Interventions for helping to turn term breech babies to head first presentation when using external cephalic version.

BACKGROUND: Breech presentation is associated with increased complications. Turning a breech baby to head first presentation using external cephalic version (ECV) attempts to reduce the chances of breech presentation at birth, and reduce the adverse effects of breech vaginal birth or caesarean section. Tocolytic drugs and other methods have been used in an attempt to facilitate ECV. OBJECTIVES: To assess interventions such as tocolysis, fetal acoustic stimulation, regional analgesia, transabdominal amnioinfusion or systemic opioids on ECV for a breech baby at term. SEARCH METHODS: We searche

A novel device for the study of somatosensory information processing.

Current methods for applying multi-site vibratory stimuli to the skin typically involve the use of multiple, individual vibrotactile stimulators. Limitations of such an arrangement include difficulty with both positioning the stimuli as well as ensuring that stimuli are delivered in a synchronized and deliberate manner. Previously, we reported a two-site tactile stimulator that was developed in order to solve these problems (Tannan et al., 2007a). Due to both the success of that novel stimulator and the limitations that were inherent in that device, we designed and fabricated a four-site stimu

Laser Doppler vibrometry of the middle ear in humans: derivation dependence, variability, and bilateral differences.

OBJECTIVE: Derivation dependence, inter- and intrasubject/intertest variability, bilateral differences of the eardrum vibration characteristics have been investigated using laser Doppler vibrometry (LDV). MATERIAL AND METHODS: A total of 31 normally hearing adults were examined. In each subject, both ears were consecutively stimulated by the chirp acoustic stimulus that covered 500-3700-Hz frequencies. The laser beam was directed to and the reflection was consecutively picked up from the tympanic membrane surface. RESULTS: LDV curves derived from different eardrum loci possessed dissimilar c…

Disturbed sensorimotor processing during control of precision grip in patients with writer's cramp.

The aim of the study was to investigate force regulation in patients with writer's cramp when performing a drawer-opening task using the precision grip. Experimental conditions included intervening load pulses and vibratory manipulations for examining grip force responses to sensory disturbances. The data revealed that grip force was increased in patients with writer's cramp compared with normal subjects, with a stronger modulation in the symptomatic compared with the asymptomatic hand. This denotes a change in force scaling capabilities and most notably for the preferred hand used in manipula

The vibroacoustic disease--an emerging pathology.

Vibroacoustic disease (VAD) is the clinical manifestation of a systemic disease that develops after long-term exposure to noise (> or = 10 yr) which is characterized by large pressure amplitude (> or = 90 dB SPL) within the lower frequency bands (< or = 500 Hz). Noisy environments produce more widespread systemic effects than initially suspected. This paper describes the chronology, population studied, and the statistical treatment used in our studies of VAD over the past 20 yr. We also describe the motivation that led to the evaluation and assessment of the exposed employees. Future research

Quantification of videostrobolaryngoscopic findings--measurements of the normal glottal cycle.

Videostrobolaryngoscopic (VSL) images of vocal fold behavior in 32 male and 33 female normal speakers were analyzed by computer. Image analysis software enhanced the glottal image, allowing extraction of the glottal area. Frame-by-frame analysis produced 20 glottal area values for each glottal cycle. The glottal area waveform (GAW) plots these values. From the GAW, features such as peak glottal area, duration of opening and closing, and rates of vocal fold opening and closing were measured. Acoustic measures of sound pressure level (SPL) and fundamental frequency were used to compare changes i

Stretch reflex latencies in spastic hemiparetic subjects are prolonged after transcutaneous electrical nerve stimulation.

Low-intensity repetitive electrical stimulation such as dorsal column and transcutaneous electrical nerve stimulation (TENS) reportedly decreases spasticity and improves voluntary motor control. However, the mechanisms mediating these effects are unclear. Recent findings suggest that spasticity may be characterized more appropriately by a decrease in the stretch reflex threshold than by an increase in gain. Our objectives were: (1) to examine possible changes in stretch reflex excitability following 45 min of TENS, (2) to map out the time course of possible post-stimulation effects via both la

[Surgery cures numbness of the hand. Long-term follow-up of carpal tunnel decompression].

