Hypoxanthine guanine phosphoribosyltransferase (HGPRT) in Gilles de la Tourette syndrome.

Hypoxanthine guanine phosphoribosyltransferase (HGPRT) and adenosine phosphoribosyltransferase (APRT) were examined from 11 individuals with Gilles de la Tourette syndrome, 10 of their first- or second-degree relatives, and 3 normal controls. It has been suggested that in some self-mutilating Tourette patients, HGPRT shows a time-related loss of activity at 4 degrees C, and an unusual isoelectrofocusing pattern. Although 3 patients experienced self-mutilation, no consistent abnormalities were found in the temperature-stability of their HGPRT at 4 degrees C and 70 degrees C, or in isoelectrofoc…

Mode of action of yeast toxins: energy requirement for Saccharomyces cerevisiae killer toxin.

The role of the energy status of the yeast cell in the sensitivity of cultures to two yeast toxins was examined by using 12K release from ce … rgy poisons in cultures. The results indicate that an energized state, maintained at the expense of adenosine 5'-triphosphate from either glycolytic or mitochondrial reactions, is required for the action of the killer toxin on the yeast cell.

The activities of plasma-membrane marker enzymes in rheumatoid synovial tissues and fluids.

The activities of 5'-nucleotidase, K+, Na+-activated Mg2+-dependent adenosine triphosphatase (ATPase) and leucine-beta-naphthylamidase were determined from 17 rheumatoid synovial fluids and from extracts of the corresponding synovial tissues. There was little correlation between the enzyme activities in the synovial fluids and those in the respective synovial-tissue extracts. In seropositive cases of rheumatoid arthritis the activities of 5'-nucleotidase and leucine-beta-naphthylamidase in the synovial-tissue extract were higher than in seronegative cases. Also, the ratios of the enzyme activi…

Alkaline phosphatase activity in the human tonsils and its relation to tonsillar diseases.

Alkaline phosphatase activity was examined in the human tonsils in fetal life and after repeated attacks of acute tonsillitis and in quinsy. Gomori's metal precipitate technique was used to demonstrate the phosphatase activity using four different substrates: sodium beta-glycerophosphate and adenosine triphosphate at pH 9, riboflavin 5-phosphate at pH 9.2 and 5-monophosphoric acid at pH 8.3. (2) The phosphatase activity differs somewhat according to the phosphate ester used as a substrate illustrating an example of 'substrate specificity'. (3) Alkaline phosphatase activity was increased in the…

Mycoplasma: isolation from prepuce and semen of bulls.

Mycoplasmas were isolated from the prepuce of 12 of 45 bulls and the semen of 2 of the 12 preputial-positive animals, and also from the semen of 1 of the 33 bulls which had not yielded mycoplasmas in the preputial sample. It appears, therefore, necessary to examine both preputial material and semen to detect potential shedders of Mycoplasma. Addition of fresh yeast extract to a solid medium of Bacto PPLO agar + Bacto PPLO serum fraction (1 %) + horse blood (5 %), and 0.005 % of 8 nucleosides (adenosine, cytidine, deoxyadenosine, deoxycytidine, deoxyguanosine, guanosine, thymidine, and uridine)…

Invasive Physiologic Assessment: From Fractional Flow Reserve to Full Physiology and Beyond.

Over 3 decades, invasive coronary physiology has evolved into a cornerstone of cardiovascular care. Fractional flow reserve (FFR) enabled lesion-specific assessment of epicardial stenoses, improving percutaneous coronary intervention (PCI) selection beyond angiography. Recognition of ischemia without obstructive disease expanded its role, with contemporary testing integrating adenosine-based flow assessment and acetylcholine-based evaluation of endothelial and vasomotor function, allowing identification of angina with nonobstructive coronary arteries endotypes, and tailored therapy. Physiologi…

A Case of Tuberculous Pleurisy Highlighting the Need to Consider Primary Tuberculosis in Interferon-Gamma Release Assay-Negative Patients.

