Do the deceleration/acceleration capacities of heart rate reflect cardiac sympathetic or vagal activity? A model study.

Despite increased application of the deceleration capacity (DC) and acceleration capacity (AC) of heart rate indices as indicators of autonomic nervous system (ANS) function, it remains controversial as to whether they reflect cardiac sympathetic or vagal activity. We addressed this problem using a cardiovascular system model that allows analysis of DC and AC under controllable levels of sympathetic and vagal activities. Multi-scale DCs and ACs with various timescales T and wavelet scales s were computed from the simulated RR interval series under randomly fluctuating levels of ANS activity, a…

Insomnia in Chinese Medicine: The Heart of the Matter.

Chronic insomnia affects a significant proportion of the general population worldwide, and is associated with several serious medical conditions. From the Western scientific literature, hyper-arousal (on the cognitive-emotional, behavioral, autonomic, or central nervous system level) is a final common pathway involved in its pathogenesis. However, from a Chinese medicine (CM) perspective, it is the Heart, capitalized to denote the functional system as described in CM theory, that is the key organ involved in insomnia due to its role as the "seat of consciousness." This article explores how ins

Cholinergic control of inflammation.

Cytokine production is necessary to protect against pathogens and promote tissue repair, but excessive cytokine release can lead to systemic inflammation, organ failure and death. Inflammatory responses are finely regulated to effectively guard from noxious stimuli. The central nervous system interacts dynamically with the immune system to modulate inflammation through humoral and neural pathways. The effect of glucocorticoids and other humoral mediators on inflammatory responses has been studied extensively in the past decades. In contrast, neural control of inflammation has only been recentl

Bladder and bowel dysfunction in Parkinson's disease.

Bladder dysfunction (urinary urgency/frequency) and bowel dysfunction (constipation) are common non-motor disorders in Parkinson's disease (PD). In contrast to motor disorder, the pelvic autonomic dysfunction is often non-responsive to levodopa treatment. Brain pathology mostly accounts for the bladder dysfunction (appearance of overactivity) via altered dopamine-basal ganglia circuit, which normally suppresses the micturition reflex. In contrast, peripheral enteric pathology mostly accounts for the bowel dysfunction (slow transit and decreased phasic contraction) via altered dopamine-enteric

The history of thought concerning the hypothalamus and its functions.

Many initial studies related to identification of the boundaries and structural components, nuclei, tracts and interconnections of the hypothalamus; this continues. Early interest also focused on hypothalamic control of somatic activities and autonomic nervous system functions. During the present century chiefly, interest has developed in the hypothalamus and control of water balance, thirst, water retention and loss (diabetes insipidus and polydipsia). Its role in control of metabolism, body weight (obesity), and the regulation of body temperature has attracted the attention of physiologists …

The role of the sympathetic system in producing cardiac disturbances determined by stimulation of the midbrain reticular formation in the cat.

The authors check the theory according to which cardiac rhythm disturbances developing after myocardial infarction or in the course of a long Q-T interval syndrome are due to stimulation of the autonomic nervous system which innervates the heart via the stellate ganglia, the tonus of the left sympathetic being predominant over the right sympathetic at cardiac level. Six semiacute experiments were carried out on curarized cats in which arrhythmia was produced by stereotaxic electric stimulation of the midbrain reticular formation (MRF). Stimulation of MRF after either left or right stellectomy …

Neuronal intranuclear hyaline inclusion disease with progressive cerebellar ataxia.

Neuronal intranuclear hyaline inclusion disease is a progressive, fatal neurologic condition characterized by eosinophilic inclusions in neurons of the central, autonomic, and peripheral nervous systems. The clinical and pathologic findings of a 4-year-old boy who presented with a rapidly progressive cerebellar ataxia and seizure disorder that had begun 2 years earlier are described. Although intraneuronal inclusions were identified in neurons of cortex, basal ganglia, brainstem, cerebellum, and spinal cord, clinical signs were restricted to cerebellar ataxia, internuclear ophthalmoplegia, and

Adverse effects of laxatives: fact and fiction.

