PURPOSE: Traumatic shoulder dislocation may be complicated by concomitant bony injury of the glenoid rim or the humeral head. In Hill-Sachs lesions, reconstruction techniques vary widely and range from open reduction to tendon transposition or humeral head derotation. These operations are extensive and have questionable outcomes. With the expertise from vertebral compression fracture reduction by kyphoplasty, we examined in a cadaver feasibility study whether reduction of the Hill-Sachs lesion via hydraulic lift might be an anatomical and minimally invasive treatment option. We postulated that

Biodegradable anchors were designed to provide secure fixation while allowing for later resorption and replacement by host tissue. First-generation implants degraded relatively rapidly and caused foreign-body reactions, synovitis, fragmentation, and osteolysis. Newer implants have similar complications. It is not known if the primary cause of the osteolysis is biological (precipitated by breakdown products of the polymer) or mechanical (caused by initial loss of implant stability). Case reports have described glenoid osteolysis around biodegradable suture anchor placement for shoulder stabiliz

OBJECTIVE: To evaluate functional outcome after hemiarthroplasty for displaced proximal humeral fractures and to review whether prosthesis type, intraoperative technique or previous ipsilateral shoulder surgery could affect the outcome.
METHODS: We reviewed the medical records and radiographs of patients who had undergone hemiarthroplasty for proximal humeral fractures between 1992 and 2000. We identified 45 patients, 39 with acute fractures and 6 with fracture-related complications. One surgeon performed 17 surgeries (38%), and the rest were carried out by 11 other orthopedic surgeons. Open periarticular shoulder fractures present a tremendous challenge for orthopedic surgeons. These injuries, albeit rare, are typically caused by high-energy mechanisms and are associated with insult to multiple organ systems resulting in high morbidity and mortality. Although the civilian trauma literature includes several articles on outcomes of closed periarticular shoulder fractures, only 1 peer-reviewed article has focused on this specific open injury pattern. No standard management technique has been adopted for these injuries, and treatment patterns have anecdotally evolved from war to

It is difficult to predict the outcome of comminuted fractures of the proximal humerus and give guidelines for optimal treatment. Different treatment modalities are available, and there are various scoring systems for postoperative evaluation. The aim of this study was to assess the functional outcome after three- and four-part fractures of the humeral head in 38 patients, with a 3-year follow-up. Twenty-eight patients were treated conservatively, seven were operated on with open reduction and internal fixation, and three received a hemiarthroplasty. All patients were examined clinically and r

BACKGROUND: Breast cancer can strike women at any age after puberty, though its incidence increases with age. The WHO's Global Breast Cancer Initiative (GBCI), between 2020 and 2040, aims to prevent 2.5 million breast cancer deaths globally. This study aimed to assess the effect of a nurse-led intervention on shoulder dysfunction among breast cancer patients undergoing modified radical mastectomy (MRM) - a randomized controlled trial.
METHODS: A total of 320 breast cancer patients undergoing MRM (Study group-160, Control group-160) who met the inclusion criteria were included in the study. Bl

BACKGROUND: While reverse shoulder arthroplasty (rTSA) is commonly utilized for rotator cuff tear arthropathy, indications have expanded to include, primary glenohumeral osteoarthritis (GHOA) with intact cuff. The presence of an intact cuff may influence outcomes after rTSA because preserved cuff musculature can contribute to shoulder stability and force which could potentially improve postoperative function and reduce complication rates. However, studies have reported contradictory results on whether or not an intact cuff would provide better outcomes in patients receiving an rTSA.

