LAMP-LFD: establishment and evaluation of a new diagnostic platform for SARS-CoV-2 rapid detection.

BACKGROUND: This study aimed to establish a rapid detection method for SARS-CoV-2 by integrating loop-mediated isothermal amplification (LAMP) with lateral flow dipstick (LFD) technology. METHOD: Specific primers targeting the nucleocapsid (N) gene and open reading frame 1ab (ORF1ab) gene of SARS-CoV-2 were designed and labeled with 6-FAM and biotin, respectively. After systematically optimizing key reaction parameters, including primer selection, primer concentration, and LAMP reaction time, the sensitivity, specificity, and clinical applicability of the method were evaluated. RESULTS: The …

Serological Benefit of SARS-CoV-2 Vaccination Relative to Infection in Children With Acute Lymphoblastic Leukemia.

BACKGROUND: Children with acute lymphoblastic leukemia (ALL) are at risk of severe outcomes from SARS-CoV-2 (SCV2). In the post-pandemic context, where most children have been infected with SCV2, there are limited data on whether vaccination remains beneficial in children with ALL. PROCEDURE: We recruited children with ALL, mostly in the maintenance phase of chemotherapy, and healthy controls who were infected and/or vaccinated with SCV2. Antibody concentrations and avidity against the SCV2 spike (S) protein and its receptor-binding domain (RBD) were measured, as were the frequencies of S-spe…

Influence of Sampling Strategies and Disease Prevalence on SARS-CoV-2 Detection Dynamics in Wastewater Surveillance.

BACKGROUND: Wastewater-based surveillance (WBS) has emerged as a valuable tool for population-level monitoring of Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) transmission, yet the interplay between sampling strategies and disease prevalence in shaping detection performance remains ambiguous. We investigated how grab and composite sampling influence SARS-CoV-2 ribonucleic acid (RNA) detection dynamics and predictive lag times across high- and low-prevalence communities in Selangor, Malaysia. METHODS: A 28-week longitudinal study was conducted in Selangor, Malaysia, comparing g…

Wastewater-Based Surveillance of Respiratory Viruses in the SARS-CoV-2 Post-Pandemic Period in Mexico.

In recent years, wastewater (WW)-based epidemiology has been increasingly used for surveillance of SARS-CoV-2 and has emerged as a potential tool for monitoring other respiratory viruses. Most evidence on the use of WW for detecting multiple respiratory viruses comes from developed countries. In this study, we assessed the feasibility of multi-respiratory virus sewage surveillance in a middle-income country and explored signals that may be potentially used as early warning signs for Public Health authorities. We examined the presence of SARS-CoV-2, influenza virus, respiratory syncytial virus …

Memory B cell development in response to mRNA SARS-CoV-2 and nanoparticle immunization in mice.

Nanoparticle immunogens excel at rapidly inducing high levels of circulating antibodies and are being deployed as part of several novel vaccines. However, their ability to elicit memory B cell responses is less well understood. Here, we compared serologic and memory B cell responses after prime boost vaccination with either SARS-CoV-2 Wuhan-Hu-1 mRNA vaccine, or protein nanoparticles: SARS-CoV-2 B.1.351 homotypic containing a single receptor binding domain (RBD); (homotypic beta) or a combination of different Sarbecovirus RBDs (mosaic 8b), respectively. The memory B cells elicited by the 3 vac…

In Search of the Most Significant Potential G-Quadruplexes in SARS-CoV-2 RNA: Genomic Analysis.

G-quadruplexes (G4s) are emerging as potential antiviral targets. SARS-CoV-2 genomic RNA contains 42 G-rich regions harboring putative G-quadruplex-forming sequences (PQSs). Here, we performed a systematic genomic and structural analysis of SARS-CoV-2 PQSs. It was proposed that non-G-tetrads or different triads may stabilize most G4s in this RNA. Many G4s may include the most stable U·A-U triad. Several G-quadruplexes may be significantly stabilized by 3' U-tetrad. Large-scale mutational analysis of RNA structural elements containing PQSs showed that most PQSs are highly conserved, while persi…

Preferential remdesivir triphosphate incorporation by SARS-CoV-2 polymerase is altered to ATP by the S759A mutation.

Nucleoside analogs are successful in treating viral infections. dNTP analogs are primarily DNA chain terminators, while NTP analog remdesivir can inhibit RNA synthesis by delayed chain termination or when embedded in the template strand. Here, enzymatic assays, mass spectrometry, and cryo-EM structures demonstrate that SARS-CoV-2 RNA-dependent RNA polymerase (RdRp) preferentially incorporates remdesivir triphosphate (RTP), outcompeting 10-fold excess ATP; however, successive RTP incorporations are disfavored when ATP is present. The RdRp structures demonstrate that 1'-cyano-imposed conformatio…

Impact of timely vaccination and genomic surveillance on controlling consecutive waves of SARS-CoV-2 variants.

