Beneficial effects of postural intervention on prehensile action for an individual with ataxia resulting from brainstem stroke.

PRIMARY OBJECTIVE: This paper examined the effectiveness of postural training on upper extremity performance in an ataxic individual. The ataxia resulted from a brain stem stroke. RESEARCH DESIGN: Before-after, single-subject experimental design. EXPERIMENTAL INTERVENTION: Four-week course of postural training, comprised of three one-hour sessions/week. MAIN OUTCOMES AND RESULTS: The patient demonstrated an increase in function of the ataxic limb, as evidenced by appreciable increases in the Fugl-Meyer score and modest increases in the Postural Assessment Scale for Stroke Patients (PASS) sc…

[Sneezing. A review of its causation and pathophysiology].

AIMS: To review some of the lesser known aspects of sneezing, which is a universal phenomenon suffered the world over but which has received little attention in the medical literature and especially by that dealing with neurology. DEVELOPMENT: Here we consider the causation, neurophysiology, pathophysiology, diagnosis and treatment of this reflex action, which sometimes goes beyond being just a simple ailment of little importance and becomes a sign associated to a series of different medical conditions. CONCLUSIONS: Despite its being such a commonplace occurrence, little is known about sneez

Internuclear ophthalmoplegia: unusual causes in 114 of 410 patients.

Internuclear ophthalmoplegia (INO) is a sign of exquisite localizing value, often due to either multiple sclerosis or infarction. To demonstrate that unusual causes of INO are more common than the 11% reported in previous series, this review considers a case series of 410 inpatients whom I personally examined during a 33-year period. In this series, the cause of INO was infarction in 157 patients (38%), multiple sclerosis in 139 (34%), and unusual causes in 114 (28%). Unusual causes included trauma (20 cases), tentorial herniation (20 cases), infection (17 cases), tumor (17 cases), iatrogenic

Revised nomenclature for avian telencephalon and some related brainstem nuclei.

The standard nomenclature that has been used for many telencephalic and related brainstem structures in birds is based on flawed assumptions of homology to mammals. In particular, the outdated terminology implies that most of the avian telencephalon is a hypertrophied basal ganglia, when it is now clear that most of the avian telencephalon is neurochemically, hodologically, and functionally comparable to the mammalian neocortex, claustrum, and pallial amygdala (all of which derive from the pallial sector of the developing telencephalon). Recognizing that this promotes misunderstanding of the f

Labeling of the scrapie-associated prion protein in vitro and in vivo.

Prion diseases are a group of infectious neurodegenerative diseases that affect both animals and humans. A characteristic of prion diseases is the aggregation and accumulation of a disease-associated isoform of the prion protein in the brains of infected individuals. The amyloid imaging probe (trans,trans)-1-bromo-2,5-bis-(3-hydroxycarbonyl-4-hydroxy)styrylbenzene (BSB) has shown potential in the diagnosis of other amyloid disorders and we hypothesized that this compound would be effective in labeling prion protein plaques in vitro and in vivo. To test this, we compared BSB fluorescence to pri

Brainstem infarction presenting with neurogenic stuttering.

We reported a sixty-year-old man who developed acquired stuttering after a brainstem infarction. Infarctions were detected in the midbrain and upper pons. Neurogenic stuttering of this patients indicated that the midbrain and upper pons could be lesion sites responsible for acquired stuttering. We speculated that the reticular network extending from the brainstem to the frontal cortices, and the periaqueductal gray matter could be closely related regions generating neurogenic stuttering.

Novel brainstem syndrome associated with prostate carcinoma.

Two patients successfully treated for prostatic cancer developed a progressive neurologic syndrome beginning with loss of voluntary horizontal eye movements followed by severe, persistent muscle spasms of the face, jaw, and pharynx. Both had mild gait unsteadiness, and one exhibited facial and abdominal myoclonus. Extensive diagnostic studies, including MRIs of the brainstem (with and without contrast), were normal. CSF examination showed mild pleocytosis and elevated IgG. Quantitative eye movement recordings documented selective involvement of voluntary horizontal saccades with sparing of hor

Noradrenergic innervation of somatosensory thalamus and spinal cord.

Monoamine systems have been shown to be an important part of an endogenous analgesic system of the central nervous system. Some aspects of the anatomical basis of monoamine modulation of nociceptive input were investigated in these studies. Two sites examined where monoamine systems are known to impinge on the pain transmission system included the grey matter of the somatosensory thalamus and the spinal cord. In particular, the connections of noradrenergic systems with these regions were emphasized. In the ventral posterolateral nucleus of the thalamus the presence of a sparse innervation by b

[Afferent projections to the uvula in the cat. II. Mossy fiber projections].

The mossy fiber projections to the uvula of the cerebellum were studied by means of retrograde axonal transport of horseradish peroxidase (HRP) in the cat. Following large and small injections into the uvula, distribution of the labeled cells in the brainstem nuclei was investigated. The results showed different afferent projections between the dorsal and ventral uvula. Major sources projecting to the dorsal uvula were the peduncular, paramedian, and lateral nuclei of the pontine nuclei. Labeled cells found in the pontine nuclei amounted to 81.6% of the total number of labeled cells in cat 1.

[Oropharyngeal dysphagia in neuromuscular diseases--differential diagnosis, examination procedure and therapy].

After a synopsis of brain stem structures participating in swallowing, the main neurologic diseases are presented as a part of which swallowing disorders may occur: central and peripheral nervous system, motor end-plates and muscles can be involved. The sequence of clinical examination is described with special reference to findings in mouth, pharynx and larynx as well as to findings in high-frequency cinematography. After description of normal swallowing pre-, intra- and postdeglutitive mechanisms of aspiration are shown. This distinction has proved useful for practical reasons. Finally we gi…

Uncrossed retinal projection to the medial terminal nucleus of the accessory optic system in the hereditary unilaterally microphthalmic rat.

The uncrossed optic projection to the medial terminal nucleus of the accessory optic system was studied in adult rats with hereditary unilateral microphthalmia. The vestigial eye of the mutant completely lacks the optic nerve. After injection of HRP into the remaining eye the ipsilateral labeled fibers of the accessory optic system via the superior fasciculus distinctly covered the three terminal nuclei. No ipsilateral inferior fasciculus of the accessory optic system was labeled. Aberrant expansion of the uncrossed superior fasciculus fibers was observed in the medial terminal nucleus.

Experimental trigeminal glycerol injection. Electrophysiologic and morphologic effects.

The mechanism by which glycerol abolishes the pain of tic douloureux with minimal trigeminal deafferentation remains unknown. To study the action of glycerol, ten cats underwent unilateral retrogasserian injection of anhydrous glycerol. The contralateral (control) side was injected with saline. Glycerol injection increased the average latencies and reduced the average amplitudes of trigeminal brain-stem evoked potentials. Histopathologic examination disclosed focal demyelination, axonal swelling, endoneurial fibrosis, and neuronal loss. Evoked potentials were severely altered or abolished in c
