Current insulin therapy fails to fully restore physiological glucose homeostasis in type 1 diabetes mellitus, with 75% of patients unable to achieve the desired management targets. While stem cell-derived islets offer promising therapy, they require an enhanced extracellular matrix support for optimal transplantation outcomes. To address this challenge, we developed biofunctional endocrine micro-pancreata using decellularized porcine lung scaffolds seeded with embryonic stem cell-derived islets. In vivo efficacy was evaluated following subcutaneous or intraperitoneal transplantation into NOD-S

INTRODUCTION: The impact of race on outcomes of allogeneic hematopoietic cell transplants (HCT) has long been a field of research. The Center for International Blood and Marrow Transplant Research (CIBMTR) studies have shown worse survival for Black and Hispanic patients within the first year after HCT, but rates evened out for one-year survivors. From our personal experience, we hypothesize that the outcomes of South Asians (age ≥ 45 years) receiving myeloablative conditioning (MAC) are also worse compared to other races.
METHODS: This is a retrospective single-centre study. All patients (ag

BACKGROUND: Cytomegalovirus (CMV) infection remains a major cause of morbidity, mortality, and increased healthcare burden in recipients of allogeneic hematopoietic stem cell transplantation (allo-HSCT). Its clinical manifestations range from asymptomatic CMV replication to end-organ diseases such as pneumonia, gastroenteritis, and retinitis, all of which are associated with a higher rate of nonrelapse mortality.
METHODS: We reviewed case records of children who underwent allo-HSCT at our center between April 2013 and October 2024. CMV monitoring was performed weekly until at least day +100 p…

Immune rejection poses a major challenge in organ transplantation, with tissue-resident memory T (TRM) cells playing a critical role in graft rejection. This study investigated the impact of CCR8 on TRM cells using single-cell RNA sequencing (scRNA-seq), flow cytometry, and immunohistochemistry. The results show that CCR8 expression was upregulated on CD8⁺ TRM cells after transplantation, peaking on day 7. Blocking or knocking out CCR8, as well as neutralizing CCL1 and CCL8, significantly reduced CD8⁺ TRM cell accumulation in the graft and their cytokine production. These treatments prolonged

Hematopoietic stem cell transplantation (HSCT) is a life-saving therapy for hematologic malignancies, such as leukemia and lymphoma and other severe conditions but is associated with significant risks, including graft versus host disease (GVHD), relapse, and treatment-related mortality. The increasing complexity of clinical, genomic, and biomarker data has spurred interest in machine learning (ML), which has emerged as a transformative tool to enhance decision-making and optimize outcomes in HSCT. This review examines the applications of ML in HSCT, focusing on donor selection, conditioning re…

Hematopoietic cell transplantation requires higher doses of chemotherapy, and practices of adjusting the weight because of concerns of organ toxicity are common. This retrospective analysis of 239 adult recipients of autologous hematopoietic cell transplantation for lymphoma assessed the effect of obesity on transplantation outcomes.BackgroundPrior data evaluating the impact of obesity in autologous hematopoietic cell transplantation (AHCT) for lymphomas have provided differing results when assessing overall (OS) and progression-free survival (PFS). Impact on survival outcomes have been descri…

We present the largest prospective real-world experience in 159 patients who received the triple combination of tacrolimus/sirolimus/mycophenolate mofetil after reduced intensity conditioning allogeneic hematopoietic stem cell transplantation (RIC-alloHSCT) from matched-related (MRD), matched-unrelated (MUD) or mismatched-unrelated donors (MMURD). Despite the high-risk and elderly population, non-relapse mortality (NRM) at day +100 and 1 year was 5.1% and 8.6%. Grades 2-4 and 3-4 acute Graft-versus-host disease (GvHD) at day +180 was 30.3% and 13%, respectively. Chronic GvHD at 1 and 3 years w…

Bloodstream infections (BSI) pose critical risks during allogeneic hematopoietic stem cell transplantation (allo-HSCT), yet data on BSI during the conditioning phase remain limited. To assess the characteristics, risk factors, and outcomes of BSI during conditioning in allo-HSCT patients with hematological diseases. We conducted a single-center retrospective cohort study including 2395 consecutive allo-HSCT recipients between September 2013 and September 2023. BSI occurred in 113 patients (5.7% cumulative incidence), with median onset on day -2. Monomicrobial Gram-negative bacteria (73.5%, 83/…

BACKGROUND: Cytokine release syndrome (CRS) is a frequent early complication of T cell-replete peripheral blood haploidentical hematopoietic stem cell transplantation (PB-haploHSCT), particularly in pediatric recipients. Post-transplant cyclophosphamide (PTCy) is the standard for graft-versus-host disease (GVHD) prophylaxis, but the role of early cyclosporine initiation in this setting remains underexplored.
METHODS: We retrospectively analyzed 50 pediatric patients with hematolymphoid malignancies who underwent PB-haploHSCT with uniform myeloablative conditioning between 2017 and 2024. GVHD …

Allogeneic hematopoietic stem cell transplantation (allo-HSCT) frequently involves patients with multiple co-morbidities. The drugs prescribed for these conditions may lead to complications during the transplant procedure, either through their mechanism of action, their adverse reaction profile or the risk of pharmacological interactions with the drugs prescribed for the allo-HSCT procedure and its complications. Therefore, each patient's drug-related risks must be assessed prior to allo-HSCT. Various therapeutic decisions may be considered to limit the occurrence of complications. The aim of …

BACKGROUND: Late-onset severe pneumonia (LOSP) is one of the most important causes of mortality after allogeneic hematopoietic stem cell transplantation (allo-HSCT). The influenza virus is a frequent viral pathogen in patients receiving allo-HSCT, accounting for at least 30% of all respiratory viral infections. We aimed to identify the clinical characteristics and outcomes of influenza-associated LOSP after allo-HSCT.
METHODS: This study enrolled 23 patients who underwent allo-HSCT at the Peking University Institute of Hematology and were subsequently diagnosed with influenza-associated LOSP.…

BACKGROUND: Giant cell myocarditis (GCM) and multisystem inflammatory syndrome in adults (MIS-A) with myocarditis that appears after SARS-CoV-2 infection (COVID-19) may manifest similarly.
CASE SUMMARY: We describe a patient with GCM who presented with cardiogenic shock and received a misdiagnosis of MIS-A myocarditis because of a lack of giant cells identified on endomyocardial biopsy (EMB). Her cardiac function improved with combined immunosuppressive therapy, including cyclosporine, but subsequently worsened, with premature cessation of immunosuppression resulting from the misdiagnosis. BACKGROUND: Given the lack of specificity of current blood and urine testing and the resistance/inability to perform pancreas allograft biopsies, additional noninvasive investigational tools to assess the risk for rejection are needed. This study examines the clinical impact of molecular HLA matching in a large single-center simultaneous pancreas-kidney (SPK) transplant program.
METHODS: The study cohort comprised 238 SPK recipients between 2012 and 2021. The number of HLA mismatches, eplet, Snow (that counts the number of protein-specific surface-accessible donor HLA amino acid mismatches),

Access to allogeneic hematopoietic stem cell transplantation (allo-HSCT) for acute lymphoblastic leukemia (ALL) remains limited in low- and middle-income countries (LMICs). Haploidentical stem cell transplantation (haplo-SCT) with post-transplant cyclophosphamide (PTCy) has expanded donor availability, but data from Latin America are scarce. To evaluate clinical outcomes of haplo-SCT in pediatric and adult ALL patients in a single center in Mexico using a standardized protocol.
