The review of 700 heart catheterizations in infants and children revealed arterial occlusion in 30 instances after 175 catheterizations (17,1%), but 29 of them after 88 arteriotomies (32,9%) and only 1 after 87 percutaneous arterial catheterizations (1,1%). In two cases thrombectomy of the femoral artery was necessary, brachial artery occlusion was never felt to indicate surgical intervention. Since percutaneous sheath catheterization is used in combination with systemic heparinization no obvious occlusion occurred after about 80 investigations. This may be due to a minimum of intimal damage b

OBJECTIVES: To assess the prevalence of aortitis and aortic dilation at diagnosis, estimate the progression of aortic diameter over time, and identify predictors of incident aortic dilation development in patients with newly diagnosed GCA.
METHODS: We conducted a retrospective cohort study involving 157 patients with new-onset GCA from two European centres. All patients underwent baseline thoracic aortic imaging within 6 months of diagnosis and at least one follow-up imaging ≥6 months later. Outcomes included baseline aortic diameter, aortic expansion (cm2/year), and incident aortic dilation.

Eosinophilic coronary periarteritis (ECPA) is a rare disease, almost exclusively diagnosed postmortem, characterized by a clinical course of vasospastic angina (VSA) attacks and histologically observed periarterial eosinophilic infiltration in the coronary arteries. Although the etiology of ECPA remains uncertain, its pathogenesis may be attributed to an allergic reaction. In addition, ECPA is often associated with spontaneous coronary artery dissection (SCAD); however, the pathogenesis underlying the link between inflammation and dissection remains unclear. In the current case, a 45-year-old …

Atherosclerosis is the underlying cause of cardiovascular disease and stroke. Endothelial cell dysfunctions are early events in atherosclerosis, resulting in the recruitment of circulating monocytes. The immune system can elicit an inflammatory response toward the atherosclerotic lesion, thereby accelerating lesion growth. Risk factors for atherosclerosis include hypertension, smoking, stress perception or low birth weight. As prenatal stress challenge decreases the birth weight and affects the offspring's postnatal immune response, we aimed to investigate whether prenatal stress contributes t

Aspergillus is the most common cause of fungal pneumonia in acute leukemia patients receiving chemotherapy or undergoing hematopoietic stem cell transplantation. Despite a high index of suspicion and prompt institution of specific antifungal therapy, it causes significant morbidity and mortality in patients with hematological malignancies. It has to be differentiated from mucormycosis because the treatment differs. Histological confirmation obtained by lung biopsy is ideal, but is difficult to obtain in those patients who often have thrombocytopenia. We report a case of acute megakaryoblastic

Aortitis syndrome is a chronic vasculitis that leads to arterial wall thickening and stiffening in large elastic arteries. However, there are no established markers for assessing arterial stiffening in aortitis syndrome. The cardio-ankle vascular index (CAVI) has recently been utilized to assess arterial stiffening that is associated with atherosclerosis-related diseases. We hypothesized that CAVI can be applicable for assessing alterations in arterial stiffness during immunosuppressive therapy for aortitis syndrome. Eosinophilic coronary monoarteritis is a rare condition of unexplained etiology involving selectively the left anterior descending coronary artery. This is a peculiar subset of the general pathoanatomic substrate of spontaneous coronary artery dissection. Dissection of the arterial wall ensues, complicated by acute coronary thrombosis and results in sudden death without any identifiable clinical features for a premortem diagnosis. Two recently investigated unrelated sudden deaths of young, otherwise completely healthy individuals, a white male and a white female, in the Oakland County Medical …

Non-arteritic anterior ischaemic optic neuropathy is the most common optic neuropathy of the elderly, characterised by unilateral, sudden, painless visual loss. No effective treatment has been proven to reverse or limit the course of this disease. We evaluated the role of intravitreal triamcinolone acetonide injection in eyes with non-arteritic anterior ischaemic optic neuropathy (NAION). Four eyes of four patients with acute NAION received a single intravitreal injection of triamcinolone acetonide (4 mg). The time between visual loss and intravitreal injection varied between four and 10 days.

PURPOSE: To compare the early and long-term outcomes of subclavian artery angioplasty in patients with aortoarteritis and atherosclerosis.
METHODS: Sixty-one subclavian artery angioplasties were performed in 55 consecutive patients with aortoarteritis (n = 32) and atherosclerosis (n = 23) between 1986 and 1995. An arch aortogram followed by a selective subclavian artery angiogram was done to profile the site and extent of the lesion, its relation to the vertebral artery, and the distal circulation. Percutaneous transluminal angioplasty (PTA) was performed via the femoral route for 56 stenotic…

A 67-year-old woman with a diagnosis of polymyalgia rheumatica presented initially with periods of confusion and incontinence. A CT scan of the brain was normal and she was treated with tapering doses of corticosteroids and clinical improvement. After a brief period off steroids, the patient presented with a progressive dementia, left-sided clumsiness, gait disturbances and left hemispatial neglect. An MRI at this time demonstrated a large area of edema over the right parietal lobe and intense cortical enhancement. A chest CT demonstrated multiple nodules. Biopsies of the lung and brain failed

True rheumatologic emergencies are rare but they do occur. In many cases, the initial assessment, diagnosis, and treatment will rest in the hands of the primary care physician. Most common is acute onset of inflammatory arthritis. Properly diagnosed, adequate to dramatic improvements are usual with appropriate treatment. No less pressing are the neurologic emergencies such as spinal stenosis, cauda equina syndrome, and GCA. Among these, the potential for neurologic complications is high, so prompt diagnosis and treatment are essential. Cervical spine involvement in RA is common; again, neurolo

A case of Takayasu's disease in a young Caucasian female is described. The major complications which developed over the ten year course of the disease include nephrotic syndrome, severe refractory hypertension, aortic valve regurgitation associated with aneurysmal dilatation of the ascending aorta, and recurrent congestive heart failure. Amyloid deposits have been demonstrated in the aorta, atrial appendage, aortic valve, and renal cortex. The association of amyloidosis and Takayasu's disease is discussed.

The authors report their experience of 17 cases of giant intracranial aneurysms some of them with particular forms and localisations: Giant serpentine aneurysms in 17.6%, carotid cavernous, carotid, Willis Circle and vertebral branches. 8 cases were operated, 2 by direct neck occlusion and 6 others by carotid or vertebral interruption. For the nine non operated cases 5 were in a very bad clinical condition and 4 had refused the operation. Aneurysmal neck occlusion gave uneventful results lasting more than 4 and 10 years. Carotid or vertebral branches occlusion showed 16.6% mortality. In one ca

The author report the cases of three women with aortic regurgitation associated with aneurysmal dilatation of the ascending thoracic aorta. Aortic valve replacement was carried out in the 3 cases, with resection of the aortic aneurysm in 2 cases. Histological examination of the aortic wall in all 3 cases showed non-specific aortitis in the adventitia and media, appearances comparable to those described in Takayasu's disease. The incidence of aortic regurgitation in Takayasu's disease is about 10%. These 3 cases are compared with 30 other cases with histological confirmation in the medical lite
