BACKGROUND: Immunosuppression increases the risk of primary cutaneous leishmaniasis (CL) infection and its reactivation. This systematic review examined CL reactivation in immunosuppressed patients, including those with autoimmune diseases and solid organ transplant recipients.

Sequential anterior ischaemic optic neuropathy was observed in a patient treated with a tumour necrosis factor α (TNF) inhibitor, adalimumab, for ankylosing spondylitis. Chronic inflammatory spondyloarthritis involves axial symptoms of the spine and sacroiliac joints, or peripheral arthritis. Many patients suffer from extra-articular manifestations. With acute anterior uveitis, rapid treatment prevents synechiae. Other organs can be involved. PURPOSE: To report a patient treated with infliximab for ankylosing spondylitis and HLA-B27 uveitis presenting with a scotoma in his left good eye due to infliximab-induced occipital demyelination mistaken for a uveitis recurrence.
DESIGN: Case report.
METHODS: Review of clinical features, visual field, and investigational procedures.
RESULTS: Loss of visual function could not be explained by mere ophthalmic examination. Magnetic resonance imaging showed several demyelinating lesions, explaining the patient's symptoms and signs. Infiliximab was discontinued and the scotoma disappeared.

Thirty-eight cases of suspected yersinia arthritis occurring in southern Sweden in 1975-6 were reviewed four to five years later. In 31 cases the diagnosis was confirmed. At follow-up three of the patients had definite ankylosing spondylitis, three radiologically confirmed sacroiliitis, three extensor tenosynovitis, five isolated articular joint disease, and 10 localised arthralgias; one patient had developed seropositive rheumatoid arthritis. Sera of children with juvenile rheumatoid arthritis and other connective tissue diseases were tested for antibodies to native DNA by a radiolabeled-binding assay. Normal values were obtained in 130 children with JRA, including 28 with uveitis and 14 with selective IgA deficiency.
