Interaction of mtROS-Immune-Inflammatory Vicious Cycle Activation in Sepsis-Induced Cardiomyopathy.

Mitochondrial reactive oxygen species (mtROS) serve as a central mediator in the pathogenesis of mitochondrial energy metabolism disorders and play a pivotal role in sepsis-induced cardiomyopathy (SICM). The production and regulation of mtROS involve multiple molecular mechanisms, including VDAC1-mediated mitochondrial impairment and the STK3/KEAP1/Nrf2 signalling pathway. Once generated, mtROS contribute to cellular damage through dual effects on the nitric oxide-lipid metabolic axis and complex interactions with ferroptosis. The pathological impact of mtROS is further amplified by mitochondr

Research progress on beta-blockers in the treatment of sepsis-induced cardiomyopathy: A mini review.

Sepsis-induced cardiomyopathy (SCM), a frequent complication of septic shock with mortality exceeding 40%, arises from catecholamine-driven cardiotoxicity, sympathetic hyperactivity, and inflammation-mediated biventricular dysfunction. Short-acting β₁-blockers (esmolol, landiolol) offer a targeted therapeutic approach by reducing heart rate (target: 80-95 bpm), myocardial oxygen demand, and proinflammatory cytokines while improving diastolic perfusion-leveraging ultra-short half-lives (t₁/₂ = 4-9 min) for rapid reversibility during instability. Clinical evidence remains divergent: a landmark s

Maternal Obesity and Risk of Hospital Readmission in Peripartum Cardiomyopathy: A Retrospective Cohort Study.

BACKGROUND: Peripartum cardiomyopathy (PPCM) is a major cause of maternal heart failure. Hospital readmission remains a common and serious complication. Maternal obesity is increasingly prevalent, but its association with readmission in PPCM has not been well defined. MATERIALS AND METHODS: We performed a retrospective multicenter cohort study using the TriNetX research network. Women with a first diagnosis of PPCM were identified using standard diagnostic codes, yielding 1,323 patients. "Maternal obesity" was defined as a body mass index >30 kg/m². The primary outcome was all-cause hospital

Noninvasive Programmed Stimulation Predicts Arrhythmia in Ischemic Cardiomyopathy With ICDs for Primary Prevention.

BACKGROUND: Implantable cardioverter-defibrillators (ICDs) are essential for preventing sudden cardiac death in patients with ischemic cardiomyopathy (ICM). Noninvasive programmed stimulation (NIPS) is a potential tool for predicting ventricle tachycardia (VT) or ventricle fibrillation (VF) and helps to select patients with higher arrhythmic risk. This study aimed to evaluate the predictive value of NIPS in identifying ICM patients at risk for VT/VF episodes and mortality after ICD implantation for primary prevention. METHODS: This prospective, single-center study included 50 patients with IC

Left Ventricular Non-Compaction Cardiomyopathy: The Tragedies & Trabeculations of the Architectural Cardiac Sponge.

Left-ventricular non-compaction (LVNC) is a recently classified cardiomyopathy that involves abnormal trabeculations inside the left ventricle, most commonly located in the ventricular apex. There are 9 distinct types of non-compaction cardiomyopathy that can impact both the left and right ventricles with subtypes involving mostly pediatric patients with concurrent congenital heart disease (CHD), to individuals in late adult-staged ages. LVNC affects the population with an estimated range of incidence from 0.014% to 1.3% and the disease can be diagnosed with the utilization of imaging studies

Red Flags for Differentiating Desmosomal "Hot-Phase" Cardiomyopathy From Acute Myocarditis.

BACKGROUND: Desmosomal "hot-phase" cardiomyopathy (HPC), characterized by bursts of myocardial inflammation mimicking acute myocarditis (AM), carries relevant risks of adverse outcomes. This study aimed to identify diagnostic "red flags" favoring HPC over AM. METHODS: Patients (n=134) receiving a first diagnosis of AM, proven by endomyocardial biopsy or cardiac magnetic resonance plus troponin elevation, were retrospectively identified at a referral center. HPC was defined by presence of pathogenic desmosomal gene variants (DGVs). Clinical, imaging, and electrical features were compared betwe

RYR1 Pathogenic Variant in a Patient With Overlapping Features of Brugada Syndrome and Arrhythmogenic Cardiomyopathy.

BACKGROUND: Brugada syndrome (BrS) and arrhythmogenic cardiomyopathy (ACM) may overlap clinically, but shared mechanisms remain unclear. RYR1 mutations have not been previously linked to these arrhythmia syndromes. CASE SUMMARY: A 32-year-old man with a family history of BrS presented with palpitations and positional ventricular tachycardia. Ajmaline testing unmasked a diagnostic type 1 BrS electrocardiographic pattern. Cardiac magnetic resonance showed subtle right ventricular dyskinesia and focal left ventricular fibrosis, meeting borderline ACM criteria. Electrophysiological study showed n

Beyond the Pump: Unravelling Immunometabolic Crosstalk and Organelle Dynamics in Sepsis-Induced Cardiomyopathy.

