Paediatric reference values for total psoas muscle area.

BACKGROUND: Sarcopenia, the unintentional loss of skeletal muscle mass, is associated with poor outcomes in adult patient populations. In adults, sarcopenia is often ascertained by cross-sectional imaging of the psoas muscle area (PMA). Although children with chronic medical illnesses may be at increased risk for muscle loss because of nutritional deficiencies, physical deconditioning, endocrine anomalies, and systemic inflammation, consistent quantitative definitions for sarcopenia in children are lacking. We aimed to generate paediatric reference values for PMA at two intervertebral lumbar l

Severe post-renal acute kidney injury, post-obstructive diuresis and renal recovery.

UNLABELLED: Study Type--Therapy (case series) Level of Evidence 4. What's known on the subject? and What does the study add? The pathophysiology of post-renal acute kidney injury (PR-AKI), i.e. caused by urinary tract obstruction, has been extensively studied in animal models but clinical studies on this subject are outdated, and/or have focused on the mechanisms of 'post-obstructive diuresis' (POD), a potentially life-threatening polyuria that can develop after the release of obstruction. In severe PR-AKI, the risk of occurrence of POD is high. POD occurrence predicts renal recovery without t

Informatics-Based Psychotherapeutic and Psychiatric Interventions in Dermatology: Scoping Review of Impacts on Skin Disease Severity and Mental Health Outcomes.

BACKGROUND: Chronic dermatologic conditions such as psoriasis, atopic dermatitis, and hidradenitis suppurativa are associated with a high burden of psychiatric comorbidities, including depression, anxiety, and suicidality. Despite growing awareness of the psychosocial impact of skin diseases, mental health needs remain underaddressed in dermatologic care. Digital technologies (including teledermatology, mobile health apps, and internet-delivered psychotherapies) offer promising avenues for integrating psychotherapeutic and psychiatric interventions into dermatology. However, the scope, effecti

Clinical characteristics and CKD care delivery in African American and American Indian or Alaska Native patients: A real-world cohort study.

BACKGROUND: Racially minoritized populations in the United States (US), notably African American (AA) and American Indian/Alaska Native (AI/AN), experience disproportionately higher rates of chronic kidney disease (CKD), diabetes, and hypertension compared to their White peers but are understudied. This real-world cohort study examines the standards of CKD care provided to these groups in two US health systems. METHODS: Using electronic health record data from the Center for Kidney Disease Research, Education, and Hope (CURE-CKD) Registry (N = 381,011) collected between 2015 and 2020, adjuste

Perinatal outcomes in twin pregnancies complicated by gestational diabetes.

BACKGROUND: Gestational diabetes in singleton pregnancies increases the risk for large for gestational age infants, hypertensive disorders of pregnancy, and neonatal morbidity. Compared with singleton gestations, twin gestations are at increased risk for fetal growth abnormalities, hypertensive disorders, and neonatal morbidity. Whether gestational diabetes further increases the risk for these outcomes is unclear. OBJECTIVE: We sought to determine the relationship between gestational diabetes and the risk for preeclampsia, fetal growth abnormalities, and neonatal intensive care unit admission

Sarcopenia is the independent predictor of mortality in critically ill patients with cirrhosis.

BACKGROUND: Sarcopenia is strongly associated with poor outcome in cirrhosis. There are little prospective data that sarcopenia influences outcomes in critically ill cirrhotics (CICs). Computed tomography (CT) is the gold standard for sarcopenia assessment in the intensive care unit (ICU), as it is independent of hydration status. AIM: This study aims to assess the prevalence of sarcopenia and study its impact on clinical outcomes in CICs. METHODS: In this prospective observational study, CICs admitted to the liver ICU were enrolled, if meeting inclusion (age 18-70 years, abdominal CT scan w

Risk of developing an abdominal aortic aneurysm after ectatic aorta detection from initial screening.

