-based family medicine clinic. The subjects were contacted again 1 year after the

Fecal incontinence: evaluation and treatment.

Fecal incontinence affects men and women of all ages, leading to personal disability and high financial costs. The evaluation of the patient should clarify the pathophysiology of the symptoms and provide guidance in choosing the appropriate treatment. A comprehensive history and physical examination including endoscopic assessment is able to identify the cause of most cases of fecal incontinence. If necessary, functional methods can be used to confirm the diagnosis. Patient selection for suitable treatment is most important and should be based on clinical and physiologic findings. Conservative…

Pediatric ocular trauma and shaken infant syndrome.

Ocular trauma is a leading cause of visual loss in children. Understanding the fundamentals of eye trauma evaluation and management will help decrease associated ocular morbidity and visual loss. Many eye injuries are preventable and should not be viewed as random accidents. The primary care provider can help reinforce the need for protective eyewear during activities associated with ocular trauma. When evaluating infants who may have suffered child abuse, an ocular fundus examination is essential to rule out shaken infant syndrome.

Pediatric gynecology: assessment strategies and common problems.

Pediatric gynecology is an emerging subspecialty involving the collaborative efforts of health professionals from gynecology, pediatrics, and urology. The gynecologic problems encountered in the pediatric population are unique to this age group and involve physician skills differing from those utilized with an adult population. The purpose of this manuscript is to describe the normal anatomy and physiology of the reproductive tract and genitalia and common problems seen in the prepubertal female. Common anatomic variations are considered. Suggestions on assessment techniques for evaluation of

Group A streptococcus and streptococcal toxic shock syndrome: a postpartum case report.

Streptococcal toxic shock syndrome from group A streptococcal disease is rare in the postpartum period, yet it is associated with high morbidity and mortality. Early diagnosis and treatment can lessen complications. Midwives can improve provision of postpartum care by being adequately educated about signs and symptoms of this disease, thereby enhancing their ability to recognize it and obtain the appropriate referral or collaborative medical care. A clinical presentation of postpartum streptococcal toxic shock syndrome, including provider follow-up, is presented.

Determinants of undiagnosed asthma.

BACKGROUND: The conformity between asthma diagnosed in a clinical setting and asthma from an epidemiological point of view has not been much studied. METHODS: History, symptoms, and clinical findings in persons with and without a previous physician diagnosed asthma were studied among persons classified as having asthma according to an epidemiological definition. RESULTS: Smoking was more prevalent in persons not diagnosed as having asthma prior to the study (n = 96), than in those with a previous asthma diagnosis (n = 119), (60% vs. 41%, P = 0.006). Positive skin prick test (SPT) was less co

A clinico-pathological study of complete and partial hydatidiform moles in a Nigerian population.

In a review of 208 cases of hydatidiform moles (HM) managed at the University College Hospital Ibadan, Nigeria between January 1966 and December 1996, there were 105 cases of complete hydatidiform mole (CHM) and 103 cases of partial hydatidiform mole (PHM). The ratio of PHM to CHM was 1:1. The institutional frequency for PHM is one per 667 deliveries, while that of CHM is one per 655 deliveries. In contrast to CHM, the presentation of PHM was less dramatic. The mean age for both types of HM was 28 years, although there was a slightly increased difference in the population of younger patients a

[Nerve entrapment syndrome of the elbow and forearm].

In this study we will discuss entrapment of the median, ulnair, radial and lateral antebrachial nerves of the elbow and the forearm. Compression of the nerves may occur when they traverse a tunnel and an incompatibility exists between the diameter of the tunnel and its contents (e.g. nerves, tendons,...). However, at the elbow and the forearm the nerves are also exposed to particularly dynamic compressions. This is due to anatomical relationships changing between the nerve and its surrounding muscles, tendons and aponevroses during the motion of flexion-extension of the elbow and the prono-sup

Periodontal diseases in children and adolescents: I. Aetiology and diagnosis.

A number of periodontal diseases, some forms of which can be aggressive, can affect children and adolescents. Plaque is the key aetiological agent but local and systemic factors that can modify the response of the periodontal tissues to plaque may be identified from the history and during examination of the child or adolescent. Periodontal screening should be an integral part of the dental examination of younger individuals. It can be used in conjunction with the history and examination to reach a diagnosis, which then forms the basis for future management.

The diagnostic challenge of infective endocarditis: cutaneous vasculitis leading to the diagnosis of infective endocarditis.

BACKGROUND: Signs and symptoms of infectious endocarditis are protean. They result from destruction of cardiac endothelium, metastatic embolization, hematogenous seeding, and immune complex deposition. Embolic manifestations of infectious endocarditis can mimic several other pathologic conditions and make the diagnosis of infectious endocarditis difficult. METHODS: We describe a case of cutaneous vasculitis leading to the diagnosis of infectious endocarditis. A review of the literature highlights the variable clinical presentations and key diagnostic strategies in the evaluation of infectious

Cold-induced urticaria.

Cold-induced urticaria is a form of physical urticaria that develops on cold exposure in susceptible individuals. The majority of cases have an unknown etiology (primary or idiopathic). In rare forms, it is associated with various disease entities (secondary). The workup of a patient includes a history and physical examination, cold challenge, and a battery of laboratory tests. The management of such patients includes precaution and avoidance measures as well as treatment with antihistamines. Life-threatening symptoms necessitate carrying a self-administered injectable epinephrine.

Radiologic case study. Pigmented villonodular synovitis.

Pigmented villonodular synovitis is a benign, locally aggressive disorder characterized by a unique clinical radiographic, and histopathologic presentation. While it is considered to be a rare disorder, a delay in diagnosis and treatment can potentially result in severe disruption of joint function due to subchondral invasion. Once the diagnosis is confirmed, treatment should consist of complete synovectomy. Recurrence is common, but malignant transformation is rare. The etiology of PVNS is still unknown, and perhaps its future discovery will assist in the definitive treatment of this disorder

[Considerations for standardization of assessment of performance capacity in rehabilitation medicine].

Specific difficulties in the assessment of reduced performance capacity in patients with gastroenterologic illness led to the development of a standardized and unified assessment method. The following five steps--exemplified for chronic liver-disease--were designed: (1) determination of disease features (and their graduations), leading to reduced performance capacity; (2) construction of a detailed performance capacity model using more concrete factors (e.g. motivation, cognition, cardiovascular-pulmonary system, etc.); (3) deduction of rules between disease features and the concrete factors o
