The effect of submaximal doses of semaglutide on the expression of micro-RNA in patients with obesity: a pilot study.

INTRODUCTION: Glucagon-like peptide 1 receptor agonists (GLP-1RA) including semaglutide have emerged as some of the most potent drugs in the treatment of obesity. This effect can be mediated by changes in the regulation of micro ribonucleic acid (miRNA) expression. However, currently, there are no data regarding the effect of GLP-1RA on the expression of miRNA in patients with obesity. OBJECTIVE: The aim of this study was to investigate the effect of semaglutide therapy administered at submaximal doses on changes of miRNA in obese patients. METHODS: A pilot, prospective, observational, post-

Endoscopic Diagnosis of Eosinophilic Esophagitis Using a Multi-Task U-Net: A Pilot Study.

PURPOSE: Endoscopically identifying eosinophilic esophagitis (EoE) is difficult due to its rare incidence and subtle morphology. We aimed to develop a robust and accurate convolutional neural network (CNN) model for EoE identification and classification in endoscopic images. MATERIALS AND METHODS: We collected 548 endoscopic images from 81 patients with EoE and 297 images from 37 normal patients. These datasets were labeled according to the four eosinophilic esophagitis endoscopic reference score (EREFS) features: edema, rings, exudates, and furrows. A multi-task U-Net with an auxiliary class

Identification of decision-making criteria for device selection by cochlear implant patients - a pilot study.

PURPOSE: Prior to a cochlear implant (CI) treatment, patients undergo an extensive consultation process. In Germany, patients themselves usually can select a device from one of the offered CI manufacturers. Patients therefore receive a large amount of information, to support the challenging decision-making process. The aim of the study was to evaluate the parameters that are most important to patients during the consultation process for choosing a CI device. METHODS: In total, 41 patients were included in our study. Questionnaires were used to assess the subjective importance of various device

Video Head Impulse Testing for Acute Dizziness in Stroke Protocols: A Prospective Pilot Study.

BACKGROUND AND OBJECTIVES: Dizziness is a common emergency department (ED) concern, yet posterior-circulation strokes can be difficult to detect with standard clinical tools and neuroimaging alone. Quantitative video head impulse test (vHIT) provides an objective bedside measure of vestibulo-ocular reflex function and can help differentiate central from peripheral causes of acute dizziness. METHODS: We conducted a prospective, single-center feasibility and diagnostic-accuracy study of adults (18-90 years) presenting on an ED stroke pathway for acute dizziness. vHIT was performed within 48 hou

A pilot study on the efficacy of cyanoacrylate glue in preventing post-ESD bleeding at the gastric angle.

BACKGROUND: Post-endoscopic submucosal dissection (ESD) bleeding at the gastric angle remains challenging due to the difficulty of closing mucosal defects. This pilot study aimed to evaluate whether prophylactic application of cyanoacrylate glue (CG) reduces bleeding risk in this high-risk location. METHODS: This multicenter, retrospective study included 385 patients who underwent ESD for gastric angle lesions at five institutions between January 2019 and December 2025. Patients were assigned to either the CG group (n = 136) or the conventional group (n = 249). To minimize selection bias, 1:1

Automated Point-Of-Care Assessment Metrics for Surgical Residents Performing Consultations: A Pilot Study.

BACKGROUND: Surgical resident assessment remains largely dependent on faculty observation and subjective evaluation. Electronic health record (EHR) metadata may enable scalable, objective assessment of resident performance during time-sensitive surgical consultations. STUDY DESIGN: We conducted a proof-of-concept study of postgraduate-year-2 general surgery residents performing consultations for adults across three hospitals from July 2020 through June 2025. EHR audit-log data were used to measure consultation duration and medication reconciliation. Performance was compared across residents a

Buprenorphine initiation from fentanyl using low-dose intramuscular ketamine: a pilot study.

BACKGROUND: Buprenorphine is an effective medication for treating opioid use disorder but is underutilized partly due to patient apprehensiveness of the severe withdrawal symptoms it can induce when started, particularly by fentanyl-dependent individuals. An emerging facilitator to buprenorphine initiation is ketamine, a dissociative anesthetic shown to reverse opioid withdrawal symptoms in case reports and small series. A 24-hour behavioral health crisis center implemented a quality improvement protocol to address difficulties transitioning patients from fentanyl to buprenorphine using a high
