Traditional shoulder arthroscopy typically requires both anterior and posterior portals. However, in the traditional anterior portal, simultaneous operation of the suture device and the gripper is not possible, necessitating an additional anterolateral portal for anchor insertion and suture delivery, which poses technical challenges for novice surgeons. Additionally, the ulnar side of the conventional rotator cuff suture hook is large, and it frequently causes iatrogenic damage to rotator cuff tissues. Therefore, we propose a modified arthroscopic technique that uses a 16 G needle for suture p

The birth prevalence of craniosynostosis (premature suture fusion) is 300-500 per 1,000,000 live births. Surgical management involves the re … tion of various vehicles and tissue engineered constructs to deliver cytokines and genes to cranial sutures. Such biologically based therapies may be used as surgical adjuncts to rescue fusing sutures or help manage postoperative resynostosis.

There are no prior reports of totally endoscopic minimally invasive cardiac surgery for combined ventricular septal defect closure and aortic valve replacement. We utilized a periareolar incision as the main access, inserting a cardioplegia line, vent tube and the aortic cross-clamp entirely through this incision. Only three ports (main incision, camera port and left-hand port) were used. A three-dimensional endoscope facilitated precise techniques. The type I ventricular septal defect was approached and closed from the aortic side using a Gore-Tex patch, with mattress sutures for the lower ed…

Interventricular septum defects are a known complication after an aortic valve replacement, but not after mitral surgery. We present a case of a 65-year-old female who underwent unsuccessful mitral valvuloplasty through port-access surgery, followed by uneventful mechanical bioprosthesis replacement. Postoperatively, the patient experienced multiple arrhythmias and recovered poorly. A systolic murmur led to echocardiography, showing an interventricular septum rupture. This was closed with 2 polypropylene pledgeted mattress sutures, via the right atrium and tricuspid valve through midsternal ac…

Sagittal craniosynostosis, the most common single suture craniosynostosis, is treated by numerous surgical techniques. Minimally invasive endoscopy-assisted procedures with postoperative helmeting are being used with reports of good cosmetic outcomes with decreased morbidity, shortened hospital stay, and less blood loss and transfusion. This procedure uses small skin incisions, which must be properly placed to provide safe access to the posterior sagittal and lambdoid sutures. However, the lambda is often hard to palpate through the skin due to the abnormal head shape. The authors describe the…

Case Description-A 4.6-month-old pot-bellied pig was evaluated because of non-weight-bearing lameness (grade 5/5) in the right forelimb of 4 days' duration. Clinical Findings-Clinical and radiographic examination revealed a closed, lateral luxation of the right shoulder joint. Treatment and Outcome-Initial attempts at closed reduction failed to provide adequate stability of the shoulder joint. Open reduction and internal fixation by placement of 2 lateral tension sutures with a system designed for canine cranial cruciate ligament repair provided adequate joint stability and a successful outcom…

OBJECTIVE: To determine the excellent operative procedure to treat mitral valve prolapse (MVP).
METHODS: Fifty seven patients with MVP were divided into three groups: Group I received traditional chordal reconstruction; Group II underwent chordal replacement using expanded polytetrafluorethylene(e-PTFE) sutures and artificial ring valvuloplasty reconstruction; Group III submitted mitral valve replacement.
RESULTS: Mitral regurgitation volume was reduced significantly in Group II [(1.3 +/- 3.8) ml] and Group III [(1.2 +/- 3.4) ml] than that in Group I [(4.7 +/- 7.7) ml] (P < 0.05).

Linear iron deposits in the corneal epithelium are well-documented phenomena in a variety of circumstances, both normal and pathologic. A characteristic annular pigmented line, appearing clinically as an "iron" line, occurred in the donor epithelium of approximately 13% of corneal grafts with retained sutures. Eight cases of normally functioning grafts had iron lines located just central to the sutures. These lines appeared at various intervals postoperatively and did not correlate with age or race. They were frequently found in association with other surface-related epithelial and subepitheli…

In 30 rats the healing process of colonic anastomosis contaminated with Staphylococcus aureus was studied. At the same time on each animals a single- and double-layer anastomosis was performed. All sutures were infected during the surgical procedure with highly infectious S. aureus. The rats were sacrificed after 1, 2, 3, 4 days, and 2 1/2 weeks. Criteria for estimation were mainly visible abscess formations, leakage spots, bacteriologic and histologic examinations. After apparent inflammatory reactions were seen on the first postoperative days with a maximum on the 4th day, all sutures were c…

