Intravenous antibiotic therapy in an outpatient setting.

A program has been developed for the outpatient administration of parenteral antibiotics. To date, more than 150 patients with osteomyelitis, septic arthritis, pyelonephritis, endocarditis, and other infections have been treated. Antibiotic solutions were prepared in the hospital pharmacy and given to the patient to be kept refrigerated at home until used. Patients administered their own antibiotics by means of a heparin lock, which was replaced every four days or when necessary. Complications were infrequent. Many patients were able to return to work while receiving therapy; others enjoyed th

Etiology, Clinical Features, and Outcomes of Acute Poisoning Among Children at a Provincial Pediatric Hospital in Vietnam: Retrospective Descriptive Study.

BACKGROUND: Acute poisoning in children remains a significant cause of emergency admission and is largely preventable. Its epidemiological profile varies across settings according to age, household environment, and locally available toxic agents. Understanding the patterns, management, and outcomes of pediatric poisoning is important for improving prevention and clinical care. OBJECTIVE: This study aimed to characterize the demographic and exposure patterns, treatment practices, and in-hospital outcomes of acute poisoning among children treated at a provincial pediatric hospital in Vietnam.

Reduction in Fire-Related Admissions to a Large, Regional Burn Center After Prevention and Risk Mitigation Interventions.

Residential fires remain a leading cause of severe burn-related injuries and fatalities, particularly in socioeconomically disadvantaged regions. Despite existing fire safety standards, limited resources in low-income communities hinder effective prevention. This study evaluates the impact of community-based fire risk-mitigation strategies on fire-related burn admissions in a high-poverty urban area. A retrospective review of all fire-related burn admissions to a regional burn center in Memphis, Tennessee, was conducted over a 5-year period (July 1, 2019 to June 30, 2024). Only incidents occur

When Prevention is an Emergency: The Imperative of Contraception Initiation in the Emergency Department.

BACKGROUND: Millions of U.S. women live in contraceptive deserts, with limited or no access to comprehensive reproductive health services. Barriers are intensified by Medicaid restrictions, provider shortages, and growing reproductive health inequities. The emergency department (ED), as the healthcare safety net, is uniquely positioned to expand contraceptive access. OBJECTIVES: To highlight the public health need for ED-initiated contraception, describe feasible approaches for implementation, and address common challenges to integrating contraception into emergency care. DISCUSSION: Women o

Changes in the epidemiology of paediatric brain abscess in China: a nationwide study during the years 2016-2023.

UNLABELLED: Paediatric brain abscess (BA) is a rare but life-threatening condition, and large-scale epidemiological studies are limited. We aimed to characterize the temporal trend of pediatric BA in China during 2016-2023. We conducted a nationwide study using the FuTang Update Medical Records (FUTURE) database, which includes face-sheets of discharge medical records from 33 tertiary children's hospitals across China. Case-identification strategies were validated, and the optimal strategy was applied to identify BA cases. Demographic information, predisposing factors, complications, and clini

Beyond capacity limits: can social cohesion offset the impact of service constraints on youth mental health?

BACKGROUND: Rates of youth anxiety, depression, and self-harm have increased substantially in recent years. Expansion of clinical service capacity is constrained by workforce shortages and system fragmentation, and even substantial investment may not achieve the scale of growth required to address unmet need. Preventive strategies - such as strengthening social cohesion - are therefore essential to alleviate mounting pressures on the mental health system, yet their potential to compensate for these constraints remains unquantified. METHODS: This study employed a system dynamics model to explo

Using a deterministic matching computer routine to identify hospital episodes in a Brazilian de-identified administrative database for the analysis of obstetrics hospitalisations.

INTRODUCTION: The absence of a unique patient identifier in the Brazilian hospital administrative database prevents the identification of hospital episodes with multiple hospitalisations of the same patient. OBJECTIVES: This study aims to evaluate the information gain by using a computer routine to identify acute Obstetrics hospital episodes and its impact on assessing marks of case severity. METHODS: The data source was a de-identified Brazilian hospital administrative database from 2017 to 2020, including hospitalisations records of women of reproductive age (10 to 49 years old) for treati

The 2024 resurgence of Bordetella pertussis in Brazil and a decade-long epidemiological overview.

