Predictors of gastroesophageal reflux disease revisited: a novel gastroesophageal reflux disease clinical prediction score.

Symptom assessment and proton pump inhibitor (PPI) trial are limited in predicting pathologic gastroesophageal reflux disease (GERD) on pH testing and endoscopy, which has led to PPI overuse. We aimed to develop a novel scoring system using demographic factors, clinical factors, and symptoms to predict pathologic GERD. Adult patients with GERD were prospectively enrolled into a Heartburn Center Registry from April 2019 to September 2021. Demographics, patient responses to validated questionnaires, results of endoscopy and pH testing were collected. We selected variables associated with patholo

Exploring small intestinal bacterial overgrowth in functional upper gastrointestinal disorder: A comprehensive case-control study.

BACKGROUND: Small intestinal bacterial overgrowth (SIBO) is suspected and excluded frequently in functional gastrointestinal (GI) disorders. Children presenting with various esophago-gastro-duodenal (upper GI) symptoms are rarely subjected to investigations for SIBO. AIM: To estimate the frequency of SIBO in children having functional upper GI symptoms (as cases) and to compare the result of the SIBO status to that of the controls. METHODS: Children aged 6 to 18 who presented with upper GI symptoms were selected for the study. All children were subjected to upper GI endoscopy before being ad

Belgian consensus on the management of patients with functional dyspepsia.

BACKGROUND: Functional dyspepsia (FD) is a disorder of gutbrain interaction characterised by epigastric pain, epigastric burning, early satiation or postprandial fullness. Despite its high prevalence, clinicians struggle with the diagnosis and management of these patients. METHODS: A Delphi consensus was conducted by 20 experts from Belgium, and consisted of a literature review, summarising the existing evidence, and a voting process on 119 statements. Grading of recommendations, assessment, development and evaluation criteria were applied to evaluate the quality of evidence. Consensus was de

Granulomatous gastritis due to gastric sarcoidosis presenting as gastric outlet obstruction: Case report of a rare entity.

INTRODUCTION: Granulomatous gastritis (GG) is a rare inflammatory condition affecting the stomach with various aetiologies and non-specific presentation. Sarcoidosis as a cause of GG is even rarer. We describe a rare case of gastric outlet obstruction caused by granulomatous gastritis, the aetiology of which was later identified as sarcoidosis. CASE PRESENTATION: A 60-year-old female presented with postprandial non-bilious, non-projectile vomiting with loss of appetite and unintentional weight loss. Physical examination revealed significant dehydration, tachycardia, a tender and distended abd

Uncovering Sternoclavicular Arthritis, Suspected Pseudogout, in a Fever of Unknown Origin by Whole-Body MRI.

An 89-year-old male developed a persistent high fever (around 39 °C) approximately two weeks following endoscopic reduction of sigmoid volvulus. He had no history of hypercalcemia but was using diuretics and proton pump inhibitors. Renal and thyroid status were normal. He was largely bedridden and asymptomatic except for fever. Laboratory tests demonstrated elevated C-reactive protein (4.75 mg/dL), but some tumor markers (including CEA, CA19-9, and CA125), anti-nuclear antibodies, MPO-ANCA, PR3-ANCA, β-D-glucan, and interferon-gamma release assay were all negative. Urinalysis was unremarkable.

Prognostic Value of the Magnesium Depletion Score for Mortality Outcomes Among NAFLD Patients.

BACKGROUND: The magnesium depletion score (MDS), a novel clinical score, incorporates alcohol consumption, kidney disease, use of diuretics and proton pump inhibitors (PPIs) to assess magnesium levels. However, the prognostic significance of the MDS individuals with nonalcoholic fatty liver disease (NAFLD) remains uncertain. This research aimed to explore the relationship between the MDS and mortality outcomes in NAFLD patients, including all-cause mortality, cancer mortality, and cardiovascular disease (CVD) mortality. METHOD: Data acquired on 16,394 NAFLD patients from the National Health a

Micronutrient Status in Patients with Short Bowel Syndrome Weaned off Parenteral Support.

Background/Objectives: In short bowel syndrome adults (SBS), about 50% of patients on parenteral support (PS) are weaned off. However, micronutrient deficiency prevalence has never been studied in weaned patients. We aimed to assess the prevalence of micronutrient depletions and potential risk factors, more than a year after PS weaning. Methods: A retrospective study was conducted on our 161 weaned SBS patients between January 2011 and December 2021. Only 42 of them had an assessment of their plasma micronutrient status more than a year after PS weaning. Results: 40/42 patients had at least on

Perforated Duodenal Ulcer 24 Years After Roux-en-Y Gastric Bypass: A Rare Presentation With Pneumoperitoneum.

Perforated duodenal ulcers post Roux-en-Y gastric bypass (RYGB) are rare, with fewer than 30 documented cases. Diagnosis can be challenging due to the absence of pneumoperitoneum on cross-sectional imaging, seen in only five published cases.  A 67-year-old female patient, 24 years post RYGB, presented with diffuse abdominal pain, fever, hypotension, and tachycardia following recent nonsteroidal anti-inflammatory drugs (NSAIDs) use. Laboratory findings showed leukopenia and elevated lipase. Computed tomography (CT) imaging revealed pneumoperitoneum, prompting emergent exploratory laparotomy. A

Multiple Endocrine Neoplasia Type 1 Presenting as Recurrent Overt Gastrointestinal Bleeding and Ulceration: A Diagnostic Challenge.

Multiple endocrine neoplasia type 1 (MEN1) is a multisystem endocrine disorder marked by tumors of the parathyroids, pancreatic islet cells, and pituitary gland. Although most cases are inherited through an autosomal dominant pattern, a proportion arises through sporadic mutation. As the clinical presentation is highly variable, it often leads to diagnostic delay in patients with de novo mutations. We report a 48-year-old man with no family history and a past medical history of renal calculi with intermittent hematuria. Over several months, he had multiple hospitalizations due to progressive e

Rationale and Trial Protocol for a Double-Blinded Randomized Controlled Trial to Assess the Impact of a Concomitant Crural Repair during Laparoscopic Sleeve Gastrectomy in Patients with a Lax Gastroesophageal Junction without Frank Hiatal Hernia: REPAIR Trial Protocol.

INTRODUCTION: Laparoscopic sleeve gastrectomy (LSG) is associated with postoperative gastroesophageal reflux disease (GERD) and erosive esophagitis (EE). The role of crural repair during LSG is still controversial. The preoperative laxity of the gastroesophageal junction (GEJ), graded by Hill's classification, is more predictive for postoperative GERD and EE after LSG than the presence of a hiatal hernia seen on endoscopy. Thus, the authors hypothesize that a concomitant crural repair in a specific subgroup of patients with a lax GEJ (Hill's III) may reduce the incidence of postoperative GERD

Endoscopic and clinical features of gastric emphysema.

Gastric emphysema is characterized by the presence of intramural gas in the stomach without bacterial infection. Due to its rarity, most reports on gastric emphysema have been limited to single-case studies, and this condition's clinical and endoscopic features have not been thoroughly investigated. In this study, we analyzed 45 patients with gastric emphysema from 10 institutions and examined their characteristics, endoscopic features, and outcomes. The mean age at diagnosis of gastric emphysema in our study population (35 males and 10 females) was 68.6 years (range, 14-95 years). The top fiv
