When is retransplantation a viable option?

As the number of recipients of heart transplantation grows over time and they survive longer, more are at risk for developing severe cardiac allograft vasculopathy and allograft dysfunction, which might lead to consideration for retransplantation. Clearly, outcomes following cardiac retransplantation are compromised, and with donor shortage, the selection of candidates must be judicious. Retransplantation appears most appropriate for those patients more than 6 months following original heart transplantation, who have severe cardiac allograft vasculopathy and associated left ventricular dysfunc

[Tako-tsubo cardiomyopathy in a man with cerebral haemorrhage].

BACKGROUND: The Tako-Tsubo syndrome is characterized by a transient left ventricular dysfunction in the absence of obstructive coronary disease, typically precipitated by severe emotional or physical stress. It is characterized by a balloon-like left ventricular appearance on angiography, ECG changes, and elevated cardiac biomarkers. We describe a case, where Tako-Tsubo cardiomyopathy was diagnosed in a patient with subarachnoid haemorrhage. CASE REPORT: A 57-year-old man was brought to the hospital after cardiac arrest and successful resuscitation. After admission, echocardiography was perfo

Desmoplakin Cardiomyopathy, a Fibrotic and Inflammatory Form of Cardiomyopathy Distinct From Typical Dilated or Arrhythmogenic Right Ventricular Cardiomyopathy.

BACKGROUND: Mutations in desmoplakin (DSP), the primary force transducer between cardiac desmosomes and intermediate filaments, cause an arrhythmogenic form of cardiomyopathy that has been variably associated with arrhythmogenic right ventricular cardiomyopathy. Clinical correlates of DSP cardiomyopathy have been limited to small case series. METHODS: Clinical and genetic data were collected on 107 patients with pathogenic DSP mutations and 81 patients with pathogenic plakophilin 2 (PKP2) mutations as a comparison cohort. A composite outcome of severe ventricular arrhythmia was assessed. RES

Coronary slow flow and myocardial contractile reserve: a speckle‑tracking echocardiography study.

BACKGROUND: The Coronary Slow Flow Phenomenon (CSF) is increasingly recognized as a distinct phenotype of Ischemia with Non-Obstructive Coronary Arteries (INOCA). However, debate persists regarding whether CSF represents fixed structural microvascular remodeling or functional dysregulation. Conventional Left Ventricular Ejection Fraction (LVEF) is often insensitive to the subtle subendocardial mechanics associated with this condition. OBJECTIVES: This study aimed to interrogate the functional reserve of the left ventricle in patients with angiographically defined CSF using Two-Dimensional Spe

Exercise capacity and peak oxygen consumption in asymptomatic patients with chronic aortic regurgitation.

BACKGROUND: In patients with chronic, hemodynamically significant aortic regurgitation (AR), a long period of left ventricular remodeling usually occurs prior to the development of symptoms or left ventricular dysfunction. The value of cardiopulmonary exercise testing in patients with asymptomatic AR is not established. MATERIAL AND METHODS: Sixty-six asymptomatic patients aged 44±14 years with hemodynamically significant, chronic AR and no indication for aortic valve replacement were evaluated by echocardiography, cardiac magnetic resonance imaging and exercise testing with measurement of pe

Cardiac disease in patients with inflammatory myopathy.

BACKGROUND AND AIMS: Inflammatory myopathies (IMs) are a group of autoimmune diseases characterized by progressive symmetric muscle weakness and various extramuscular manifestations. Cardiac involvement in IM has been associated with worse outcomes, but evidence to support specific screening and management algorithms for cardiac comorbidities in IM is still limited. METHODS: For this observational study, 37 adult IM patients recruited from the rheumatology outpatient clinic at Ludwig-Maximilians-University Hospital between August 2023 and January 2025 completed a questionnaire on IM character

Cardiovascular involvement in Erdheim-Chester diseases is associated with myocardial fibrosis and atrial dysfunction.

PURPOSE: Erdheim-Chester disease (ECD) is a rare multisystem histiocytosis, whose cardiovascular involvement has not been systematically characterized so far. We aimed to systematically (qualitatively and quantitatively) describe the features of cardiovascular involvement in a large cohort of ECD patients and to evaluate its impact on myocardial fibrosis extension and cardiac function. MATERIAL AND METHODS: Among 54 patients with biopsy-proven ECD, 29 patients (59 ± 12 years, 79% males) underwent 1.5-T CMR using a standardized protocol for qualitative and quantitative assessment of disease lo

Mineralocorticoid receptor antagonist treatment of established pulmonary arterial hypertension improves interventricular dependence in the SU5416-hypoxia rat model.