Pertussis, or whooping cough, is a highly contagious respiratory illness caused by Bordetella pertussis. It primarily affects humans by adhering to the cilia of the upper respiratory tract, releasing toxins that impair ciliary function, and inducing inflammation. It is transmitted through respiratory droplets, and the disease is particularly severe in infants under 1 year of age, often resulting in high morbidity and frequent admission to intensive care units. Globally, the incidence of pertussis has been increasing, with notable increases observed in countries such as the United States, Brazi

Calculating Carbon Reduction via Pediatric Outreach Clinics From a Surgical Patient Cohort.

INTRODUCTION: Transportation is a major contributor to carbon emissions. This study evaluates the carbon footprint reduction associated with surgical outreach clinics as an alternative to patients traveling long distances to a tertiary medical center. METHODS: We retrospectively analyzed pediatric pectus excavatum (PE) patients seen in two outreach clinics between 2012 and 2024. Distances from patients' homes to both the children's hospital and outreach clinics were calculated. Using the average distance traveled per PE patient and Environmental Protection Agency's emission factor of 400 g of

Norwegian scabies seen at the University of Nigeria Teaching Hospital, Ituku-Ozalla, Enugu State: a case report.

BACKGROUND: Norwegian scabies, also known as crusted scabies, is a severe form of scabies caused by an infestation of the skin by the Sarcoptes scabiei var. hominis mite. It is usually diagnostically challenging because of its rare nature and the potential for misdiagnosis with other skin diseases. This case report presents a rare case of Norwegian scabies seen at the University of Nigeria Teaching Hospital, Ituku-Ozalla, Enugu State. CASE PRESENTATION: The patient, a 16-year-old male Human Immunodeficiency Virus (HIV) positive patient, diagnosed with HIV 2 years before presentation but was n

The COVID-19 pandemic in children and young people during 2022-24: what new did we learn?

The research conducted between 2022 and 2024 has advanced our understanding of COVID-19 in children and young people (CYP), particularly with the emergence of the Omicron variant and its subvariants. The findings have reinforced that, while Omicron infections are often milder compared to earlier variants, the overall seroprevalence of SARS-CoV-2 in children has increased, with notable regional and demographic disparities. COVID-19-related hospitalisation rates in children rose during Omicron waves, especially among infants, unvaccinated individuals, and CYP at higher risk, i.e. with comorbidit

Racial and Ethnic Disparities in Patient Restraint in Emergency Departments by Police Transport Status.

IMPORTANCE: Black patients are more likely than White patients to be restrained during behavioral crises in emergency departments (EDs). Although the perils of policing mental health for Black individuals are recognized, it is unclear whether or to what extent police transport mediates the association between Black race and use of physical restraint in EDs. OBJECTIVE: To evaluate the degree to which police transport mediates the association between Black race and use of physical restraint in EDs. Using mobile health to expedite access to specialty care for youth presenting to the emergency department with concussion at highest risk of developing persisting symptoms: a protocol paper for a non-randomised hybrid implementation-effectiveness trial.

INTRODUCTION: Paediatric concussion is a common injury. Approximately 30% of youth with concussion will experience persisting postconcussion symptoms (PPCS) extending at least 1 month following injury. Recently, studies have shown the benefit of early, active, targeted therapeutic strategies. However, these are primarily prescribed from the specialty setting. Early access to concussion specialty care has been shown to improve recovery times for those at risk for persisting symptoms, but there are disparities in which youth are able to access such care. Mobile health (mHealth) technology has th

Point-of-Care Ultrasound Evaluation of Pediatric Osteomyelitis in the Emergency Department: A Case Series.

Musculoskeletal and soft tissue complaints commonly present to the pediatric emergency department and literature supports the use point-of-care ultrasound (POCUS) in the diagnosis of these complaints. The diagnosis of osteomyelitis typically involves laboratory testing with inflammatory biomarkers, imaging with x-ray, and often magnetic resonance imaging with test results often not immediately available. We report a case series of children initially evaluated with POCUS for osteomyelitis. The POCUS may expedite diagnosis and treatment when used as the initial test in children with suspected os
