Distal Hamstring Injuries and Disorders.

Hamstring injuries are a common injury sustained by athletes with most injuries occurring as strain injuries within the muscle belly or at the proximal musculotendinous junction. Distal hamstring pathology is relatively uncommon but comprises a collection of both acute and chronic diagnoses that can manifest with symptoms either on the medial or lateral side of the knee based on which hamstring tendons are involved. Pes anserinus bursitis is the most common of these distal hamstring pathologies with other chronic diagnoses, including snapping medial hamstrings or snapping biceps femoris. Acute

Extracorporeal Shockwave Therapy in the Management of Sports Medicine Injuries.

Treatment of musculoskeletal conditions in athletes with extracorporeal shockwave therapy (ESWT) is gaining popularity as greater evidence supports its use. ESWT protocols (describing energy flux density, number of impulses, type of shockwave (focused or radial), number/frequency/duration of treatment session, area of application, and postprocedural therapy protocols) can be adjusted in the clinical setting. Protocols vary across studies, and optimal protocols for most indications are yet to be determined. ESWT can safely be used to treat various musculoskeletal conditions in athletes, includi

Is Quadriceps Tendon Autograft a Better Choice Than Hamstring Autograft for Anterior Cruciate Ligament Reconstruction? A Comparative Study With a Mean Follow-up of 3.6 Years.

BACKGROUND: The quadriceps tendon (QT) autograft is known as an effective graft for anterior cruciate ligament (ACL) reconstruction and shows a similar functional outcome to the bone-patellar tendon-bone (BPTB) in randomized controlled trials, with a lesser incidence of complications. Up until now, only 2 studies have compared QT to hamstring tendon (HT) autograft. HYPOTHESIS: The functional outcomes of the QT technique are at least as good as those of the HT technique, with the same morbidity. STUDY DESIGN: Cohort study; Level of evidence, 3. METHODS: Ninety-five patients underwent isolate

Could targeted exercise programmes prevent lower limb injury in community Australian football?

BACKGROUND: Australian football is a popular sport in Australia, at both the community and elite levels. It is a high-speed contact sport with a higher incidence of medically treated injuries when compared with most other organized sports. Hamstring injuries, ligament injuries to the knee or ankle, hip/groin injuries and tendinopathies are particularly common and often result in considerable time lost from sport. Consequently, the prevention of lower limb injuries is a priority for both community and elite Australian football organizations. There is considerable literature available on exercis

Surgical management of Achilles tendon re-ruptures: a prospective cohort study.

PURPOSE: The purpose of this study was to compare outcomes of different minimally invasive techniques for reconstruction of Achilles tendon re-ruptures. METHODS: We prospectively enrolled 21 patients undergoing minimally invasive reconstruction using a transfer of the ipsilateral peroneus brevis (PB) (five patients) or the free ipsilateral semitendinosus tendon (ST) graft with or without interference screw fixation (ten and six patients, respectively). We assessed the maximum calf circumference and isometric plantar flexion strength before surgery and at the last follow up. The Achilles tendo

Ultrasound strain mapping of Achilles tendon compressive strain patterns during dorsiflexion.

Heel lifts are commonly prescribed to patients with Achilles tendinopathy, yet little is known about the effect on tendon compressive strain. The purposes of the current study were to (1) develop a valid and reliable ultrasound elastography technique and algorithm to measure compressive strain of human Achilles tendon in vivo, (2) examine the effects of ankle dorsiflexion (lowering via controlled removal of a heel lift and partial squat) on compressive strain of the Achilles tendon insertion and (3) examine the relative compressive strain between the deep and superficial regions of the Achille

Delayed repair of distal biceps tendon ruptures is successful: a case-control study.

BACKGROUND: The literature has shown an increased complication rate with a delay to surgical repair of acute distal biceps tendon ruptures; however, little has been documented regarding the outcome of delayed repairs. This case-control study compared a study cohort of delayed (>21 days) distal biceps tendon repairs with a control cohort repaired acutely (<21 days). METHODS: Sixteen delayed repair cases were reviewed and matched with acute controls (1:3) based on repair technique, age, and workers' compensation status. The delayed cohort was reviewed and completed isometric strength testing an

Decision-making in massive rotator cuff tear.

Treatment of massive rotator cuff tears has developed over many years ranging from conservative treatment to open and arthroscopic repair, muscle transfers and reversed arthroplasty. The evolution of more advanced techniques in arthroscopic repair has changed the treatment approach and improved the prognosis for functional outcome despite low healing rates. Due to this rapid development, our evidence-based knowledge today is mainly founded in Level 3 and Level 4 studies. Based on the literature, the current knowledge on treatment of symptomatic massive rotator cuff tears is proposed in an algo

Ultrasound characteristics of the patellar and quadriceps tendons among young elite athletes.

