Mitochondrial dysfunction in neurodegenerative diseases associated with copper imbalance.

Copper is an essential transition metal ion for the function of key metabolic enzymes, but its uncontrolled redox reactivity is source of reactive oxygen species. Therefore a network of transporters strictly controls the trafficking of copper in living systems. Deficit, excess, or aberrant coordination of copper are conditions that may be detrimental, especially for neuronal cells, which are particularly sensitive to oxidative stress. Indeed, the genetic disturbances of copper homeostasis, Menkes' and Wilson's diseases, are associated with neurodegeneration. Furthermore, copper interacts with

Factors and determinants of disease emergence.

Emerging infectious diseases can be defined as infections that have newly appeared in a population or are rapidly increasing in incidence or geographic range. Many of these diseases are zoonoses, including such recent examples as avian influenza, severe acute respiratory syndrome, haemolytic uraemic syndrome (a food-borne infection caused by certain strains of Escherichia coli) and probably human immunodeficiency virus/acquired immune deficiency syndrome. Specific factors precipitating the emergence of a disease can often be identified. These include ecological, environmental or demographic fa

Growing significance of myeloperoxidase in non-infectious diseases.

Myeloperoxidase (MPO) is a glycoprotein released by activated polymorphonuclear neutrophils, which takes part in the defense of the organism through production of hypochlorous acid (HOCl), a potent oxidant. Since the discovery of MPO deficiency, initially regarded as rare and restricted to patients suffering from severe infections, MPO has attracted clinical attention. The development of new technologies allowing screening for this defect has permitted new advances in the comprehension of underlying mechanisms. Apart from its implications for host defense, the expression of MPO restricted to m

Inherited disorders of fatty alcohol metabolism.

The importance of long-chain aliphatic alcohols to human biology has recently been emphasized by the discovery of several inborn errors of fatty alcohol metabolism. These inherited diseases include isolated defects in the oxidation of fatty alcohol to fatty acid (Sjögren-Larsson syndrome) and deficient incorporation of fatty alcohol into ether lipids (isolated alkyl dihydroxyacetone phosphate synthase deficiency). In addition, disorders of peroxisomal biogenesis (Zellweger syndrome, neonatal adrenoleukodystrophy, infantile Refsum disease) and peroxisomal protein import (rhizomelic chondrodyspl

[Vaccinations and systemic diseases. Vaccinations and immunosuppression].

The hypothesis of systemic disease flare up or onset triggered by vaccination cannot be confirmed by the analysis of data from the literature. In immune deficiency of any cause, immunization is of great interest to avoid preventable infectious diseases but the risk of adverse events and suboptimal immunologic responses has to be considered. For live, attenuated vaccines the main risks are postimmunization complications, specially in persons severely immunocompromised. For killed or inactivated vaccines there are no contraindications, but the immune response is reduced leading to consider highe

A deficiency in respiratory complex I in heart mitochondria from vitamin A-deficient rats is counteracted by an increase in coenzyme Q.

Defects of NADH:coenzyme Q oxidoreductase (complex I) of mitochondria have been described in many congenital and acquired diseases. Administration of coenzyme Q (CoQ, ubiquinone) has been shown to benefit patients with some of these diseases. However, the mechanisms by which CoQ exerts the therapeutic effects are not clearly understood. A reason could be the lack of saturation of CoQ, in kinetic terms, for complex I activity. However, this hypothesis has not been proved in vivo because of the difficulty to incorporate CoQ into the mitochondrial membranes. We have found a deficiency in respirat

Healthy eating in infancy.

Dietary prescriptions meant to reduce the risk of adult diseases could retard growth and development in infants and young children. Parents who pay attention to their infant's hunger or satiety signals will avoid under-feeding or over-feeding their infant. A useful guiding principle is that, as long as overall growth compares favourably with the standards for age, it is likely that all is well. Nutrition priorities during infancy should reflect the promotion of growth and development rather than the prevention of diseases later in life. For this reason, fat restriction is not appropriate under

On the short arm of the 17th chromosome is a peripheral myelin protein (PMP22) the duplication or point mutation of which causes the development of some congenital autosomal dominant hereditary demyelinization neuropathies: the most frequent variants of Charcot-Marie-Tooth disease (CMT1A), some cases of Déjérine-Sottas disease and microdeletion of PMP22 and hereditary pressure neuropathies. The pericentric section of the long arm of chromosome 17 comprises a locus conditioning the development of the most frequent phacomatosis-neurofibromatosis 1. As to rarer neuromuscular diseases, genome muta

Extrahepatic manifestations of hepatitis C.

