Spontaneous rupture of the urinary bladder is extremely rare. We report a case of a 70-year-old man with spontaneous bladder rupture secondary to neglected giant vesicle calculi who presented as acute renal failure. The patient was stabilised with per-urethral catheterisation and extravesical drain placement. About 700 mL pus mixed with urine was drained through the per-urethral catheter and approximately 2000 mL of pus was drained through the extravesical drain. Cystolithotomy showed two large calculi which were removed.

The authors report the case of a 26 year old man consulting for primary sterility due to retrograde ejaculation, which was associated with urinary incontinence secondary to repeated surgery to the bladder neck for neurogenic bladder in a patient with incomplete spina bifida. Pericervical implantation of an AMS 800 artificial sphincter restored perfect continence and normal ejaculation followed by a pregnancy for his wife.

We studied renal function and structure in 42 patients with meningomyelocele, 28 treated with intermittent catheterization and 14 with ileal loop diversion. Patients were observed for a minimum of 60 months. Nine of the 28 patients who underwent intermittent catheterization had evidence of unilateral or bilateral reflux, and all patients with ileal loop diversion had free ureteral reflux. Urethral fibrosis and sphincter incompetence commonly are associated in the genesis of stress incontinence. Other factors are often associated, such as bladder hyperactivity and increased demand on the sphincter mechanism created by contracted or large atonic bladder or polyuria. The frequent association of several factors and the limited information given by the classical methods suggest a need for the routine use of objective urodynamic studies in the assessment of stress incontinence. A more conservative, better oriented, therapeutic approach may result from this concept.

PURPOSE: A simplified Kropp bladder neck reconstruction was done to achieve urinary continence.
MATERIALS AND METHODS: A total of 23 patients with an incompetent bladder neck underwent the procedure, including 22 with a neurogenic bladder secondary to myelodysplasia and 1 with bladder exstrophy. The effect of the new alpha-adrenoreceptor blocking agent prazosin on hypermotility of the urinary bladder was investigated, using controlled cystometry in neurological patients with normal bladder or with uninhibited neurogenic bladder. In short-term administration of prazosin, the bladder capacity was markedly increased and the uninhibited detrusor contraction was diminished. Hydatid disease, the parasitic infestation caused by the cestode, echinococcus granulosus involves mainly the liver and the lungs though no organ is immune. Genito urinary involvement has been found mainly in the kidneys and rarely in other structures such as, bladder and epididymis. Isolated retrovesical location of the hydatid cyst is a very rare condition whose manifestations appears after a long course of the disease and are due to compression of bladder, causing the bladder out flow obstruction.
