Robot-assisted intrafascial simple prostatectomy: novel technique.

PURPOSE: We describe our initial experience with intrafascial robot-assisted simple prostatectomy (IF-RSP). Potential advantages include reduced blood loss, elimination of the need for postoperative bladder irrigation, and elimination of the risk of residual or future prostate cancer, without interrupting potency or continence. PATIENTS AND METHODS: From June 2011 to March 2012, 10 patients with symptomatic prostatomegaly on transrectal ultrasonography (TRUS) (mean 81 g) underwent IF-RSP. Three patients had acute urinary retention. Demographic perioperative and outcome data were recorded up t

Detection of urinary leakage after radical retropubic prostatectomy by contrast enhanced ultrasound - do we still need conventional retrograde cystography?

INTRODUCTION: To prospectively evaluate the accuracy of transvesical contrast-enhanced ultrasound (CEUS) as an alternative method for the detection of anastomotic leakage after radical retropubic prostatectomy (RRP) in comparison with the current standard method of conventional retrograde cystography (CG). PATIENTS AND METHODS: Forty-three patients underwent RRP for histologically proven localized prostate cancer. The vesico-urethral anastomosis was evaluated 8 days after RRP by CG and CEUS. Any peri-anastomotic leakage was assessed and determined in CG and CEUS as follows: no extravasation (

Management and outcome of positive urine cultures in a neonatal intensive care unit.

OBJECTIVES: The aim of this study was to describe the management and outcome of positive urine cultures in a neonatal intensive care unit (NICU). STUDY DESIGN: A chart review was completed of infants born October 1, 2004 to December 31, 2006 and admitted to the NICU at the Royal Alexandra Hospital, Edmonton, Alberta with any growth of bacteria or fungi in urine. RESULTS: Positive urine cultures were obtained in 64 of 2936 admissions (2%) and were classified as contaminated urines (n=34), possible urinary tract infection (UTI) (n=14), definite UTI (n=10), and candidal UTI (n=6). Management wa

Conservative management of spontaneous rupture of the urinary bladder.

We report a very rare case of intra-peritoneal spontaneous rupture of the urinary bladder which was successfully managed conservatively in a 77-year-old woman, who presented with lower abdominal pain, acute urinary retention and pelvic organ prolapse (POP). Computed tomography raised the suspicion of bladder rupture which was later confirmed by cystoscopy and cystogram. Following 8 weeks of urinary catheterisation, the bladder healed spontaneously. A subsequent sub-total abdominal hysterectomy and sacrocervicopexy were performed to treat the POP. The management of bladder rupture is discussed.

Endoscopic management of urologic complications following renal transplantation: impact of ureteral anastomosis.

OBJECTIVE: To evaluate the success of ureteral stent placement to treat or prepare for surgical treatment of urologic complications after renal transplantation, according to a type of ureteral anastomosis. PATIENT AND METHODS: From May 1989 to December 2006, we performed 703 kidney transplantations including 412 extravesical ureteroneocystostomy (according to Lich-Gregoire technique) and 265 transvesical ureteroneocystostomy (according to Politano-Leadbetter technique). We retrospectively analyzed our endoscopic management of urinary leaks and ureteral strictures. The criteria of success were

[Ureteral-abdominal aortic fistula during the exchange of ureteral stenting: a case report].

We report a case of a patient with a fistula between the left ureter and abdominal aorta. The patient was a 44-year-old male who had undergone radiation therapy (intraoperative: 25 Gy, external beam: 50 Gy) and chemotherapy (CDDP: 250 mg) for retroperitoneal lymph node metastasis from seminoma. His postoperative course was complicated by stenosis of bilateral ureters, which were treated by indwelling double J-stents. Fifteen years after the operation, gross hematuria occurred from the left ureteral orifice when exchanging the left ureteral double J-stent. Computed tomographic scan demonstrated

Outcomes of stenting after uncomplicated ureteroscopy: systematic review and meta-analysis.

OBJECTIVE: To investigate the potential beneficial and adverse effects of routine ureteric stent placement after ureteroscopy. DESIGN: Systematic review and meta-analysis of randomised controlled trials. DATA SOURCES: Cochrane controlled trials register (2006 issue 2), Embase, and Medline (1966 to 31 March 2006), without language restrictions. Review methods We included all randomised controlled trials that reported various outcomes with or without stenting after ureteroscopy. Two reviewers independently extracted data and assessed quality. Meta-analyses used both fixed and random effects mo

Evidence for the use of silver-alloy-coated urethral catheters.

Catheter-associated urinary tract infections (CAUTIs) are a common occurrence and are associated with increased patient morbidity and mortality. In addition, they delay patient discharge from hospital, substantially increase hospital costs and promote the emergence of resistant organisms. Any intervention resulting in a decrease in the incidence of CAUTIs would have a significant impact on patient quality of life and hospital costs. By reviewing the current literature, it can be seen that the use of silver-alloy-coated hydrogel catheters can reduce CAUTIs by up to 45%. Despite reducing CAUTIs

Clinical significance of Aerococcus urinae: a retrospective review.

A retrospective chart review was performed on 54 patients with positive urine cultures during a 1-year period to assess the clinical significance of Aerococcus urinae. Based on predetermined criteria, patients were classified into 2 groups: those with urinary tract infections (UTIs) and those who were considered colonized. The majority of the patients were > or =65 years old and were female. Only 31% of patients with UTI and 45% of colonized patients had A. urinae isolated in pure cultures. Both groups had significant but similar underlying medical conditions, with urologic conditions being pr

Laparoscopic radical prostatectomy - results of 200 consecutive cases in a Canadian medical institution.

PURPOSE: Since Guillonneau and Vallancien reintroduced transperitoneal laparoscopic radical prostatectomy in 1998, several other medical institutions in Europe have published their series and confirmed that laparoscopic radical prostatectomy (LRP) is a feasible and reproducible approach to the management of clinically localized prostate cancer; however, there have been few reports from North American medical institutions. We now report the results of our first 200 consecutive cases of LRP in a Canadian Medical Institution. MATERIALS AND METHOD: From February/2000 until April/2003, laparoscopi
