Disruption of Undergraduate Clinical Training in Postconflict Syria: Insights from Clinical Practice and Implications for Future Health Workforce Readiness.

Clinical training is a fundamental component of medical education and plays a critical role in preparing future physicians for clinical practice. In Syria, the prolonged effects of conflict continue to shape health care institutions and training environments despite the decline of large-scale hostilities. Drawing on firsthand observations from clinical-stage medical students at Syrian public universities, we highlight recent changes in undergraduate clinical training, together with recent policy changes that discontinued practical assessments for fourth- and fifth-year students and concentrate

Closing the research-to-practice gap: Using a plan-do-study-act approach to test innovative interventions to increase HPV immunization uptake in Canada.

In 2020, the Canadian Partnership Against Cancer commissioned the Urban Public Health Network to lead a multi-phased project to identify the inequalities and barriers to HPV vaccination for school-aged children across Canada. In Phase One, the UPHN team collaborated with public health and community partners across Canada to identify barriers to vaccination at a sub-jurisdictional and sub-populational level using a multiple methods approach. In Phase Two, UPHN is working with existing and new partners to achieve system-level outcomes through developing and testing innovative education and progr

How illuminating, clarifying and contextualising shame can transform healthcare: the shame lens.

Despite widespread efforts to advance the Quadruple Aim, persistent challenges continue to impede progress. One important yet largely unacknowledged contributor may be shame-a complex and deeply human emotion that can profoundly shape behaviours, relationships and organisational dynamics throughout healthcare. In this paper, we propose the 'shame lens' as a novel framework for advancing the Quadruple Aim. We describe three functions of the shame lens: illuminating shame, clarifying shame-driven behaviours and contextualising shame within social and institutional structures, and we discuss thei

[Treatment type 1 diabetes in children].

Type 1 diabetes in children is an increasingly condition, even in young children. Medical care combines insulin therapy, a balanced diet meeting nutritional children's needs, physical activity and therapeutic education of patients and caregivers. Blood glucose monitoring can be performed using capillary blood tests or use of continuous monitoring glucose sensors help providing comfort and accuracy. Insulin treatment follows a basal bolus regiment which can be delivered by insulin pen or insulin pump. Functional insulin therapy is often proposed and consists in adjusting insulin dose according

Rehabilitation for individuals with post-tuberculosis lung disease.

Tuberculosis (TB) continues to be a global clinical and public health threat, particularly in low- and middle-income countries. TB care has primarily been centred around timely diagnosis and expanding access to treatment. Post-TB lung disease (PTLD), characterised by persistent respiratory symptoms, functional impairment and structural lung changes, is highly prevalent among TB survivors, with a prevalence ranging from 40% to 90%. This requires an effort to evaluate TB survivors in need of further care at the end of anti-TB treatment, for consideration of pulmonary rehabilitation. This review

Severe Hypoglycemia-Induced Seizure in a Four-Year-Old Following Initiation of Nadolol for Supraventricular Tachycardia.

Severe hypoglycemia is a rare but potentially life-threatening complication of non-selective beta-blockers in children. We report a four-year-old boy who developed a prolonged generalized tonic-clonic seizure shortly after initiation of nadolol for supraventricular tachycardia. The bedside random blood glucose was 0.9 mmol/L. He was promptly treated with intramuscular glucagon and intravenous dextrose, resulting in stabilization. History included refractory supraventricular tachycardia previously managed with esmolol and propranolol. Nadolol had been started the evening prior to the seizure. L

A Call to Action: Addressing the Mental Health Crisis in Schools.

School nurses play a key role in addressing the growing mental health crisis faced by school-age children. Rates of anxiety, attention deficit hyperactivity disorder, autism spectrum disorder, depression, and suicide continue to rise. These conditions reflect a complex interplay of genetic predisposition, environmental influences, and policy factors. School nurses can act at the federal, state, and local level to address the crisis. At the federal and state levels, nurses can engage policy discussions surrounding initiatives such as the 2025 MAHA report, universal screening mandates, school nu

Therapeutic Landscape of Early Symptomatic Alzheimer's Disease Translated into Everyday Practice for Geriatric Providers.

Alzheimer's disease (AD) accounts for 60-80% of all dementia cases. Recent advances in diagnostic biomarkers of early symptomatic AD (ie, mild cognitive impairment and mild dementia due to AD) and amyloid-targeting therapies (ATTs) have the potential to improve outcomes for patients with AD. Two ATTs (donanemab and lecanemab) are currently approved and available for use in the US. Both ATTs can slow disease progression as well as cognitive and functional decline in patients with early symptomatic AD. Treatment with ATTs is associated with specific safety concerns such as amyloid-related imagin

Carrying Life While Carrying Loss: Grief, PTSD, and Depression in Subsequent Pregnancy.

Background: Pregnancy loss is a profoundly traumatic event that can leave lasting emotional and psychological effects. While subsequent pregnancies are often thought to ease grief, many women continue to experience unresolved distress. Objective: This pilot study examined demographic, reproductive, and psychosocial correlates of grief intensity after pregnancy loss among women in the third trimester of a subsequent pregnancy. Methods: Twenty partnered women who had experienced a prior pregnancy loss, now beyond the gestational age of that loss, were recruited from childbirth classes. Participa

Antimicrobial stewardship in critical care.