Tendons adapt in response to sports-specific loading, but sometimes develop tendinopathy. If the presence of ultrasound changes like hypoechoic areas and neovascularization in asymptomatic tendons precede (and predict) future tendon problems is unknown. The aim of this prospective cohort study was to investigate the relationship between the development of ultrasound changes in the patellar and quadriceps tendons and symptoms of jumper's knee, as well to examine the medium-term effects of intensive training on tendon thickness among adolescent athletes. Elite junior volleyball athletes were fol

[Achilles tendon ruptures: 25 year's experience in sport-orthopedic treatment].

From 1972 - 1996 570 Achilles tendon ruptures in 565 patients were treated in the Sportklinik Stuttgart. The 499 men and 66 women had an average age of 38 years. For the diagnosis of a Achilles tendon rupture Ultrasound and MRI are important procedures, but clinical history and examination are still the best methods to find an Achilles tendon rupture (100%). However, the Actiology of the Achilles tendon rupture is still controversial and cannot be answered by these methods. Opposed to the degenerative theory, biomechanical experiments show that any Achilles tendon can tear when the calf muscle

Delayed Diagnosis of Quadriceps Tendon Rupture.

Quadriceps tendon (QT) injury classically presents with pain, inability to actively extend the knee, and a palpable defect in the suprapatellar region. Unilateral eccentric contraction and hyperflexion after ground level fall are the most common mechanisms of injury. Delayed QT repair may require additional operative procedures and is associated with poorer post-operative functional outcomes, persistent knee extensor weakness, and decreased range of motion. This report demonstrates a case of delayed diagnosis of QT rupture after a fall with injury to the contralateral extremity. Initial evalua

Lisfranc Joint Ligament Complex Reconstruction: A Promising Solution for Missed, Delayed, or Chronic Lisfranc Injury Without Arthritis.

The current classifications of "Lisfranc injury" can be purely ligamentous (low-grade midfoot sprains) or involve the osseous and articular structures (high-grade Lisfranc fracture displacements). The first type is often difficult to detect. If these patients are not properly treated, long-term disability can result. The rate of missed or delayed diagnoses has ranged from 13% to 24%, primarily owing to the subtlety of the radiographic findings. This is relatively more common in cases of subtle ligamentous injury (19%). The aim of the present report was to provide a new technique for missed or

Anatomic outcome of percutaneous release among patients with trigger finger.

PURPOSE: To investigate the adequacy and safety of percutaneous trigger finger release on symptomatic patients. METHODS: Two orthopedic non-hand surgeons performed percutaneous A1 pulley release on the thumb, index, middle, and ring fingers with the use of a 19-gauge needle in 25 fingers of 24 patients. Open inspection was then performed to determine the extent of release and any injury to the surrounding anatomic structures. RESULTS: Triggering was eliminated in all fingers. Of the 25 A1 pulleys, 19 were partially released; only 6 were completely released. Noted injury included only superfi

Patella tendon anterior tibial cortex angle (PTATA): An indirect marker of anterior cruciate ligament (ACL) tear?

BACKGROUND: Anterior cruciate ligament (ACL) injuries are common in high-impact sports. While direct MRI evaluation is preferred for diagnosis, indirect signs are often crucial. The Patella Tendon Anterior Tibial Cortex Angle (PTATA) could serve as an additional indirect marker of ACL injury, reflecting altered knee biomechanics. We hypothesise that knees presenting complete ACL rupture will exhibit a smaller PTATA on MRI due to the associated anterior tibial translation. METHODS: We reviewed 100 knee MRI scans of patients under the age of 30 years, presenting with acute knee injuries and cli

Gene Therapy to Enhance Bone and Cartilage Repair in Orthopaedic Surgery.

Musculoskeletal conditions are a major public health problem. Approximately 66 million individuals seek medical attention for a musculoskeletal injury in the United States, with current medical costs being estimated at $873 billion annually. Despite advances in pharmaceuticals, implant materials and surgical techniques, there remains an unmet clinical need for successful treatment of challenging musculoskeletal injuries and pathologic conditions, particularly in the setting of compromised biological environments. Tissue engineering via gene therapy attempts to provide an alternative treatment

Analysis of foot kinematics wearing high heels using the Oxford foot model.

Wearing high heels is thought to lead to various foot disorders and injuries such as metatarsal pain, Achilles tendon tension, plantar fasciitis and Haglund malformation. However, there is little available information explaining the specific mechanisms and reasons why wearing high heels causes foot deformity. Therefore, the purpose of this study was to investigate the foot kinematics of high heel wearers and compare any differences with barefoot individuals using the Oxford Foot Model (OFM). Fifteen healthy women aged 20-25 years were measured while walking barefoot and when wearing high heels