BACKGROUND AND AIMS: Data from randomized trials investigating different access closure strategies after transfemoral transcatheter aortic valve implantation (TF-TAVI) remain scarce. In this study, two vascular closure device (VCD) strategies to achieve haemostasis after TF-TAVI were compared.
METHODS: The ACCESS-TAVI (Comparison of Strategies for Vascular ACCESS Closure after Transcatheter Aortic Valve Implantation) is a prospective, multicentre trial in which patients undergoing TF-TAVI were randomly assigned to a strategy with a combined suture-/plug-based VCD strategy (suture/plug group)

BACKGROUND: Glycomer 631 and lactomer 9-1 are absorbable suture materials indicated for soft tissue approximation in non-infected wounds. Pigs are often used as surgical models in translational research; however, reports of tissue reactions to both suture materials in pigs are limited. The aim of this study was to assess clinical and histologic responses of skin incisions closed with a subcuticular technique using glycomer 631 and lactomer 9-1 in pigs.
RESULTS: Skin incisions on 17 pigs were closed with glycomer 631 and lactomer 9-1, and a clinical reactive score (CRS) including erythema, swe

PROBLEM: Perineal repair is generally not a widely practiced skill amongst Australian midwives, the reasons for this are uncertain and may result in technique variations.
BACKGROUND: Many birthing women experience perineal tears that require suturing. As midwives attend the majority of vaginal births, they would be ideally placed to undertake perineal repair.
AIM: To describe the current level of midwifery perineal repair skill acquisition, knowledge, techniques and utilization by Australian midwives.
METHODS: An online survey was distributed to Australian College of Midwives members and sh

The peripheral nerve injury is a common injury in clinical practice. For centuries, traditional epineurial and perineurial suture techniques have been used to repair nerve transection after peripheral nerve injuries. As there are several types of nerve fibers in neural stump, accurate and effective regeneration of the fibers is limited. The outcome of repairing nerve injury has little improvement till now. Meanwhile , the selective regeneration of peripheral nerve fibers has been gradually confirmed by researchers. The limitation of traditional epineurial and perineurial suture techniques and

Heterozygous loss of Twist1 function causes coronal synostosis in both mice and humans. We showed previously that in mice this phenotype is associated with a defect in the neural crest-mesoderm boundary within the coronal suture, as well as with a reduction in the expression of ephrin A2 (Efna2), ephrin A4 (Efna4) and EphA4 in the coronal suture. We also demonstrated that mutations in human EFNA4 are a cause of non-syndromic coronal synostosis. Here we investigate the cellular mechanisms by which Twist1, acting through Eph-ephrin signaling, regulates coronal suture development. We show that Ep

The most important risk factor for the development of wound dehiscence and incisional hernia is the suture technique that is totally in the hands of the surgeon. A continuous suture line in one layer with a monofilament material should close midline incisions. Self-locking knots should be used for the anchor knots. The length of the suture used must be at least four times the length of the wound. The only way to ascertain a suture length to wound length ratio of at least four is to measure and document the ratio at every laparotomy. An adequate ratio should be achieved by placing many stitches

The aim of this experimental study was to evaluate the effect of Translaryngeal Percutaneous Arytenoid Lateralization (TPAL) in dogs with experimentally created laryngeal paralysis. All dogs (n=5) underwent bilateral recurrent laryngeal neurectomy before TPAL. Two TPAL suture techniques were evaluated. TPAL-CranialCaudal (TPAL-CC) was performed first, followed 11 to 14days later by TPAL-DorsalVentral (TPAL-DV). For both techniques, a mattress suture was placed through the arytenoid cartilage via an oral approach. Laryngeal examination was performed before, immediately after, and on days 1, 3 a

Bones of the craniofacial skeleton are derived from two distinct cell lineages, cranial neural crest and mesoderm, and articulate at sutures and synchondroses which represent major bone growth sites. Premature fusion of cranial suture(s) is associated with craniofacial dysmorphogenesis caused in part by alteration in the growth potential at sutures and can occur as an isolated birth defect or as part of a syndrome, such as Apert syndrome. Conditional expression of the Apert FGFR2 S252W mutation in cells derived from mesoderm was previously shown to be necessary and sufficient to cause coronal …

BACKGROUND: Biliary anastomotic strictures (BAS) after liver transplant (LT) are a significant contributor to post-transplant morbidity. Although surgical technique has been proposed as a risk factor, accurate evaluation of technique has been limited by inherent bias in conventional definitions for BAS. This study aimed to evaluate the incidence of clinically significant BAS (csBAS) with absorbable suture material and variable anastomotic suture technique in patients undergoing LT with duct-to-duct (DD) anastomosis.
METHODS: A retrospective medical record review was conducted of adult patient
