Increased IL-18 production by dendritic cells in active inflammatory myopathies.

Inflammatory myopathies (IM) are chronic disorders characterized by muscular accumulation of inflammatory cells that promote cytotoxicity and tissue damage. Overexpression of chemokines and cytokines as well as imbalance of dendritic cells (DC) homeostasis have been postulated to exert a role in both dermatomyositis (DM) and polymyositis (PM). We studied the T helper (Th)-1 and Th2 cytokine levels by enzyme linked immunosorbent assay (ELISA) and the muscular expression of IL-18 and its receptor by both histochemistry (HIC) and in situ hybridization (ISH) in both patients and normal controls. A…

Autoantibodies in idiopathic inflammatory myopathy: an update on clinical and pathophysiological significance.

PURPOSE OF REVIEW: Idiopathic inflammatory myopathy is characterized by the production of autoantibodies to various cellular constituents. These autoantibodies closely correlate with certain clinical conditions and prognosis of disease. This review examines recent progress in myositis-specific autoantibodies, particularly in their clinical significance and pathophysiological roles. RECENT FINDINGS: During the 1-year review period, novel myositis-specific autoantibodies were identified in clinically amyopathic dermatomyositis (anti-CADM-140 antibody) and malignancy-associated myositis (anti-p1…

Intravenous immunoglobulin preparations and autoimmune disorders: mechanisms of action.

Intravenous immunoglobulins (IVIg) have been used as a substitutive treatment for primary and secondary humoral immune deficiencies for several decades. In the meantime, increased experience has been acquired with IVIg in the management of other inflammatory and autoimmune disorders, such as Kawasaki's disease, idiopathic thrombocytopenic purpura, dermatomyositis or Guillain-Barré syndrome, in which several clinical trials have demonstrated its efficacy. In other pathologies, IVIg seem to be effective, although further studies are required. Nevertheless, the exact mechanism by which IVIg exert…

Four patients fulfilled the Bohan and Peter criteria of polymyositis and dermatomyositis. In the other cases paravertebral muscular biopsies led to the diagnosis of a congenital myopathy, a mitochondrial myopathy and an amyloid myopathy. Anticentromere antibody as a risk factor for cancer in patients with systemic sclerosis.

This study has estimated the cancer risk among patients with systemic sclerosis (SSc) using a population-based analysis. Using the inpatient and outpatient registries for patients at Kyushu University Hospital between 1982 and 1996, standardised incidence rates (SIRs) (ratio of observed-to-expected cancers) were calculated in 43 patients with SSc, 24 patients with polymyositis (PM) and 17 patients with dermatomyositis (DM). Risk factors predisposing to cancers were also investigated in the SSc patients. Compared with the Japanese general population, the SIR for developing cancer in SSc patient…

Myositis-specific and -associated autoantibodies: a review from the clinical perspective.

Twenty-five percent to 50% of patients with polymyositis (PM) and dermatomyositis (DM) have autoantibodies that are specific for or associated with the presence of myositis. Because of a relatively low sensitivity for the diagnosis of PM or DM, these autoantibodies are inadequate as a screening test for PM or DM in patients with suspected inflammatory myopathy. However, they may be used to support the diagnosis of myositis, and they help exclude the diagnosis of inclusion body myositis in which autoantibodies are infrequent. In addition, these antibodies are associated with homogeneous syndrom…

[Clinical significance of magnetic resonance imaging of skeletal muscles in idiopathic inflammatory myopathies of adults].

The purpose of this study was to evaluate the clinical significance of magnetic resonance imaging (MRI) of skeletal muscles in Japanese patients with idiopathic inflammatory myopathies (IIM). MRI was performed in 23 adult patients with IIM, including 10 with polymyositis, 12 with dermatomyositis, and 1 with focal myositis. Seven (73%) of 11 patients with active IIM and 2 (17%) of 12 patients with inactive IIM showed hyperintensity of T 2-weighted images and normal intensity of T 1-weighted images, indicating "edema-like abnormalities" (MRI findings for active myositis). Muscle lipomatosis and …

Epidemiology of idiopathic inflammatory myopathies: a population-based cohort in England, 2002-2021.

OBJECTIVE: Idiopathic inflammatory myopathies (IIM) are a heterogenous group of conditions with substantial morbidity but population-based epidemiology is lacking. We aimed to provide a contemporary epidemiological assessment of IIM. METHODS: Population-based study using the Clinical Practice Research Datalink (CPRD) Aurum primary care database with linkage to hospital episode data. All patients (aged >2 years) from England registered between 1 March 2002 and 31 March 2021 were included. Incidence was estimated for three main IIM subtypes: dermatomyositis (DM), inclusion body myositis (IBM) a…

Intravenous Immunoglobulin in Idiopathic Inflammatory Myopathies: a Practical Guide for Clinical Use.

PURPOSE OF REVIEW: Idiopathic inflammatory myopathies (IIM) are a complex family of autoimmune systemic disorders which often affect muscle and/or skin. IIM cause significant morbidity and mortality, but optimal treatment is uncertain. This review provides a practical guide for using intravenous immunoglobulin (IVIG) and subcutaneous immunoglobulin (SCIG) in the management of IIM, including dermatomyositis (DM), polymyositis (PM), immune-mediated necrotizing myositis (IMNM), and spontaneous inclusion body myositis (IBM), based on relevant recent literature and experience. Development and Preliminary Validation of an Electromyography-Scoring Protocol for the Assessment and Grading of Muscle Involvement in Patients With Juvenile Idiopathic Inflammatory Myopathies.

INTRODUCTION: We performed a pilot study in order to investigate the feasibility of an electromyography (EMG)-scoring protocol for the assessment of disease activity in juvenile idiopathic inflammatory myopathies (JIIM). METHODS: Children with JIIM followed up in a tertiary-level care center underwent standardized clinical, laboratory, and EMG assessment. An EMG-scoring protocol was devised by a consensus panel including a pediatric neurophysiologist and two pediatric rheumatologists, based on a combined score obtained as the sum of (1) the presence of denervation signs (fibrillation potentia