Treatment with mineralocorticoid receptor (MR) antagonists beginning at the outset of disease, or early thereafter, prevents pulmonary vascular remodeling in preclinical models of pulmonary arterial hypertension (PAH). However, the efficacy of MR blockade in established disease, a more clinically relevant condition, remains unknown. Therefore, we investigated the effectiveness of two MR antagonists, eplerenone (EPL) and spironolactone (SPL), after the development of severe right ventricular (RV) dysfunction in the rat SU5416-hypoxia (SuHx) PAH model. Cardiac magnetic resonance imaging (MRI) in

Diastolic function during hemorrhagic shock in rabbits.

Hemorrhage (H) is associated with a left ventricular (LV) dysfunction. However, the diastolic function has not been studied in detail. The main goal was to assess the diastolic function both during and 120 min after bleeding, in the absence and in the presence of L-NAME. Also, the changes in mRNA and protein expression of nitric oxide synthase (NOS) isoforms were determined. New Zealand rabbits were divided into three groups: Sham group, H group (hemorrhage 20% blood volume), and H L-NAME group (hemorrhage treated with L-NAME). We evaluated systolic and diastolic ventricular functions in vivo

Follow-Up After Catheter Ablation of Papillary Muscles and Valve Cusps.

OBJECTIVES: The goal of this study was to determine the impact of catheter ablation in the region of papillary muscles (PMs) and valvular cusps (VC) on mitral, tricuspid, or aortic valve function. BACKGROUND: Ventricular arrhythmias arising from PMs and VCs often require extensive catheter ablation. Little is known regarding the risk of valve dysfunction after radiofrequency catheter ablation of such arrhythmias. METHODS: A retrospective analysis was completed for 149 PM and VC VT/premature ventricular contraction (PVC) ablations from 2008 to 2018 at our institution. Patient and procedural d

Adverse hemodynamic effects observed with inhaled nitric oxide after surgical repair of total anomalous pulmonary venous return.

The following is a case report of a 1-month-old patient who developed adverse hemodynamic sequelae during the use of nitric oxide (NO) in the postoperative period for pulmonary hypertension after correction of total anomalous pulmonary venous return. At the time of diagnosis, the patient had evidence of systemic right ventricular pressures estimated by continuous-wave Doppler. He was sedated and paralyzed for hyperventilation in preparation for surgery and underwent pulmonary vein confluence to left atrial anastomosis. Postoperative pulmonary hypertension was managed by hyperventilation, sedat

Activation of sarcolipin expression and altered calcium cycling in LMNA cardiomyopathy.

Cardiomyopathy caused by A-type lamins gene (LMNA) mutations (LMNA cardiomyopathy) is associated with dysfunction of the heart, often leading to heart failure. LMNA cardiomyopathy is highly penetrant with bad prognosis with no specific therapy available. Searching for alternative ways to halt the progression of LMNA cardiomyopathy, we studied the role of calcium homeostasis in the evolution of this disease. We showed that sarcolipin, an inhibitor of the sarco/endoplasmic reticulum (SR) Ca2+ ATPase (SERCA) was abnormally elevated in the ventricular cardiomyocytes of mutated mice compared with w

Quantitative 4D flow MRI assessment of left ventricular hemodynamics in bicuspid aortic valve across regurgitation severities and its association with myocardial deformation.

Bicuspid aortic valve (BAV) extends beyond aortic valve and wall disease, also affecting the left ventricle (LV). It leads to energetic alterations, myocardial deformation, and diastolic dysfunction, which are further exacerbated by persistent aortic regurgitation. As such, we aimed to assess LV dynamic reciprocal energy adjustments and their association with structural remodeling in BAV considering regurgitation severities. Seventy-six individuals who underwent 4D flow magnetic resonance imaging (4D Flow MRI) were recruited, including 29 BAV patients without valvular complications, 12 with mi

Aortic regurgitation assessment by cardiovascular magnetic resonance imaging and transthoracic echocardiography: intermodality disagreement impacting on prediction of post-surgical left ventricular remodeling.

Transthoracic echocardiography (TTE) is the primary clinical imaging modality for the assessment of patients with isolated aortic regurgitation (AR) in whom TTE's linear left ventricular (LV) dimension is used to assess disease severity to guide aortic valve replacement (AVR), yet TTE is relatively limited with regards to its integrated semi-quantitative/qualitative approach. We therefore compared TTE and cardiovascular magnetic resonance (CMR) assessment of isolated AR and investigated each modality's ability to predict LV remodeling after AVR. AR severity grading by CMR and TTE were compared