Sepsis-induced cardiomyopathy (SICM) has traditionally been viewed as pump-centered contractile failure, but this paradigm fails to explain the clinical spectrum and recovery patterns. This review presents an integrative framework where immunometabolic crosstalk and organelle dysfunction drive disease, including metabolic routing defects, mitochondrial fission, ER stress, and epigenetic regulation via m6A modification and lactylation. Clinically, it proposes a four-phenotype taxonomy (hyperdynamic, hypodynamic, right ventricular-predominant, Takotsubo-like) and advocates for strain imaging and

Exercise Training in Hypertrophic Cardiomyopathy: A Systematic Review, Meta-analysis, and Meta-regression.

BACKGROUND: A paradigm shift has emerged regarding exercise participation in patients with hypertrophic cardiomyopathy (HCM), with recent studies suggesting that structured training may improve functional capacity and cardiopulmonary responses. However, the effects of structured exercise interventions in HCM have not been comprehensively synthesized. AIM: The main objective was to determine the effect of exercise training on bodyweight, functional capacity, echocardiography, blood pressure, heart rate (HR), double product, and NT-proBNP in HCM patients. METHODS: Randomized controlled trials

A Case Report on Opioid Withdrawal-Induced Stress Cardiomyopathy and a Review of the Current Literature.

Takotsubo cardiomyopathy (TTC), also known as stress-induced cardiomyopathy or apical ballooning syndrome, is a temporary heart condition characterized by a sudden and reversible weakening of the heart muscle, often triggered by significant emotional or physical stress. First identified in Japan in 1990, the term "Takotsubo" describes the heart's distinctive shape during this condition, resembling an octopus trap as seen in imaging studies. Opioid withdrawal-induced stress cardiomyopathy is likely mediated by increased sympathetic activity and catecholamine surge. It may present with acute che

When the Flu Breaks the Heart: A Case Report of Influenza A-Induced Takotsubo Cardiomyopathy.

We present a case of Takotsubo cardiomyopathy (TCM), commonly known as broken heart syndrome, triggered by influenza A infection. Known cardiac complications of influenza include myocardial infarction, myocarditis, and pericarditis. Only a few cases of influenza have so far been reported that led to TCM. We discuss the presentation, investigations, and management of a 67-year-old woman who presented to the emergency department (ED) with chest pain and shortness of breath two weeks after being diagnosed with influenza A. We aim to highlight a rare cardiac complication of influenza infection so

Reverse Takotsubo Cardiomyopathy Mimicking ST-Elevation Myocardial Infarction in a Postpartum Woman.

Takotsubo cardiomyopathy, also known as Gebrochenes-Herz syndrome, transient apical ballooning syndrome, apical ballooning cardiomyopathy, stress-induced cardiomyopathy, or broken heart syndrome, is a rare disease triggered by stressful events, such as a high plasma concentration of catecholamines and their metabolites, which resembles acute coronary syndrome. It is characterized by transient regional systolic dysfunction of the left ventricle mimicking acute myocardial infarction. The term Takotsubo originates from the Japanese word for an "octopus trap," which resembles the shape of the vent

Vasospasm-Induced Takotsubo Cardiomyopathy: An Underrecognized Phenotype of Ischemia With Nonobstructive Coronary Arteries.

Coronary vasospasm is a mechanistically distinct precipitant of takotsubo cardiomyopathy (TCM) that may produce transient left ventricular (LV) dysfunction through ischemia-induced myocardial stunning rather than adrenergic excess. Vasospasm-induced TCM remains underrecognized, and this case illustrates its distinct therapeutic implications, particularly in patients with ischemia with nonobstructive coronary arteries (INOCA). A 47-year-old perimenopausal woman with chronic tobacco use, hypertension, and a prior coronary angiogram performed approximately two years prior during a non-ST-elevatio

Potential Contribution of DES (p.Leu88Met) and MYH7 (p.Arg787His) Variants to Familial Restrictive Cardiomyopathy.

BACKGROUND: Restrictive cardiomyopathy (RCM) is a rare, severe cardiac disease with a heterogeneous genetic basis. Both genetic and nongenetic factors contribute to RCM pathogenesis. Identifying the underlying molecular causes is important for diagnosis and family screening. In this study, we investigated the genetic basis of RCM in a 52-year-old woman with a family history of RCM and heart disease. METHODS: Genetic predisposition was evaluated using whole-exome sequencing (WES). Candidate variants identified in the proband were validated by Sanger sequencing and interpreted using bioinformat

Fatal Cardiomyopathy Secondary to Seronegative Immune-Mediated Necrotizing Myopathy: A Case Report.

BACKGROUND Idiopathic inflammatory myopathies (IIM) are disorders of autoimmune inflammation of muscle tissue. IIM subtypes include polymyositis, dermatomyositis, inclusion body myositis, and immune-mediated necrotizing myopathy (IMNM); which can be further subdivided based on serology. These disorders primarily affect skeletal muscle, and cardiac involvement is uncommon. We present a case of a patient with progressive cardiomyopathy as a rare complication of seronegative IMNM. CASE REPORT A previously healthy 35-year-old man presented with proximal muscle weakness and myalgias. Initial workup
