A profile of alcohol and prescription drug abuse in a high-risk community-based elderly population.

Substance abuse among the elderly is relatively common but often remains undetected or ignored by health and social workers. Psychosocial and health factors related to the aging process are the major contributors to alcoholism and other drug abuse. It is estimated that between two and ten percent of individuals over the age of 60 suffer from alcoholism. This article profiles alcohol and prescription drug abuse among the geriatric clientele of Elderly Services of Spokane (ESS), a division of the Spokane (Washington) Community Mental Health Center. In addition to traditional channels, ESS uses a

Getting Cancer Is "Just Bad Luck": Exploring Bereaved Emerging and Young Adults' Cancer Risk Uncertainty After Caring for a Parent With Advanced Cancer.

BACKGROUND: Emerging and young adult caregivers (EYACs) who provide care to a parent with advanced cancer are underrepresented in caregiving scholarship, and yet, are not uncommon. Little is known about the psychosocial impacts of caring for a parent at this age or how EYACs manage their uncertainty regarding their own, potentially elevated, future cancer risk. AIMS: To employ Uncertainty Management Theory (UMT) to examine how bereaved EYACs of a parent who died of advanced cancer appraise and manage their uncertainty regarding their personal cancer risk. METHODS: We conducted a secondary an

A European Delphi study on the management of anaemia of CKD: unmet needs in personalized treatment.

BACKGROUND: Anaemia is a common complication of chronic kidney disease (CKD) impacting clinical outcomes and quality of life (QoL). Despite available treatment options, unmet needs exist, especially in relation to QoL. We aimed to develop a consensus of European nephrologists' opinion on the management of anaemia of CKD. METHOD: Seventy-six nephrologists from seven European countries participated in a modified Delphi panel, consisting of two survey rounds followed by an online consensus meeting. Consensus was defined as ≥ 80% agreement or disagreement. The responses were initially assessed ag

Update on Perioperative Medication Management for the Hand Surgeon: A Focus on Diabetes, Weight Loss, Rheumatologic, and Antithrombotic Medications.

Diabetes mellitus and obesity are growing health concerns. New pharmacologic interventions have recently begun to play a more notable role in the treatment pathway of these separate but related conditions. In particular, glucagon-like peptide-1 receptor agonists, such as semaglutides (Ozempic, Wegovy) and tirzepatide (Mounjaro), and sodium glucose co-transporter 2 inhibitors, such as dapagliflozin (Farxiga) and empagliflozin (Jardiance), have emerged as treatment options. Multiple clinical trials have demonstrated their efficacy in regulating metabolism, improving glycemic control, and managin

Management of occupational asthma.

The importance of occupational asthma and its management is usefully set in perspective by considering recent trends of increasing asthma incidence, morbidity and mortality in the population at large. The contribution to incidence made by asthma of occupational origin is of the order 20-100 cases per million workers per year; the individual worker's lifetime risk approaching 5% in some industrial environments. Management of the affected industry inevitably follows different pathways from that of the affected individual, though both need to start from a definitive diagnosis. For the affected in

Parents' experiences of psychotherapeutic support on the neonatal unit: A mixed methods systematic review to inform intervention development for a multicultural population.

BACKGROUND: Parents of infants admitted to neonatal intensive care require support to minimize the impact on their mental health and to encourage engagement with their infants to support infant neurodevelopment. Many interventions aim to address this need, but there is a lack of research considering the accessibility of these for a multicultural population. AIM: To systematically identify sources of psychotherapeutic support available for parents with infants admitted to neonatal care (NNU, neonatal intensive care unit [NICU] and special care units), assess their accessibility and acceptabili

Exploring Competencies and Skills of Australian Rural Psychologists.

BACKGROUND: The ratio of psychologists to the population is vastly lower in rural communities compared to urban areas, and more clinicians are critically needed in rural Australia. Other health disciplines have implemented generalist frameworks and training to encourage and prepare clinicians to remain in rural practice. Research often references rural psychology practice in conjunction with the broader allied health discipline. OBJECTIVE: Though prior research has identified skills needed for rural psychology practice, further research is needed to expand and update the current understanding

Oral microbiota and urinary system diseases: from mechanistic insights to clinical implications-a comprehensive review.

The human oral microbiome has attracted considerable attention due to its role in oral health and potential implications for systemic diseases. Oral microbes provide real-time insights into health and disease status, making them valuable for early disease risk stratification and treatment outcome prediction. Accumulating evidence indicates that oral microbiota contribute to the pathogenesis of urinary system diseases. Notably, in pediatric populations, the oral microbiome-shaped by age, feeding patterns, and immune maturation-may modulate susceptibility to renal-related systemic conditions; cl

Bedaquiline, Pretomanid, Linezolid, and Moxifloxacin: Mechanisms of Action, Drug Interactions, Adverse Effects and Use in Special Situations.

Tuberculosis (TB) remains a critical global public health challenge, requiring therapeutic strategies that ensure high cure rates while minimizing bacillary transmission. The 2022 World Health Organization (WHO) update for drug-resistant TB treatment prioritized a novel, 6-month, all-oral regimen composed of bedaquiline, pretomanid, linezolid, and moxifloxacin (BPaLM) as the preferred treatment for rifampicin- and multidrug-resistant tuberculosis (RR-/MDR-TB). However, the clinical success of this shortened therapy is intrinsically linked to managing complex drug-drug interactions and treatmen

[Current status and future perspectives on age-related hearing loss and cognitive impairment].

Objective:Age-related hearing loss(ARHL) is one of the most common sensory degenerative disorders in the elderly, characterized by high prevalence, insidious onset, and progressive deterioration. Recent studies indicate that ARHL not only impairs communication and quality of life in older adults, but is also significantly associated with the development of cognitive impairment and Alzheimer's disease. However, its underlying pathogenesis remains incompletely understood, and the relationship between clinical phenotypes of ARHL and cognitive decline has yet to be clearly defined. ARHL occupies a