Laxatives are generally well tolerated and may be considered safe drugs. When taken at much higher than the recommended doses (laxative abuse) some side effects may occur (e.g. hypokalemia, metabolic alkalosis, renal tubular damage). It is controversial whether the laxatives currently used may lead to morphologic changes of the autonomous nervous system of the colon. Melanosis coli is due to pigment-laden macrophages within the submucosa. It occurs after long-term intake of anthraquinones and has no functional consequences. No case of 'cathartic colon' has been observed during the last few dec

Perforation of the nasal septum in patients with renal failure.

Spontaneous perforation of the nasal septum was observed in 8 out of 104 patients (74 on maintenance hemodialysis; 30 after cadaveric renal transplantation). All patients showed a round or oval defect of the non-osseous septum, which was accompanied by marked atrophic rhinitis. Epistaxis was common in uremic patients in general and was also present in 5 patients with septal perforation, but otherwise the defect was hardly associated with any symptoms. Various factors may play a role in the pathogenesis of this lesion; local trauma from nasal catheters postoperatively; impaired mucosa cell prol

[Multiple recurrent paralysis of the cranial nerves (author's transl)].

The authors describe three patients presenting several episodes of paralysis of the motor and sensory cranial nerves and discuss possible aetiological factors. The paralytic attacks were regressive and were repeated at irregular intervals over a period of several years. It was confirmed that no local or general cause for the symptoms existed and the conclusion was reached that they were part of an autonomous clinical syndrome of unknown aetiology. Other cases have been reported in the published literature in which a non-specific inflammatory mechanism is suggested but no biological or patholog

Preliminary evidence of cardiac dysfunction in subacute sclerosing panencephalitis: a systematic review and meta-analysis.

PURPOSE: Subacute sclerosing panencephalitis (SSPE) is a catastrophic neurological disorder that typically presents in infancy and early adolescence. Cardiac arrhythmia in SSPE is a significant but often under-recognized complication of the disease. The persistent measles virus infection that characterizes SSPE can have widespread effects on the central nervous system, including areas that regulate autonomic function, which in turn can impact cardiac rhythm. In this article, we aim to comprehensively review articles describing cardiac dysfunction associated with SSPE. METHODS: We conducted a

Assessment of Autonomic Dysfunction in Patients With Empty Nose Syndrome Using the Composite Autonomic Symptom Scale-31.

BACKGROUND AND OBJECTIVES: Empty nose syndrome (ENS) can occur as a consequence of aggressive turbinate surgery. However, some patients with similar surgical histories do not develop ENS, suggesting that host-related factors may contribute to its onset. Dysregulation of the autonomic nervous system (ANS), which innervates the nasal mucosa, may play a role in triggering ENS. This study used the Composite Autonomic Symptom Scale-31 (COMPASS 31) questionnaire to compare ANS dysfunction between patients with ENS and asymptomatic controls. METHODS: Twenty-six patients with ENS and 43 control parti…

Peripheral Nervous System Involvement of Hereditary Transthyretin Amyloidosis in the United States: A Multi-Center Perspective.

INTRODUCTION/AIMS: Hereditary transthyretin amyloidosis (ATTRv) is an autosomal dominant multisystem disorder that occurs worldwide. The most common mutation in the United States, V142I, has previously been described as having a primarily cardiac presentation. However, the prevalence of peripheral neuropathy (PN) in V142I ATTRv patients is unclear. We aimed to characterize and compare the peripheral nervous system involvement of the Val142Ile (V142I, previously V122I) ATTRv mutation with other known ATTRv mutations. METHODS: A retrospective, cross-sectional study was carried out on patients w

Exploring autonomic regulation in children with ADHD with and without comorbid anxiety disorder through three systematic levels of cardiac vagal control analysis: Rest, reactivity, and recovery.

Autonomic nervous system (ANS) dysregulation, characterized by reduced vagally mediated Heart Rate Variability (HRV), has been associated with Attention-Deficit/ Hyperactivity Disorder (ADHD). This study investigated the dynamic vagal modulation of cardiac output in response to shifts in environmental demands in children and adolescents with ADHD, with and without a comorbid anxiety disorder. High-frequency HRV (HF-HRV) measures were obtained from 46 children and adolescents ranging from 6 to 17 (M = 9.38; SD = 2.31) years old, during three successive experimental conditions: a baseline record…
