Treatment of metabolic syndrome.

The metabolic syndrome is intended to identify patients who have increased risk of diabetes and/or a cardiac event due to the deleterious effects of weight gain, sedentary lifestyle, and/or an atherogenic diet. The National Cholesterol Education Program's Adult Treatment Panel III definition uses easily measured clinical findings of increased abdominal circumference, elevated triglycerides, low high-density lipoprotein-cholesterol, elevated fasting blood glucose and/or elevated blood pressure. Three of these five are required for diagnosis. The authors also note that other definitions of metab…

Heyde's syndrome: a review.

Bleeding from gastrointestinal angiodysplasia in patients with aortic stenosis (AS), termed Heyde's syndrome, has been recognized for many years. Intestinal angiodysplasia (IA) and AS are chronic degenerative diseases that are often asymptomatic, with a higher prevalence in the population than is clinically apparent. The incidence of both conditions increases with age, and both are associated with traditional cardiovascular risk factors. Many studies suggest that there is an increased prevalence of IA in AS and vice versa, but there is wide variation between studies. Evidence is mounting that …

[Bartter syndrome--case report].

The authors present the case of 4-month-old girl, who was admitted to our hospital with hypokalemia, metabolic alkalosis, hyperaldosteronism, hyperreninism with normal blood pressure and high urine concentration of PGE2. All the clinical and biochemical features have led to the diagnosis of Bartter syndrome. Treatment consisted of 15% KCI, spironolacton and indometacin.

[Wolff-Parkinson-White syndrome].

WPW syndrome is a rare congenital heart disease due to an accessory pathway between the atrium and the ventricle. Its ECG pattern is characterized by a short PR interval, a delta wave, a wide QRS complex and an abnormal ventricular repolarisation. Patients are usually asymptomatic, or have frequent paroxysmal episodes of tachycardia. These tachycardias are supraventricular tachycardias related to either reentry between the AV node and the accessory pathway, or atrial tachyarythmias descending through this bypass tract, or both. In the latter clinical situation, sudden cardiac death may occur i…

Gardner's syndrome associated fibromas.

Three patients with Gardner's syndrome having benign fibrous proliferations in the soft tissues and the oral cavity are presented. Lesions in all three patients were morphologically different. Two excised lesions in Case 1 were histologically identical to nuchal-type fibroma (NTF) and one lesion in this patient was an epidermal cyst. Case 2 had a lesion located in the soft tissues of the shoulder, which was morphologically similar to NTF and, in addition, contained mildly atypical and multinucleated fibroblastic cells. The oral lesion in Case 3 looked like a fibroma and was morphologically dif…

[Cardiac syndrome in opisthorchiasis].

According to the intensity and duration of invasion, superinvasion opisthorchiasis in laboratory animals and human beings involves all cardiac layers; the myocardium is mostly damaged. In its early phase, vascular inflammation of the microcirculatory bed, dystrophy, necrosis of striated muscle fibers progressing to diffuse cardiosclerosis result from abnormal immunological processes; due to reflex exposures (Botkin's syndrome), there may be persistent cardialgias that may progress to the clinical manifestations of chronic ischemic heart disease. Unpredictable dissipative clusters of metabolic …

The Wiskott-Aldrich syndrome.

The Wiskott-Aldrich Syndrome (WAS) is an inherited immunodeficiency caused by a variety of mutations in the gene encoding the WAS protein (WASp). WASp is expressed in hematopoetic cells and facilitates the reorganization of the actin cytoskeleton in response to many important cell stimuli. Extensive study of WAS and more recently WASp has given great insight into the relevance of this molecule and related molecules to both basic cell biology and human immune defenses.

Adult respiratory distress syndrome.

ARDS or acute respiratory distress syndrome continues to be a considerable critical care challenge. Mortality has not decreased significantly over the last more than 30 years. This article presents an overview of origin, evaluation, and treatment of ARDS. Recent findings relative to onset and precipitators of ARDS have led to changes in evaluation and treatment plans. Clinical and radiologic descriptors in assessment of the patient with ARDS are discussed. Ventilatory modes and nursing interventions to optimize patient outcomes are identified. The challenges of outcomes issues presented offer …

Neurosarcoidosis mimicking Sjögren's syndrome.

PURPOSE: To describe a patient with a long disease history who was finally diagnosed with neurosarcoidosis and to discuss the reasons behind the delayed diagnosis. CASE REPORT: A 58-year-old man with sick sinus syndrome and bradycardia, which was treated with a pacemaker, developed first right and then left facial palsy. Subsequently, multiple cranial nerve palsies developed and later spontaneously resolved. Neurosarcoidosis was suspected at that stage, but excluded because the patient had no typical sarcoid lung changes, his serum and cerebrospinal fluid angiotensin converting enzyme activit…

Arterial thoracic outlet syndrome.

Arterial vascular complications resulting from thoracic outlet compression, although rare, can be substantial and potentially limb threatening. Bony abnormalities such as cervical ribs can lead to chronic trauma to the subclavian artery. Early on, the clinical symptoms can be subtle and confusing, causing potential delay in diagnosis.

DiGeorge syndrome: an update.

PURPOSE OF REVIEW: This article is an update on DiGeorge syndrome research focusing on the synergy of human and model systems genetics toward the understanding of conotruncal and aortic arch defects. RECENT FINDINGS: The identification of mutations of the human T-Box1 (TBX1) gene and progress on research of Tbx1 function in mouse development demonstrate the pathogenetic role of this gene in DiGeorge syndrome and generate new hypotheses about its function in cardiovascular development. SUMMARY: The Tbx1 genetic pathway and the cell biology of tissues contributing to pharyngeal arch arteries a…
