PURPOSE: In this report we describe an alternative approach to catheter placement for continuous selective median nerve blockade. It spared the finger movements and therefore allowed early postoperative rehabilitation in a patient who underwent surgical repair of the index finger flexor tendon.
CLINICAL FEATURES: A patient with a complicated history of traumatic index finger flexor tendon rupture, surgical repair, failed rehabilitation due to poor postoperative pain control, adhesion formation, and subsequent rerupture due to tenolysis was admitted for reconstructive surgery. This time, a con

Ocular complications, transient loss of vision and diplopia, and blanching of the skin of the infraorbital region were reported in a female patient after an inferior alveolar nerve block for extraction of the permanent mandibular left third molar tooth. Injection of the anesthetic solution into the maxillary artery could result to such complications. The anatomy related to this case, with suggestions for management of such a patient is discussed.

OBJECTIVES: We aimed to determine the effects of intraperitoneal administration of levobupivacaine on pain after laparoscopic cholecystectomy in a prospective, randomized, double-blinded, placebo-controlled trial.
METHODS: In all patients, infiltration of levobupivacaine 0.25% (15 mL) was used prior to skin incisions for trocar insertion. After pneumoperitoneum was achieved, patients were allocated randomly to receive intraperitoneally either 40 mL of 0.25% levobupivacaine (LB group, n=20) or normal saline (NS group, n=20) under direct vision into the hepatodiaphragmatic lodge and above the g

Local anesthetic is administrated intrathecally to produce spinal anesthesia. This study examines the effect of spinal anesthesia induced by intrathecal tetracaine on cerebral pial microcirculation in rats. We monitored changes in the mean arterial pressure, internal diameter (ID) of the pial arteriole, intracranial pressure, and red blood cell velocity. The regional cerebral blood flow was calculated from the product of cross sectional area and red blood cell velocity. To induce spinal anesthesia, tetracaine was administered via a polyethylene tube at the L4-5 intervertebral space after lamin

BACKGROUND: Lesser metatarsal overload may lead to pathologic conditions ranging from plantar metatarsophalangeal joint pain to dislocation. The flexor tendon sheath lies directly plantar to the joint. The increased pressure associated with lesser metatarsal overload may lead to synovitis and pain of the flexor tendon sheath.
METHODS: Fifteen consecutive patients with lesser metatarsal overload had visual analog scale scores determined at three metatarsophalangeal joint areas to determine the source of pain. The patients underwent seven maneuvers to determine the pain scale score: single-leg

OBJECTIVE: Rhinophyma is characterized by an enlarged, bulbous, and red nose resulting from the enlargement of the sebaceous glands beneath the surface of the skin on the nose. Its etiology remains unclear. Treatment of the disease is controversial. In this study, we discussed the treatment of rhinophyma by radiofrequency in 5 patients.
CLINICAL REPORT: We presented 5 patients with rhinophyma. The patients' median age was 57.4 years. A radiofrequency unit was used in cut/coagulate mode at 10 U. A 0.38-in loop electrode was placed ensuring symmetry until the nose's shape was re-created. All pa

Despite a favorable clinical experience, there are little evidence existing about the effects of greater occipital nerve (GON) block in migraine treatment. In an open, preliminary trial we evaluated the use of GON block with 0,5 % bupivacaine, in prevention of migraine attacks. Ten women suffering from migraine diagnosed according to International Headache Society criteria were evaluated in a six-month study period. During the treatment and entire follow-up period, the patients avoided prophylactic therapy. Patients were given 3 times GON blocks with 0.5 % bupivacaine one week intervally. Afte

The difficulty of anesthetic management of the critically ill, especially cardiovascular risk patients is discussed. The usually applied practice of general anesthesia or neuroaxial blockades for bilateral orchiectomy has a considerable cardiovascular risk profile. This case report presents an alternative procedure with an ilioinguinal nerve blockade completed by periscrotal infiltration. This is a generally unknown and uncommon minimally invasive locoregional procedure, which is generally judged to be safe, efficient, economical with low grade side effects or complications and suitable for ou

The purpose of this study was to compare the efficacy of intraosseous injection and inferior alveolar (IA) nerve block in anesthetizing mandibular posterior teeth with irreversible pulpitis. Thirty human subjects were randomly assigned to receive either intraosseous injection using the IntraFlow system (Pro-Dex Inc, Santa Ana, CA) or IA block as the primary anesthesia method. Pulpal anesthesia was evaluated via electric pulp testing at 4-minute intervals for 20 minutes. Two consecutive 80/80 readings were considered successful pulpal anesthesia. Anesthesia success or failure was recorded and g

PURPOSE: Cystoscopy is one of the most common examinations in urologic outpatient clinics. Various anesthetic approaches have been used to make cystoscopy more tolerable for patients. The aim of the present prospective randomized study was to evaluate the efficacy of lidocaine hydrochloride gel compared to dimethyl sulfoxide (DMSO) with lidocaine in rigid cystoscopy.
MATERIALS AND METHODS: Male patients requiring 17F rigid cystoscopy were eligible for inclusion in this study. A total of 140 patients were divided into two groups: group 1 (n=70) received approximately 11 mL of 2% lidocaine gel

We report a rare complication of spinal anesthesia-divergence paresis-which is characterized by an acquired horizontal diplopia at distance without evidence for abducens palsy. A 64-yr-old man underwent prostatectomy under spinal anesthesia with 2.5 mL of dibucaine hydrochloride 0.3% injected through a 20-gauge cutting-tip spinal needle. Seventeen days after the operation, the patient noticed horizontal diplopia for distant objects. Although cranial magnetic resonance imaging demonstrated diffuse pachymeningeal gadolinium enhancement and subdural effusion, characteristic findings of cerebrospi
