An unusual occurrence of chondromyxoid fibroma with secondary aneurysmal bone cyst in the cervical spine.

BACKGROUND CONTEXT: Chondromyxoid fibroma (CMF) and aneurysmal bone cysts (ABCs) are rare bone tumors and even rarer in the spine. To date, no report has been made of CMF with secondary ABC in the cervical spine. PURPOSE: The purpose of this study was to describe the diagnosis and surgical treatment of a case of CMF with secondary ABC of C6, a rare occurrence in an uncommon location. STUDY DESIGN: The study design is a case report. METHODS: A 27-year-old woman presented with numbness with paresthesias of the right upper extremity. Diagnostic imaging revealed diffuse enlargement of the right…

A comparison of cervical spine movement during laryngoscopy using the Airtraq or Macintosh laryngoscopes.

The Airtraq laryngoscope has an oropharyngeal airway-shaped blade that provides a non-line-of-sight view of the glottis. The configuration of the blade should mean that less movement of the cervical spine is required during laryngeal visualisation. We compared the degree of cervical spine movement in laryngoscopy performed using the Airtraq and conventional Macintosh laryngoscope. In 20 patients requiring general anaesthesia and tracheal intubation, we measured cervical spine movement using radiography in the same patient during consecutive procedures using the two laryngoscopes. Although sign…

[Extension reserve of the hip in relation to the spine: Comparative study of two radiographic methods].

Free-hip movement is necessary for good spinal function. Limitation generally affects extension. The range of hip extension from the standing position can be considered as the hip's "extension reserve". The amplitude of this reserve must be known because any deficit requires a pathological solicitation of the vertebral column. Measurement of the extension reserve of the hip is useful for analyzing spinal disease and for preoperative planning. Physical examination can measure extension, but cannot differentiate movement produced by the hip from that originating in the spine. We have been unable…

Balloon kyphoplasty in the treatment of metastatic disease of the spine: a 2-year prospective evaluation.

There is currently little data on the longer term efficacy and safety of balloon kyphoplasty (BKP) in patients with metastatic vertebral compression fractures (VCFs). To prospectively assess the long-term efficacy and safety of BKP in treating thoracic and lumbar spinal metastatic fractures that result in pain or instability. Sixty-five patients (37 men, mean age: 66 years) underwent 99 BKP procedures. Patient-related outcomes of pain visual analogue scale (VAS) and Oswestry Disability Index were assessed pre- and post-operatively and after 3, 6, 12 and 24 months. Correction of vertebral heigh

Anterior corpectomy and fusion for severe ossification of posterior longitudinal ligament in the cervical spine.

Between May 2002 and October 2006, 19 patients (17 men and 2 women; average age 57.2; range 47-71 years) received anterior corpectomy and fusion for severe ossification of the posterior longitudinal ligament (OPLL) in our department. Preoperative radiological evaluation showed the narrowing by the OPLL exceeded 50% in all cases, and OPLL extended from one to three vertebrae. We followed-up all patients for 12-36 months (mean 18 months). The Japanese Orthopaedic Association (JOA) score before surgery was 9.3 +/- 1.8 (range 5-12) which significantly increased to 14.2 +/- 1.3 (range 11-16) points

Kinematic analysis of the relationship between sagittal alignment and disc degeneration in the cervical spine.

STUDY DESIGN: Retrospective analysis using kinetic magnetic resonance images (MRIs). OBJECTIVE: To investigate the relationship of changes in the sagittal alignment of the cervical spine on the kinematics of the functional motion unit and disc degeneration. SUMMARY OF BACKGROUND DATA: Normal lordotic alignment is one of the most important factors contributing to effective motion and function of the cervical spine. Loss of normal lordotic alignment may induce pathologic changes in the kinematics and accelerate degeneration of the functional motion unit. However, the relationship of altered al…

Mobilizations of the asymptomatic cervical spine can reduce signs of shoulder dysfunction in adults.

Generalized shoulder pain is a common problem that is difficult to treat and frequently recurrent. The asymptomatic cervical spine must be ruled out as a cause of any shoulder pain, as it can have a similar presentation to an isolated shoulder disorder. Previous studies have shown that lateral cervical glide mobilizations to the asymptomatic cervical spine at C5/6 can affect peripheral pain, but none have examined shoulder pain. A randomized, blinded, placebo-controlled, cross-over trial was used to examine the immediate effects of cervical lateral glide mobilizations on pain intensity and sho…

Multifocal Ewing's sarcoma of the brain, calvarium, leptomeninges, spine and other bones in a child.

Ewing's sarcoma (ES) is a malignant osseous neoplasm that mostly affects children and young male adults, and usually presents as a solitary bony lesion. Multifocal ES of the central nervous system is extremely rare, with an incidence ranging from 1.1% to 4.3%. Clinically, ES may mimic osteomyelitis. In this report, we describe the case of an 11-year-old boy who had multiple calvarial, leptomeningeal, spinal and various other bony lesions of ES, which were diagnosed radiologically and histopathologically. To the best of our knowledge, this is the first time that multiple brain, calvarial and sp

Magnetic resonance imaging of renal abnormalities in patients with congenital osseous anomalies of the spine.

BACKGROUND: Patients with congenital osseous anomalies of the spine are known to have a high prevalence of abnormalities in the renal system and of the spinal cord. Today, the screening tools of choice to detect these abnormalities include ultrasonography of the kidneys and collecting system and magnetic resonance imaging of the spine. A single screening tool that can identify both renal and intraspinal anomalies would be ideal. METHODS: Imaging studies of all patients with a congenital osseous anomaly of the spine seen at our institution during a ten-year period were retrospectively reviewed…

Neurotransmitter receptor heteromers and their integrative role in 'local modules': the striatal spine module.

'Local module' is a fundamental functional unit of the central nervous system that can be defined as the minimal portion of one or more neurons and/or one or more glial cells that operates as an independent integrative unit. This review focuses on the importance of neurotransmitter receptor heteromers for the operation of local modules. To illustrate this, we use the striatal spine module (SSM), comprised of the dendritic spine of the medium spiny neuron (MSN), its glutamatergic and dopaminergic terminals and astroglial processes. The SSM is found in the striatum, and although aspects such as …

Can periods of static loading be used to enhance the resistance of the spine to cumulative compression?

Results of in vitro studies conducted on isolated bone specimens have indicated a higher tolerance to static load than exists when exposed to cyclic loading, when controlled for creep rate. If this difference in load tolerance exists, it may be exploited to extend the life of vertebral bone exposed to repetitive compression, and potentially alter the development of spinal injury. However, little work has been conducted on functional spinal units to determine if bone displays this characteristic within an intact joint. Additionally, static loading may result in load redistribution within the in