We report a case of tuberculous pleurisy in a 54-year-old man who initially presented with negative interferon-gamma release assay (IGRA) results and elevated pleural fluid adenosine deaminase (ADA) levels. Bacteriological confirmation was obtained only when the pleural fluid culture grew Mycobacterium tuberculosis one month later. The IGRA yielded positive results approximately two months after symptom onset. This case highlights the limitations of IGRAs and ADA as definitive diagnostic tools and underscores the need to consider primary tuberculosis in early IGRA negative cases. Clinicians sh…

Tuberculous Meningitis With Persistent Hiccups: A Case Report.

A 69-year-old Japanese male presented with atypical symptoms of tuberculous meningitis (TBM), including intermittent left temporal headache and persistent hiccups that were unresponsive to initial therapies. Despite comprehensive imaging, no definitive intracranial lesions were identified. His persistent hiccups, along with typical meningitis symptoms, highlighted the diagnostic challenges of TBM, particularly in the context of negative microbiological tests. Early empirical treatment, initiated based on clinical suspicion and elevated cerebrospinal fluid (CSF) adenosine deaminase levels, led …

Advances in the treatment of severe combined immunodeficiency.

Severe Combined Immunodeficiency (SCID) is the most profound inborn error of immunity affecting cellular and humoral immunity. Hematopoietic stem cell transplantation has been a curative treatment since 1968. Huge progress has been made since then in understanding the underlying genetics, improving outcomes from transplant, and introducing gene therapy in particular for adenosine deaminase deficient- and IL2 receptor gamma-deficient SCID. Newborn screening has been widely introduced across the world to enable definitive treatment before infection occurs. This article aims to review the latest …

Deficiency of ADA2 mimicking autoimmune lymphoproliferative syndrome in the absence of livedo reticularis and vasculitis.

Adenosine deaminase-2 (ADA2) deficiency (DADA2) is associated with early onset polyarteritis nodosa and vasculopathy. Classic presentation includes livedo reticularis, vasculitis, and stroke. However, the phenotype and disease severity are variable. We present a 5-year-old female who presented with features that mimicked autoimmune lymphoproliferative syndrome (ALPS) in the absence of classic features of DADA2. Exome sequencing identified a novel homozygous splicing variant in ADA2 c.882-2A > G. Patient responded to anti- tumor necrosis factor medication and is in complete remission. Hematolog…

Dyserythropoiesis in a child with pyruvate kinase deficiency and coexistent unilateral multicystic dysplastic kidney.

Pyruvate kinase (PK) deficiency is the commonest enzyme deficiency in the glycolytic pathway leading to hemolytic anemia secondary to decreased Adenosine Triphosphate (ATP) synthesis in the red cells. synthesis. PK deficiency due to mutations in the PKLR (1q21) gene leads to highly variable clinical presentation ranging from severe fetal anemia to well compensated anemia in adults. We describe dyserythropoiesis in the bone marrow of a child with transfusion dependent anemia and unilateral multicystic dysplastic kidney (MCDK) mimicking Congenital Dyserythropoietic Anemia type I (CDA type I). Pe…

CREB-binding protein modulates repeat instability in a Drosophila model for polyQ disease.

Although expansion of trinucleotide repeats accounts for over 30 human diseases, mechanisms of repeat instability remain poorly understood. We show that a Drosophila model for the CAG/polyglutamine (polyQ) disease spinocerebellar ataxia type 3 recapitulates key features of human CAG-repeat instability, including large repeat changes and strong expansion bias. Instability is dramatically enhanced by transcription and modulated by nuclear excision repair and a regulator of DNA repair adenosine 3',5'-monophosphate (cAMP) response element-binding protein (CREB)-binding protein-a histone acetyltran…

[A case of tuberculous pleurisy with a rapid decrease in pleural effusion].

We report a rare case of tuberculous pleurisy with a rapid decrease in pleural effusion by levofloxacin. A 73-year-old woman admitted due to dyspnea on exertion of one month duration. She had aortitis syndrome which had been treated with oral prednisolone for 6 months. Chest roentgenogram showed left pleural effusion, which was exudative with lymphocyte predominance and an increased level of adenosine deaminase. One-week administration of levofloxacin remarkably decreased effusion. as demonstrated on chest roentgenogram 2 weeks after treatment. Since Mycobacterium tuberculosis was isolated in …

Caffeine analogs: biomedical impact.