OBJECTIVE: Current abdominal aortic aneurysm (AAA) surveillance guidelines lack any follow-up recommendations after initial abdominal aortic screening diameter of less than 3.0 cm. Some reports have demonstrated patients with late AAA formation and late ruptures after initial ultrasound screening detection of patients with an aortic diameter of 2.5 to 2.9 cm (ectatic aorta). The purpose of this study was to determine ectatic aorta prevalence, AAA development, rupture risk, and risk factor profile in patients with detected ectatic aortas in a AAA screening program. METHODS: A retrospective cha

Risk factors for chronic ulceration in patients with varicose veins: a case control study.

BACKGROUND/OBJECTIVE: Identifying which patients with varicose veins are at risk of progressing to more severe forms of chronic venous disease could help in assigning clinical priorities and targeting appropriate treatments. The aim of this study was to determine, in subjects with varicose veins, the characteristics of venous disease and other factors associated with an increased risk of ulceration. METHODS: One hundred twenty subjects with varicose veins and an open or healed venous leg ulcer were compared with 120 controls with varicose veins and no history of venous ulcer on this case cont

Developing and Testing a Brief Mindfulness Just-in-Time Adaptive Intervention to Reduce Stress Among Caregivers of People With Dementia: Quasi-Experimental Study.

BACKGROUND: Dementia caregiving entails chronic, fluctuating stress with downstream risks to caregivers' mental health and quality of care. Mindfulness-based interventions can reduce caregiver stress; however, moment-to-moment fluctuations in stress may limit receptivity to practice at any given time. We developed a brief mindfulness just-in-time adaptive intervention (JITAI) that aims to deliver support at the right moment by using machine learning algorithms to optimize notification timing based on receptivity to engage in brief mindfulness practices. OBJECTIVE: This study aims to evaluate

Machine Learning-Based Prediction of Ultrasound-Detected Hepatic Steatosis Within the Metabolic Dysfunction-Associated Steatotic Liver Disease Spectrum Using Routine Clinical and Biochemical Parameters.

Background/Objectives: Metabolic dysfunction-associated steatotic liver disease (MASLD) is now the leading cause of chronic liver disease globally, mirroring the increasing prevalence of obesity, insulin resistance, and type 2 diabetes. Early detection of hepatic steatosis is vital for cardiometabolic risk assessment; however, conventional imaging is costly and impractical for population screening. This study aimed to develop interpretable machine-learning models to predict ultrasound-detected hepatic steatosis within the MASLD spectrum using routinely available clinical and biochemical data.

Incidence of and Factors Associated With Prolonged and Persistent Postoperative Opioid Use in Children 0-18 Years of Age.

BACKGROUND: Long-term opioid use has negative health care consequences. Opioid-naïve adults are at risk for prolonged and persistent opioid use after surgery. While these outcomes have been examined in some adolescent and teenage populations, little is known about the risk of prolonged and persistent postoperative opioid use after common surgeries compared to children who do not undergo surgery and factors associated with these issues among pediatric surgical patients of all ages. METHODS: Using a national administrative claims database, we identified 175,878 surgical visits by opioid-naïve c

Sex Differences in Veterans Admitted to the Hospital for Chronic Obstructive Pulmonary Disease Exacerbation.

Rationale: As chronic obstructive pulmonary disease (COPD) prevalence in women has outpaced that in men, COPD-related hospitalization and mortality are now higher in women. Presentation, evaluation, and treatment of COPD differ between women and men. Despite higher smoking rates in Veterans, little work has characterized differences in Veterans with COPD by sex. Objectives: To determine risk factors for 30-day readmission among Veterans hospitalized for COPD exacerbations and how they differed by sex. Methods: We performed a retrospective observational analysis of Veterans receiving primary ca

Asthma-COPD overlap syndrome in the US: a prospective population-based analysis of patient-reported outcomes and health care utilization.

