Effects of Prednisone on a Patient with Dysferlinopathy Assessed by Maximal Voluntary Isometric Contraction: Alternate-Day Low-Dose Administration for a 17-Year Period.

Glucocorticoids are candidates for the pharmacological treatment of dysferlinopathy. Deflazacort, however, showed a worse effect on muscle s … as taken every other day in the morning until 41 years of age. The MVIC of flexion of the knees and elbows increased gradually and significantly. The MVIC of extension of the knees and elbows increased to a lesser extent. Isometric grip strength showed no remarkable increase, but strength was sustained over 10 years. Muscle fiber types account for these …

Tennis injuries: epidemiology, pathophysiology, and treatment.

Tennis places high loads on the joints of players, with supraphysiologic forces being generated at the shoulder and elbow hundreds of times per match. Acute injuries tend to affect the lower extremity; chronic injuries usually involve the upper extremity. Commonly encountered upper extremity conditions include rotator cuff injury, internal impingement, superior labral tears, and epicondylitis of the elbow. Serving is the most strenuous stroke in tennis, with the highest peak muscle activity in the shoulder and forearm occurring during this stroke. The kinetic chain links upper extremity, lower…

The middle radioulnar joint and triarticular forearm complex.

The forearm is composed of the radial and ulnar shafts, which are linked by the interosseous membrane and intercalated between the elbow and wrist. The radius and ulna are connected by three joints, the proximal, middle, and distal radioulnar joints. The forearm ensures pronation/supination and longitudinal load transfer. The biomechanical and clinical relevance of the proximal and distal radioulnar joints is well established. In contrast, the middle radioulnar joint was considered relatively unimportant until studies published in the last decade showed that it fulfils crucial biomechanical fu…

Delayed presentation of a brachial artery pseudoaneurysm after a supracondylar humerus fracture in a 6-year-old boy: a case report.

Supracondylar fractures of the humerus are the most common elbow fractures in children, accounting for 60% to 70% of all pediatric elbow fractures. These fractures often have neurovascular complications because of deformity and the sharp nature of the fracture fragments. The management of patients who present with diminished or absent pulses, but a well-perfused extremity is a topic of debate. Between 3% and 14% of patients present with an altered vascular examination and a consistent treatment logarithm has not been proposed in the literature. To our knowledge, a brachial artery pseudoaneurys…

Dynamics model for analyzing reaching movements during active and passive torso rotation.

We have developed an inverse dynamics model of unrestrained natural reaching movements. Such movements are usually not planar and often involve complex deformation of the shoulder girdle as well as rotary and linear torso motion. Our model takes as its input kinematic data about the positions of the finger, wrist, elbow, left and right acromion processes, and the sternum and produces the torques and forces developed at the shoulder, elbow, and wrist joints. The model can also be used to simulate the consequences of introducing passive torso rotation or linear acceleration on arm movements and …

Shoulder abduction-induced reductions in reaching work area following hemiparetic stroke: neuroscientific implications.

A stroke-related loss of corticospinal and corticobulbar pathways is postulated to result in an increased use of remaining neural substrates … duction torque. We have provided data demonstrating an incremental increase of abnormal coupling of elbow flexion for greater levels of shoulder abduction in the paretic limb that results in a reduction in available work area as a function of active limb support. The progressive increase in the expression of abnormal shoulder/elbow coupling can be expla …

Modified intramuscular transposition of the ulnar nerve.

The ulnar nerve passes posterior to the medial epicondyle and experiences longitudinal strain when subjected to elbow flexion. Furthermore, Osborne's ligament and the arcade of fibers between the 2 heads of the flexor carpi ulnaris (Osborne's fascia) compress the nerve during elbow flexion with narrowing of the cubital tunnel passageway. Some patients experience the added element of nerve subluxation over the posterior edge of the epicondyle. When changes to daytime ergonomic behavior and sleep posture prove insufficient to relieve a patient's symptoms, surgical treatment is warranted. A range…

Radial head fractures: MRI evaluation of associated injuries.

The purpose of this study was to evaluate the incidence of combined osteochondral and ligamentous injuries by magnetic resonance imaging (MRI) in 24 patients with an acute radial head fracture (Mason type II and III) without documented dislocation or tenderness at the distal radioulnar joint. Elbow radiographs (anteroposterior and lateral views) were obtained on all patients as well as magnetic resonance images in the sagittal, coronal, axial, axial oblique, and coronal oblique planes with the injured elbow in a splint. The incidence of associated injuries revealed by MRI was medial collateral…

Fixation of fractures of the shaft of the humerus by dynamic compression plate or intramedullary nail. A prospective, randomised trial.

We randomised prospectively 44 patients with fractures of the shaft of the humerus to open reduction and internal fixation by either an intramedullary nail (IMN) or a dynamic compression plate (DCP). Patients were followed up for a minimum of six months. There were no significant differences in the function of the shoulder and elbow, as determined by the American Shoulder and Elbow Surgeons' score, the visual analogue pain score, range of movement, or the time taken to return to normal activity. There was a single case of shoulder impingement in the DCP group and six in the IMN group. Of these…

The value of electrophysiological examination of the flexor carpi ulnaris muscle in the diagnosis of cubital tunnel syndrome.

In the electrodiagnosis of cubital tunnel syndrome, the abductor digiti minimi (ADM) muscle is frequently used to evoke compound muscle action potentials (CMAP) in order to measure the motor nerve conduction velocity (MNCV) in the ulnar nerve. Slowing of the MNCV across the elbow is a major criteria in the electrodiagnosis of cubital tunnel syndrome. However, in advanced cases, slowing of the MNCV is also seen in the forearm segment, thus, localization of the nerve palsy is sometimes difficult. We evoked CMAP from the flexor carpi ulnaris (FCU) muscle to assess the value of electrophysiologica…

Long-Term Functional Outcomes and Complications of Intra-Articular (AO type B, C) Distal Humerus Fractures in Adults: A Retrospective Review.

Introduction Treatment of intra-articular fractures of the distal humerus is challenging due to their complexity, comminution, and associated complications. The evolution of surgical approaches and the design of elbow-specific implants over the last decades have failed to improve clinical and radiological outcomes. Studies are sparse regarding the long-term influence of surgical treatment of these types of fractures in the upper limb function. The purpose of the current study was to retrospectively review the long-term functional outcome and complications of all intraarticular (AO type B, C) d…

Sagittal Plane Residual Deformity in Pediatric Type II Supracondylar Humerus Fractures.

INTRODUCTION: The incidence of residual deformity in the sagittal plane of the humerus (RDSPH) after nonoperative management of type II supracondylar humerus fractures (SCHFs), and the effects of such deformity on the overall arc of motion (AOM) of the elbow, are unknown. Our purpose was to analyze data collected prospectively on a large cohort of type II SCHF's to establish the incidence and extent of RDSPH, and the effects of the deformity on the elbow function, to further support our previously published recommendations on the treatment of type II SCHF. METHODS: The clinical data and radio…

Management of anteromedial coronoid fractures according to a protocol focused on instability assessment provides good outcomes with infrequent need for coronoid fixation.

BACKGROUND: Anteromedial coronoid fractures (AMCFs) are associated with persistent elbow instability and post-traumatic arthritis if managed incorrectly. It is unclear exactly which AMCFs require surgical intervention and how to make this decision. The aims of this study were to report outcomes of AMCFs managed using a protocol based on reproduction of instability using radiographic and clinical testing and to ascertain a threshold size of AMCF associated with instability. METHODS: Forty-three AMCFs were studied. Thirty-two patients formed the primary study group (group A). All were treated u…
