In this study we investigated the relationship between the size of adenoids and/or tonsils and obstructive sleep apnea syndrome (OSAS) in 19 children aged 3 to 7 years. After clinical examination by an ENT specialist, electrocardiogram (ECG), vectorcardiogram (VCG) and echocardiogram (EC) were performed while the patients were awake, and static charge sensitive bed (SCSB) method recording (noninvasive monitoring of ballistocardiogram, respiration and body movements) while asleep. After adenoidectomy and/or tonsillectomy, the volume of the tonsils and the adenoids was determined. RVH was found …

Nine males with sleep apnea DOES syndrome and three males with sleep apnea DIMS syndrome were treated with prosthetic mandibular advancement (PMA). The method uses a prosthesis, which is designed to advance the mandible 3-5 mm to prevent upper airway occlusion during sleep. The apnea index in the obstructive-type apnea and the percentage of time spent in obstructive apnea decreased significantly with PMA. Although the apnea index showed merely a tendency to decrease in central apnea (p < 0.1), the percentage of time spent in central apnea decreased significantly with PMA. A marked improvement …

Sleep obstructive apnea syndrome (SOAS) is a common condition with a strong male predominance. Its incidence is more than 1 percent in the population as a whole. It exists in snorers. Both snoring and SOAS are linked to the presence of abnormalities (congenital or acquired) of the upper respiratory tract. The nocturnal cardiovascular consequences of SOAS are directly linked to apnea. Bradycardia occurs during apnea and tachycardia when ventilation restarts. Paroxysmal nocturnal hypertension is a constant feature. Even in individuals who are normotensive during the day, each restarting of venti…

To test whether active hyperventilation activates the "afterdischarge" mechanism during non-rapid-eye-movement (NREM) sleep, we investigated the effect of abrupt termination of active hypoxia-induced hyperventilation in normal subjects during NREM sleep. Hypoxia was induced for 15 s, 30 s, 1 min, and 5 min. The last two durations were studied under both isocapnic and hypocapnic conditions. Hypoxia was abruptly terminated with 100% inspiratory O2 fraction. Several room air-to-hyperoxia transitions were performed to establish a control period for hyperoxia after hypoxia transitions. Transient hy…

The prevalence of periodic limb movements in sleep (PLMS) in a randomly selected elderly sample is reported. In San Diego, 427 elderly volunteers aged 65 yr and over were recorded in their homes. Forty-five percent had a myoclonus index, MI greater than or equal to 5. Correlates of PLMS included dissatisfaction with sleep, sleeping alone and reported kicking at night. Although statistically significant, the strengths of the associations between interview variables and myoclonus indices were all small. No combination of demographic variables and symptoms allowed highly reliable prediction of PL…

Polysomnography, including electroencephalography, electromyography and electro-oculography was performed in three patients with palatal myoclonus (PM). The amplitude of the myoclonus decreased during sleep. The frequency did not change during non-REM sleep, but increased during REM sleep in two patients. Ocular myoclonus synchronized with PM disappeared during deep sleep stages in two patients and reappeared during REM sleep in one of them. In the other patient, ocular myoclonus was noted only in REM sleep, being absent even when the patient was awake. All patients showed episodic EEG activit…

This study describes a prediction method for central apnea episodes of newborns during sleep. The cross-correlation between heart rate and respiration obtained from newborns is calculated. Employing the cross-correlation, the displacement between heart rate and respiration before central sleep apnea episodes was much larger than the displacement in the stationary state. As the result, it is possible to predict central sleep apnea episodes several minutes before their occurrence.

Several types of neurologic diseases can contribute to disturbed respiration during sleep. Abnormal function of respiratory and upper airway muscles can occur with neuromuscular diseases and lead to upper airway obstruction and alveolar hypoventilation during sleep. Central nervous system disorders that cause dysarthria and dysphagia can also lead to obstructive and central sleep apnea, even in the absence of the usual anatomic deformities associated with OSA. Appropriate treatment requires an understanding of the effect of the underlying disease process on upper airway and respiratory functio…

We studied the mechanisms of occurrences of apneas and bradyarrhythmias during sleep in five Wistar-Kyoto rats. We recorded electroencephalograms, electrocardiograms, chest wall movement, and diaphragmatic electromyograms (EMGdi) for three continuous days in each freely moving rat and demonstrated that: 1) 99% of the apneas and 99% of the bradyarrhythmias occurred during paradoxical sleep (PS), 2) 98% of the apneas were due to spontaneous cessations of respiratory drive, 3) the percentages among apneas accompanied with bradyarrhythmias were only about 30% and independent of the apneic duration…

Myotonic dystrophy is a genetic disorder inherited as an autosomal dominant trait. It is known to be associated with endocrine dysfunction, … respiratory function parameters and PaO2 were partly responsible for the hypoxemia observed during sleep in patients with MYD. Overnight polysomnography showed that 9 of the 10 patients with MYD and 1 of the 5 patients with LG presented apneas during sleep, particularly during REM, stage 1 and stage 2. Almost all apneas were central type, with a low pe …

Evidence exists that strongly suggests that an imbalance in both the magnitude and timing of electrical activity between the upper airway and chest wall inspiratory muscles can exist during sleep. Too little or delayed activity of upper airway inspiratory muscles is associated with upper airway narrowing or collapse in patients with obstructive apnea. Preferential activation of upper airway muscles is associated with a decrease in upper airway resistance or in reopening of the airway following an apnea. These interrelationships between the upper and lower respiratory muscles may be exacerbated…

Apnea in sleeping patients with syringobulbomyelia can be accounted for by damages in afferent and efferent impulsation from the respiratory centre due to cysts in medulla oblongata and spinal channel distension. Controlled lung ventilation has been applied to such patients in sleep for more than 8 years. In Arnold-Chiari syndrome apnea in sleep is associated with paroxysms of occlusive hydrocephalus. Periodic controlled lung ventilation during sleep is obligatory in these patients. Both nosological forms are characterized by lower sensitivity of the respiratory centre to hypoxia and hypercapn…

An impaired ability to concentrate, loss of intellectual performance, and changes in personality are frequently-mentioned psychological symptoms of sleep apnoea. Apnea-associated disturbances of sleep structure as well as nocturnal cerebral hypoxia are possible causes. Twenty men and two women with an average age of 51.5 years underwent psychological testing. The average apnea index was 36 (range 11 to 92). The following psychodiagnostic procedures were used for screening; attention-strain test (d2) to measure short term concentration ability, numbers-connecting test (ZVT) for the evaluation o…

The pathophysiologic results of respiratory disturbances caused by adenoid-tonsillar hypertrophy are reported. In each case, we monitored intraesophageal pressure to accurately assess respiratory effort, and recorded a sleep diary to detect disorganized sleep habits and disturbed sleeping-waking rhythms. We noticed that great respiratory effort existed not only during light sleep, but was also continuously observed during deep sleep despite the total disappearance of obstructive apneas. Furthermore, upper airway obstruction caused by adenoid-tonsillar hypertrophy and hypoxemia during sleep in …

The electroencephalographic sleep profile of a group of recurrent depressives who had been depressed for less than four weeks was compared with their sleep profile in a prior episode of depression. The findings in these 19 cases indicate that early in the episode, rapid eye movement (REM) sleep findings are more abnormal, including shortened REM latency, REM sleep percent, and REM activity. Other sleep variables, such as sleep continuity measures and decreased delta-wave sleep, are abnormal in a similar fashion in both episodes. The results are not explainable on the basis of clinical severity…