Antimicrobial resistance (AMR) remains one of the most significant threats to global health, largely driven by inappropriate antibiotic use. This review outlines the current principles and practices of antimicrobial stewardship (AMS) in the context of critical care. We address the global burden of AMR, the development of resistance and the implementation of AMS strategies from multidisciplinary, diagnostic and pharmacokinetic perspectives. The use of rapid diagnostics, biomarkers such as procalcitonin, and the integration of electronic health records are discussed as enablers of targeted thera

Advancing Heart Failure Care Through Disease Management Programs: A Comprehensive Framework to Improve Outcomes.

Heart failure (HF) is a major global health challenge, characterized by high morbidity, mortality, and frequent hospital readmissions. Despite the advent of guideline-directed medical therapies (GDMTs), the burden of HF continues to grow, necessitating a shift toward comprehensive, multidisciplinary care models. Heart Failure Disease Management Programs (HF-DMPs) have emerged as structured frameworks that integrate evidence-based medical therapy, patient education, telemonitoring, and support for social determinants of health to optimize outcomes and reduce healthcare costs. Improving Post-PCI Cardiac Rehabilitation Referrals: A Collaborative Approach.

BACKGROUND: Clinical practice guidelines recommend referring all eligible patients who have undergone percutaneous coronary intervention (PCI) to cardiac rehabilitation (CR). PROJECT RATIONALE: The COVID-19 pandemic severely decreased patient participation in CR programs after PCI. Many CR facilities either closed or paused their services, and this decline in participation continued even after the pandemic ended. PROJECT SUMMARY: The hospital implemented a new process to improve referrals for CR, which included providing daily education to patients who had undergone PCI, submitting a CR refe

The ongoing challenge of prevention of pertussis in infants: what's new in 2024?

INTRODUCTION: Pertussis, a respiratory disease caused primarily by Bordetella pertussis, is undergoing a resurgence despite decades of high rates of vaccination. The prevention of pertussis in infants presents several challenges, including the waning immunity of the acellular pertussis (aP) vaccine, the limited protection afforded to newborns before they complete the vaccine series, and the existence of gaps in maternal vaccination. Furthermore, the unwillingness or refusal of a considerable number of individuals, including some healthcare workers, to receive vaccinations represents another si

An overview of the laboratory diagnosis of Pneumocystis jirovecii pneumonia.

Pneumocystis jirovecii (P. jirovecii) is a fungal pathogen associated with significant morbidity in immunocompromised patients, including both HIV- and non-HIV-infected patients. The nonspecific clinical and radiological presentation makes clinical diagnostic challenging, emphasizing the need for accurate laboratory diagnostic tests. However, P. jirovecii does not grow in routine culture media, which presents diagnostic challenges in the laboratory as well. Recent publications from the European Organization for Research and Treatment of Cancer and the Mycoses Study Group Education and Research

Best practice guidelines for viral hepatitis service delivery in prisons.

Viral hepatitis constitutes a major health burden among people living in prison globally. There is poor access to prison-based viral hepatitis services, and people living in prison are rarely prioritised for care, undermining global elimination efforts. This Review is a narrative summary of the first Global Guidelines for Viral Hepatitis Service Delivery in Prisons, which included a systematic review and an expert-led GRADE process to develop best practice recommendations for viral hepatitis management in prisons. 703 articles were included, underpinning the development of 30 recommendations a

Advocating for Change: Addressing Barriers To Tuberculosis Care for Immigrants and Refugees in Canada.

Tuberculosis (TB) disproportionately affects immigrants and refugees in Canada, who accounted for 80% of active TB cases in 2023 despite making up only 23% of the population. This commentary highlights the urgent need to address systemic barriers that hinder access to timely and effective TB care across the cascade, from screening and diagnosis to treatment completion. Drawing on recent policy reports and emerging evidence, this paper focuses on four main intersecting challenges: language barriers, limited cultural competency among providers, healthcare system inefficiencies, and misinformatio

Outcome measurement: Design of a social impact framework to measure shifts in the ecosystem for inclusion.

Children with disabilities need comprehensive intervention spanning health, education, and social systems from infancy through adulthood, with ongoing assessment of outcomes. While assistive technology is crucial, it must be complemented by inclusive learning options and other systemic interventions. Bronfenbrenner's Ecological Systems Theory guides this approach. A project in South Africa's Northern Cape Province utilized Acumen's Lean Data methodology for monitoring and evaluation. This methodology emphasizes rapid, technology-enabled data collection to inform stakeholders effectively. The p

A Qualitative Study of the Social Relationship Experiences Across the Life Course Among Black/African American Women Aging With HIV in the South.

Black/African American women continue to be disproportionately affected by HIV, facing multiple intersecting challenges that influence how they age and effectively manage their health. Supportive social relationships have been shown to help mitigate challenges and improve health in women with HIV, but little is known about Black/African American women's perceptions of social relationships. Guided by Life Course Theory, in-depth life history interviews were conducted with 18 Black/African American women aged 50+ years. In older adulthood, most important relationships among Black/African America