BACKGROUND: Prior work suggests that asthma-COPD overlap syndrome (ACOS) has a greater health burden than asthma alone or COPD alone. In the current study, we have further evaluated the health burden of ACOS in a nationally representative sample of the US population, focusing on patient-reported outcomes and health care utilization and on comparisons with asthma alone and COPD alone. Patient-reported outcomes are especially meaningful, as these include functional activities that are highly valued by patients and are the basis for patient-centered care. METHODS: Using data from the Medical Exp

Comorbidity patterns and inpatient expenditure among patients with coronary heart disease in China.

BACKGROUND: Coronary heart disease (CHD) often co-occurs with other chronic conditions. This increases care complexity and hospital costs. Most previous studies have relied on single diseases or simple comorbidity counts. Less is known about whether different multimorbidity patterns are associated with different levels and structures of inpatient expenditure. We aimed to identify comorbidity patterns among middle-aged and older inpatients with CHD and to examine their associations with hospital costs. METHODS: We conducted a retrospective study using electronic health record data from a terti

Sexual dysfunction, depressive symptoms, and perceived impact of HBV infectivity among noncirrhotic patients with chronic hepatitis B: a cross-sectional study.

BACKGROUND: Chronic hepatitis B (CHB) remains a major global public health burden, yet sexual health is rarely addressed in routine care. This study aimed to assess the prevalence of sexual dysfunction (SD) and examine demographic, clinical, and psychological factors associated with sexual function among noncirrhotic patients with CHB. We also explored the association between sexual function and the perceived negative impact of HBV infectivity on sexual life. METHODS: This cross-sectional study was conducted from December 2023 to July 2024, enrolling 657 patients with CHB and 121 non-HBV cont

Changing Epidemiological Patterns of Epilepsy in China, 1990-2023, with Projections to 2050.

INTRODUCTION: Epilepsy remains a major source of chronic neurological morbidity and long-term care need. In China, its contemporary burden is likely being reshaped by population ageing and by shifts in the balance between idiopathic and secondary epilepsy, yet updated national evidence integrating subtype-specific burden, demographic decomposition, and future projections remains limited. METHODS: We performed a secondary analysis of the Global Burden of Disease Study 2023 to assess epilepsy burden in China from 1990 to 2023, using global estimates for contextual comparison. We examined preval

Early laboratory trajectories and short-term tolerability after finerenone initiationin diabetic kidney disease: the multicenter FINE-TURK study.

BACKGROUND: Real-world evidence describing the early laboratory course after finerenone initiation in contemporary nephrology practice remains limited, particularly in cohorts with high background use of sodium-glucose co transporter 2 inhibitors (SGLT2i). We evaluated early kidney-function, potassium, and albuminuria trajectories and short-term treatment tolerability. METHODS: FINE-TURK was a multicenter retrospective cohort study without a comparator group. Adults with diabetes and chronic kidney disease who initiated finerenone in routine care were evaluated at baseline and using the first

Translating Patient-Professional Divergence into Care-Improvement Priorities in Pelvic Floor Rehabilitation: A Delphi-Dialogue Study.

Background/Objectives: Pelvic floor dysfunctions (PFDs) are highly prevalent chronic conditions associated with substantial functional and psychosocial burden. Although multidisciplinary pelvic floor rehabilitation is recommended as first-line management, its effectiveness depends not only on intervention efficacy but also on care delivery, communication, patient engagement, and long-term adherence. This study applied the Delphi-Dialogue Patients-Professionals (DDPP) framework to quantify patient-professional alignment and translate identified patient-professional perceptual differences into c

Short-term outcomes of asphyxiated neonates depending on requirement for transfer in the first 24 h of life.

IMPORTANCE: In neonates with birth asphyxia (BA) and hypoxic-ischemic encephalopathy, therapeutic hypothermia (TH), initiated within six hours, is the only safe and established neuroprotective measure to prevent secondary brain injury. Infants born outside of TH centers have delayed access to cooling. OBJECTIVE: To compare in-hospital mortality, occurrence of seizures, and functional status at discharge in newborns with BA depending on postnatal transfer for treatment to another hospital within 24 h of admission (transferred (TN) versus non-transferred neonates (NTN)).