One hundred and ninety-one hands with suspected carpal tunnel syndrome (CTS) were treated with surgical decompression of the median nerve. In a retrospective analysis 2.5-3 years later, an inquiry was carried out by questionnaire and the patients' records were examined to evaluate the final outcome. The questionnaire was answered by all 158 still living patients (ie, 175 hands), of whom 91 per cent reported numbness in the hand to have been completely relieved after surgery, while 23 per cent had had a recurrence of numbness, or other hand problems not clearly related to CTS, though just under

Diagnostic value of quantitative sensory testing (QST) in carpal tunnel syndrome.

The initial sensory symptoms of carpal tunnel syndrome (CTS) are usually intermittent and the clinical neurological examination is often normal. The aim of the present study was to determine the rate of impairment of different somatosensory modalities in CTS by means of the following tests: vibrametry, tactile pulses, von Frey hairs, two-point discrimination (2-PD), graphesthesia and warm and cold perception thresholds. The material consisted of 33 hands with CTS from 22 patients. Each of the first 3 tests was abnormal with elevated thresholds in 17 CTS hands (52%), 2-PD was abnormal in 10 han

Facilitation of the maximum voluntary contraction in hemiplegia by concomitant cutaneous stimulation.

The effects of rapid brushing on the maximum voluntary contraction (MVC) and tonic vibratory reflex (TVR) of various muscles requiring facilitation (quadriceps, biceps femoris and tibialis anterior) were studied in two samples of hemiplegics. Brushing was studied under experimental and placebo conditions. In the first experiment, brushing produced significant facilitation of MVC and TVR in the quadriceps, replicating the findings of Spicer and Matyas (1980) and Matyas and Spicer (1980). However, the effect of the TVR failed to predict the gain in MVC that resulted from brushing. In the second

A comparative electrophysiological and histological study of sensory conduction velocity and Meissner corpuscles of the median nerve in pneumatic tool workers.

The sensory conduction velocity of the median nerve has been determined according to the technique described by Buchthal and Rosenfalck in 32 pneumatic tool workers and in 32 normal controls of the same age group. In each worker and in 8 normals of the same age group the Meissner corpuscles of the skin of the thumb of the side investigated were studied according to the technique described by Ridley and Dickens et al. The only relevant electrophysiological finding was represented by a decreased amplitude of the evoked response of the median nerve at the wrist, probably pointing to some degree o

A Multimodal Stimulation System for Conveying Diverse Feedback in Hand Prosthetics: Preliminary Assessment.

Artificial somatosensory feedback plays a crucial role in compensating for tactile and proprioceptive loss in prosthesis users. Although modern prosthetic systems can acquire rich sensory data, effectively conveying this multimodal information to the user remains a significant challenge. This study presents a wearable somatosensory feedback armband with two configurations: a multimodal version using combined vibrotactile-electrotactile (VEC) stimulation, and a unimodal version based on vibrotactile-only (VO) stimulation. In both configurations, proprioceptive feedback is conveyed via spatiotem

Interactions Between Vibroacoustic Discomfort and Visual Stimuli: Comparison of Real, 3D and 360 Environments.

The building industry and the design of interior environments are increasingly focusing on the user experience, incorporating sensory analysis to reconsider how office environments can be optimized. New immersive technologies offer significant opportunities for sensory science, enhancing our understanding of human perception and enabling the collection of multi-sensory data under controlled laboratory conditions. While the potential of Virtual Reality (VR) for these types of studies is well recognized, certain limitations still need to be addressed, including the lack of standardized research

Assigning viscoelastic and hyperelastic properties to the middle-ear soft tissues for sound transmission.

A finite element model of the human ear considering viscoelasticity and hyperelasticity of the middle-ear (ME) soft tissues is developed in this paper. The present model is validated by comparing the static and dynamic responses to experimental data. The model-derived results are in good agreement with existing measurements. On this basis, the dynamic response of the ME under static pressure is re-evaluated. The results show that the static pressure mainly affects the low-frequency responses of the ME below 1000 Hz. In the case of static pressure preloading, the low-frequency displacement of t

Decoding vibrotactile choice independent of stimulus order and saccade selection during sequential comparisons.

Decision-making in the somatosensory domain has been intensively studied using vibrotactile frequency discrimination tasks. Results from human and monkey electrophysiological studies from this line of research suggest that perceptual choices are encoded within a sensorimotor network. These findings, however, rely on experimental settings in which perceptual choices are inextricably linked to sensory and motor components of the task. Here, we devised a novel version of the vibrotactile frequency discrimination task with saccade responses which has the crucial advantage of decoupling perceptual