During the global COVID-19 pandemic, different SARS-CoV-2 variants of concern (VOCs) emerged, causing massive transmission waves that affected infection outcomes, economies and public health. From January to July 2021 in Uruguay, the transmission wave was primarily driven by the Gamma VOC amidst a largely immunologically naive population. Following the implementation of a nationwide vaccination campaign, the detection of the Delta VOC in July 2021 did not trigger a significant transmission wave, even in the face of increased mobility during the winter holidays (late June to mid-July). We prese…

Structural analysis of the flexibility of the Ubl2 domain within the papain-like protease of SARS-CoV-2.

The papain-like protease (PLpro) of SARS-CoV-2 is part of the multi-domain nonstructural protein 3 (NSP3) and consists of two domains: a ubiquitin-like domain 2 (Ubl2) and a protease domain. PLpro plays a crucial role in the replication cycle of SARS-CoV-2, facilitating host immune-system evasion and the formation of double-membrane vesicles where replication occurs. While the function of the Ubl2 domain is still not clear, it is critical for the stability and the functional efficiency of PLpro. Despite its predicted inherent flexibility, nearly all SARS-CoV-1 and SARS-CoV-2 PLpro crystal stru…

Clinical Manifestations and Immunological Impacts in Co-Infections and Sequential Infections of SARS-CoV-2 and Arboviruses.

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) and arboviruses represent major global public health threats, each with distinct epidemiological and pathogenic profiles that often overlap in endemic regions. Reports of co-infection and cross-reactivity between these viruses highlight the need for deeper understanding of their interactions. Co-infections pose diagnostic challenges due to overlapping clinical symptoms and immune modulation, which may exacerbate disease severity. Moreover, antibodies generated from prior virus infections may influence the outcome of subsequent infect…

Clinical predictors of SARS-CoV-2 vaccine immunogenicity in kidney transplant recipients at a rural center.

BACKGROUND: Kidney transplant recipients (KTR) have impaired immune responses to vaccination and remain at increased risk for severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection despite mRNA coronavirus disease 2019 (COVID-19) vaccination. Data from rural and medically underserved transplant populations remain limited. AIM: To evaluate serologic response to a two dose mRNA SARS-CoV-2 vaccination series and identify clinical factors associated with antibody response in KTR. METHODS: This single center retrospective observational study included adult KTR who completed a two …

Longitudinal assessment of SARS-CoV-2 spike and nucleocapsid antigenemia in the ORCHESTRA post-COVID cohort.

OBJECTIVES: Spike antigenemia has been proposed as a mechanism underlying post-COVID condition (PCC). We determined whether persistent free SARS-CoV-2 spike or nucleocapsid antigenemia is associated with PCC or systemic inflammation. As a secondary methodological aim, we evaluated whether dithiothreitol (DTT) treatment reliably unmasks antibody-bound spike. METHODS: Within the ORCHESTRA project, we quantified free spike and nucleocapsid in 200 participants (112 PCC, 58 non-PCC, and 30 prepandemic controls), analysing 576 samples collected during acute infection (n = 99) and at 3, 6, 12, and 1…

The Impacts of the SARS-CoV-2 Pandemic on Incarceration Dynamics and Racial Disparities in North Carolina Jails.

Disruptions to the US criminal legal system during the SARS-CoV-2 pandemic temporarily reduced the numbers of people in prisons while exacerbating racial disparities in their populations, increasing the percentage of incarcerated people who are Black. Less is known about jail populations, including disparate impacts by race. We used web-scraped data from the publicly available rosters of North Carolina (NC) jails to examine their population sizes, admission and release rates, and racial composition during 2020-2022. Data were available from 45 jails (of 93 total in NC) representing 194,230 inc…

Screening and Evaluation Scheme for the Identification of Antisense Oligonucleotides with Antiviral Activity Against SARS-CoV-2.

Antisense oligonucleotides (ASOs) are advantageous for the development of antiviral drugs against respiratory viruses. However, the methodology for efficiently identifying antiviral ASOs has not been well established, and to date no ASOs have successfully been developed for treating viral respiratory diseases. In this chapter, we describe a scheme that consists of three stepwise evaluation assays to identify ASOs having antiviral effects against severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), the causative pathogen of coronavirus disease 2019 (COVID-19). The primary evaluation em…