BACKGROUND: Bankart lesions and Hill-Sachs lesions can be detected in up to the majority of recurrent dislocations. This study aims to develop a grading system for Hill-Sachs lesions and assess its association with Bankart lesions. The proposed grading system, based on the dimensions of Hill-Sachs lesions, offers a standardized approach for classifying these defects, thereby facilitating the comparison of research findings across studies. Additionally, this study tends to estimate the highly significant association between Hill-Sachs lesions and the presence of Bankart lesions compared to prev

OBJECTIVE: The number of shoulder arthroplasties is increasing along with the need for revision surgeries. Determining the stability of the implant is crucial in preoperative planning. This study aims to investigate whether radiolucent lines (RLL) in preoperative radiographs predict component loosening.
MATERIALS AND METHODS: Preoperative radiographs of 93 cases in 88 patients who underwent shoulder arthroplasty revision were evaluated regarding the presence of RLL. Correlation analyses were performed for radiographic findings and demographic factors (age, gender, BMI, prior surgeries) compar

BACKGROUND: Shoulder arthroplasty is increasingly performed for patients with symptoms of glenohumeral arthritis. Advanced imaging may be used to assess the integrity of the rotator cuff preoperatively because a deficient rotator cuff may be an indication for reverse shoulder arthroplasty (RSA) rather than anatomic total shoulder arthroplasty (TSA). However, the cost-effectiveness of advanced imaging in this setting has not been analyzed.
QUESTIONS/PURPOSES: In this cost-effectiveness modeling study of TSA, all patients underwent history and physical examination, radiography, and CT, and we c

BACKGROUND: Staged bilateral shoulder arthroplasty procedures have been shown to have good functional outcomes. The next step is to explore the option of simultaneous bilateral shoulder arthroplasty (SBSA). We report on the first case series of SBSA in the United States. The purpose of this study was to examine the safety and postoperative complication profile of SBSA and provide a technique reference for surgeons considering performing or investigating this procedure.
METHODS: We conducted a retrospective record review of all the SBSA procedures performed by the senior author between 2007 an

BACKGROUND: The primary purpose of this study was to compare 2-year anatomic total shoulder arthroplasty (TSA) functional and radiographic outcomes between Walch type A and B glenoids treated with an all-polyethylene glenoid designed for hybrid fixation with peripheral cement and central osseous integration. The secondary purpose was to evaluate outcomes based on central peg technique. The hypotheses were that there would be no difference in short-term radiographic or functional outcome scores based on preoperative glenoid morphology or central peg technique.
METHODS: We performed a multicent

BACKGROUND: Cutibacterium acnes is the main pathogen in periprosthetic shoulder infections. In acne vulgaris therapy, benzoyl peroxide-miconazole nitrate cream effectively reduces the superficial C acnes burden of the skin. Its additional potential in the subcutaneous and capsular layers (eg, for prevention of future periprosthetic shoulder infections) is unknown. The aim of this study was to investigate the efficacy of a topical acne vulgaris cream (benzoyl peroxide-miconazole nitrate) to reduce subcutaneous and capsular C acnes in individuals with C acnes skin colonization undergoing open sh

BACKGROUND: Irreparable subscapularis (SSc) tears alter the dynamic force coupling of the shoulder, resulting in pain, weakness, and impaired shoulder function. Pectoralis major (Pma), pectoralis minor (Pmi), and latissimus dorsi (LD) transfers are treatment options for irreparable SSc tears, but clinical outcomes vary. The purpose of this study was to compare the biomechanical properties of Pma, Pmi, and LD transfers in an SSc-deficient shoulder using a computational model.
METHODS: A computer shoulder model was used to investigate the moment arms of Pma, Pmi, and LD tendon transfers compare

Inflammation of the long head of the biceps tendon is a common cause of shoulder pain. Bicipital peritendinous effusion (BPE) is the most common biceps tendon abnormality and is related to various shoulder injuries. Physicians usually use ultrasound imaging to grade the inflammation severity of the long head of the biceps tendon. However, obtaining a clear and accurate ultrasound image is difficult for inexperienced attending physicians. To reduce physicians' workload and avoid errors, an automated BPE recognition system was developed in this article for classifying inflammation into the follo

BACKGROUND: An understanding of the average range of movement of the shoulder that is normally achievable is an important part of treatment for shoulder disorders. The average range of active shoulder flexion, abduction and external rotation was measured in a population cohort aged 20 years and over without shoulder pain and/or stiffness in order to provide normative shoulder range data.
METHODS: Cross-sectional analysis using participants in a community-based longitudinal cohort study. There have been three stages of data collection - Stage 1 (1999-2003), Stage 2 (2004-2006) and Stage 3 (200