Shortly after the discovery of the hepatitis C virus, it was realized that this infectious agent caused more than just liver disease. A remarkable array of extrahepatic manifestations of hepatitis C has now been described. Many of these associated syndromes implicate the hepatitis C virus as a mediator of autoimmunity or of immune complex formation. These disorders include mixed essential cryoglobulinemia, autoimmune hepatitis, glomerulonephritis, thyroiditis, and possibly Sjogren's syndrome. The hepatitis C virus has also been strongly linked to two skin disorders: prophyria cutanea tarda and

Agriculture-related anaemias.

Man evolved as a hunter-gatherer, and the invention and spread of agriculture was followed by changes in diet, the environment and population densities which have resulted in globally high prevalences of anaemias due to nutritional deficiencies of iron, folate and (locally) vitamin B12, to infestations by hookworm and schistosomes, to malaria, and to the natural selection for the genes for sickle-cell diseases, beta-thalassaemias, alpha-thalassaemias, glucose-6-phosphate dehydrogenase deficiency, ovalocytosis and possibly (locally) elliptocytosis. The present explosion of population is driving

Raynaud's phenomenon.

Raynaud's phenomenon is episodic digital ischemia provoked by cold and emotion; it is associated with other disorders, such as Raynaud's syndrome, especially the connective tissue diseases. Recent information suggests links with Sjögren's syndrome, malignancy, and obstructive vascular disease. The vasospasm can affect areas outside the periphery such as the inner ear in vibration white finger syndrome and the placental vessels in pregnancy. The initial presence of autoantibodies increases the chance by 63-fold of developing connective tissue diseases, and such immunologic testing can be useful

Stress and auto-immune endocrine diseases.

Auto-immunity may occur in all endocrine tissues, with a particular prevalence in thyroid and pancreatic islets. The most demonstrative clinical expressions of this auto-immunity are Graves' disease and insulin-dependent diabetes. In the former, extensive data are available upon the immunological disorders seen in peripheral blood as well as in the thyroid itself. The predisposal profile of such diseases is quite well delineated (genetic markers in HLA system, organ-specific T suppressive deficiency). On the other hand, the mechanisms implicated in the onset of auto-immunity are still question

Neuromuscular diseases of AIDS.

Neuromuscular diseases are common in acquired immune deficiency syndrome (AIDS). Although the clinical incidence of peripheral neuropathy has not been systematically studied, various reports suggest that up to 40% of AIDS patients have clinical symptoms. Biopsy and autopsy studies have shown an inflammatory neuropathy with a variable component of demyelination and axonal loss. Evidence of direct involvement by the human immunodeficiency virus (HIV) is scant. Immunosuppression followed by cytomegalovirus (CMV) infection appears to be a direct cause of polyradiculoneuropathy and perhaps other fo

Red cell autoantibodies in patients with acquired immune deficiency syndrome.

Mild-to-profound anemia, thrombocytopenia, and rarely neutropenia have been observed in patients with the acquired immune deficiency syndrome (AIDS). To investigate a possible immune mechanism, blood samples from 28 hospitalized AIDS patients, four asymptomatic homosexual men, four homosexual men with the AIDS-related lymphadenopathy syndrome, 30 hospitalized patients with diseases other than AIDS, and 60 blood donors were tested for the presence of atypical red cell antibodies. Eighteen AIDS patients (64%) had anti-i, nine (32%) had autoanti-U, and 12 (43%) had a positive direct antiglobulin

Neuromuscular diseases as models of cardiovascular regulation during exercise.

This article reviews the research performed to date on the cardiovascular responses to exercise in patients with neuromuscular diseases and lesions affecting the transmission of afferent impulses from skeletal muscle. These studies have provided important information about the roles of central command and reflexes from skeletal muscle afferents in circulatory control. Few animal models of neuromuscular diseases are available. Studies of patients with specific defects in skeletal muscle energy metabolism are particularly valuable because the local metabolic state participates in both systemic a

[Relevance of biochemistry in diagnosis and development of alcoholic liver disease (author's transl)].

In a group of 205 patients with alcoholic diseases of liver the diagnostic relevance of biochemical tests (GOT, GPT, AP, GGTP, BSP) was reconsidered with discriminatory process (separation of diagnosis). The group contained 16 patients with nutritional-caused and 41 cases with alcoholic-caused fatty-infiltration of liver. 148 patients showed a toxic chronic liver disease; 52 a chronic hepatitis and 96 cirrhosis of liver. Laparoscopy and morphology guaranteed the clinical diagnosis and therefore the accuracy of biochemistry in separation of diagnosis was given. The biochemical tests were not ab