Caffeine, widely consumed in beverages, and many xanthine analogs have had a major impact on biomedical research. Caffeine and various analogs, the latter designed to enhance potency and selectivity toward specific biological targets, have played key roles in defining the nature and role of adenosine receptors, phosphodiesterases, and calcium release channels in physiological processes. Such xanthines and other caffeine-inspired heterocycles now provide important research tools and potential therapeutic agents for intervention in Alzheimer's disease, asthma, cancer, diabetes, and Parkinson's d…

[The effect of cyclic nucleotides on the membrane potential of intact endothelium of isolated rabbit aorta].

The influence of intracellular cyclic nucleotides concentration increase on the membrane potential of intact endothelium of isolated rabbit aorta has been investigated. Both applications of bucladesine, an adenosine 3',5'-cyclic monophosphate (cAMP) membrane permeable analogue, and phoshophodiesterase blockers 3-isobutyl-1-methylxanthine (IBMX) and theophylline depolarized endothelial membrane. IBMX-induced depolarization was significantly smaller then theophylline-induced depolarization. It could be explained by the existence of IBMX-insensitive phosphodiesterases. Non-selective protein kinas…

Intravenous calcium chloride in the conversion of paroxysmal supraventricular tachycardia to normal sinus rhythm.

This brief report describes several cases of paroxysmal supraventricular tachycardia that converted promptly to normal sinus rhythm within 1 to 2 minutes of receiving intravenous calcium salts as pretreatment in anticipation of verapamil therapy. A review of calcium's hemodynamic and dromotropic effects suggests that this probably was due to electrophysiological effects rather than mere coincidence. Calcium raises blood pressure, which may reflexively increase cardiac parasympathetic tone, and also has a direct slowing effect on atrioventricular conduction. Adenosine remains the drug of choice…

Severe aplastic anemia induced by ticlopidine: report of a case.

Ticlopidine is a powerful antiplatelet activator that inhibits adenosine diphosphate (ADP)-induced platelet aggregation. Its most common side-effects are skin rashes, diarrhea and neutropenia. Aplastic anemia is rare. This paper reports a patient with severe aplastic anemia that developed after the use of ticlopidine. The 85-year-old woman developed fever, chills and chest pain 5 weeks after starting ticlopidine 250 mg twice daily. Severe aplastic anemia was proved by blood examination, bone marrow aspiration and biopsy. In spite of the recovery of absolute neutrophil count to more than 1,000/…

Effects of ceruletide, administered singly and in combination with central-acting drugs, on discrete shuttle avoidance response in mice.

The single administration of ceruletide at 10-300 micrograms/kg, i.p., slightly but significantly decreased the response rate (frequency of shuttles) under a discrete avoidance task in mice. Over 10 micrograms/kg of ceruletide attenuated the increase in the response rate induced by methamphetamine (0.5 mg/kg, s.c.). However, ceruletide, at 1-300 micrograms/kg, did not significantly enhance the response- and/or avoidance-decreasing effects of chlorpromazine (1 mg/kg, s.c.), haloperidol (0.1 mg/kg, s.c.), pilocarpine (4 mg/kg, s.c.) and N6-(L-2-phenylisopropyl)-adenosine (0.1 mg/kg, s.c.), but r…

Pseudohypoparathyroidism.

Pseudohypoparathyroidism includes a genotypically diverse group of syndromes of primary resistance to hormones whose actions are mediated by cyclic adenosine 3':5'-monophosphate. In most cases, clinical presentation results from resistance to actions of parathyroid hormone, while resistance to other hormones remains subclinical. The largest subgroup of patients with this disorder, characterized by a typical somatic phenotype termed Albright's hereditary osteodystrophy, exhibits generalized deficient activity of a membrane protein that couples hormone receptors to stimulation of adenylate cycla…

Management of salicylate intoxication.

Salicylate intoxication is common. It results in impaired generation of adenosine triphosphate and produces a primary respiratory alkalosis. In adults the clinical manifestations may closely simulate a cerebrovascular event or alcoholic ketoacidosis. Central nervous system dysfunction, fever, glycosuria, ketonuria, respiratory alkalosis with an elevated anion gap, tinnitus, dehydration, hypokalaemia and haemostatic defects are common. The diagnosis may be made rapidly by the ferric chloride test or Phenistix test. Standard therapy includes gastric emptying, activated charcoal and alkalinisatio…
