Motor learning in essential tremor.

Classical conditioning is abnormal in patients with cerebellar degeneration. Pathological studies have recently demonstrated the finding of Purkinje cell loss in a subset of essential tremor (ET). We studied 10 subjects with long duration ET using a blink reflex conditioning paradigm and compared them with nine controls. A 55.6% reduction in motor learning was seen in the first block of stimulation and this finding persisted across all subsequent trials. We conclude that motor learning is significantly abnormal in subjects with advanced ET and likely reflects pathological involvement of the ce

Lipomatosis of the sciatic nerve: report of a case and review of the literature.

We report a case of lipomatosis of the sciatic nerve, also known as lipofibromatous hamartoma. A male, 26-year-old, presented with gait impairment, leg pain, and foot drop compatible with sciatic nerve's compression. The preoperative magnetic resonance imaging (MRI) revealed diffuse thickening of the proximal third of the sciatic nerve with adipose tissue interspersed among the nerve fascicles. Internal neurolysis was performed with microsurgical techniques under high magnification. The patient was followed up for a period of 34 months. He significantly recovered, and there was no clinical rec

Patellofemoral pain and the mechanomyographic responses of the vastus lateralis and vastus medialis muscles.

The purpose of this investigation was to examine the influence of patellofemoral pain (PFP) on the amplitude of the mechanomyographic (MMG) and electromyographic (EMG) signals from the vastus lateralis and vastus medialis muscles of the quadriceps femoris. Nine females reporting current signs and symptoms of PFP and 8 healthy females service as the control (CTL) group volunteered to participate in this study. Participants completed maximal and submaximal (25, 50, 75% MVC) isometric muscle actions of the quadriceps femoris at a leg flexion angle of 45 degrees below the horizontal plane of the l

Exercises for the torso performed in a standing posture: spine and hip motion and motor patterns and spine load.

The purpose of this study was to document the muscle activity, spine motion, spine load, and stiffness during several movement-based or "functional" exercises and to assess the effect of technique change. Eight subjects, all healthy men from a university population, were instrumented to obtain surface electromyography of selected trunk and hip muscles, together with video analysis and electromagnetic lumbar spine position sensor to track spine posture. Exercises included a walkout in the sagittal plane that compared an upright form against a wall with those performed on the floor, overhead cab…

Vocal cord dysfunction in amyotrophic lateral sclerosis: four cases and a review of the literature.

We describe 4 patients with amyotrophic lateral sclerosis (ALS) and glottic narrowing due to vocal cord dysfunction, and review the literature found using the following search terms: amyotrophic lateral sclerosis, motor neuron disease, stridor, laryngospasm, vocal cord abductor paresis, and hoarseness. Neurological literature rarely reports vocal cord dysfunction in ALS, in contrast to otolaryngology literature (4%-30% of patients with ALS). Both infranuclear and supranuclear mechanisms may play a role. Vocal cord dysfunction can occur at any stage of disease and may account for sudden death i

C5 and C6 human dermatomes: a clinical, electromyographical, imaging and surgical findings.

There is substantial controversy in literature about human dermatomes. In this work, C5 and C6 superior limb dermatomes were studied. The method consisted of comparing clinical signs and symptoms with conduction studies, electromyographical data, neurosurgical findings, and imaging findings obtained by computerized tomography (CT) or magnetic resonance imaging (MRI), for each patient. Data analysis from superior members in 18 patients suggests that C5 is located in the lateral aspect of the shoulder and arm, and C6 in the lateral aspect of the forearm and 1st, 2nd, and 3rd fingers. To our know

Babbling, chewing, and sucking: oromandibular coordination at 9 months.

PURPOSE: The ontogeny of mandibular control is important for understanding the general neurophysiologic development for speech and alimentary behaviors. Prior investigations suggest that mandibular control is organized distinctively across speech and nonspeech tasks in 15-month-olds and adults and that, with development, these extant forms of motor control primarily undergo refinement and rescaling. The present investigation was designed to evaluate whether these coordinative infrastructures for alimentary behaviors and speech are evident during the earliest period of their co-occurrence. MET

Interspike interval analysis in a patient with peripheral nerve hyperexcitability and potassium channel antibodies.

Neuromyotonia or Isaacs' syndrome is a rare peripheral nerve hyperexcitability disorder caused by antibodies against potassium channels of myelinated axons. We present the high-density surface electromyographic (EMG) recordings of a patient with fasciculations and cramps due to neuromyotonia. To characterize the time course of hyperexcitability, we analyzed the interspike intervals (ISIs) between fasciculation potentials, doublet, and multiplet discharges. ISI duration increased within each burst. The ISI histograms found can be explained by the recovery cycle of the myelinated axon and its de

Recurrent miller fisher syndrome with abnormal terminal axon dysfunction: a case report.

Miller Fisher syndrome (MFS) is a localized variant of Guillain-Barré syndrome (GBS), characterized by ophthalmoplegia, areflexia, and ataxia. Recent neurophysiological studies have suggested that abnormal terminal axon dysfunction occurs in some cases of Miller Fisher syndrome and Guillain-Barrd syndrome. We present a rare case report of recurrent MFS with abnormal terminal axon dysfunction. To the best of our knowledge, this is the first case report of recurrent MFS with terminal axon dysfunction that persisted up to nine months after the initial presentation of the second attack with positi

Geste antagonistes in idiopathic lower cranial dystonia.

Geste antagonistes, or sensory tricks, are well described in focal dystonia affecting the neck, hand, and face. Improvement in dystonic movements is typically maintained while the trick is performed, but disappears when the geste ends. We investigated the phenomenological features of geste antagoniste maneuvers in 19 patients with idiopathic lower cranial dystonia who were prospectively evaluated over a period of 6 years. Of the 19, 10 were men, mean age of onset was 49.8 years, and the most commonly involved lower cranial area was the jaw (10 patients). In most patients, dystonia was task-spe

Comparison of the vastus-splitting and median parapatellar approaches for primary total knee arthroplasty: a prospective, randomized study. Surgical technique.

BACKGROUND: A vastus-splitting approach for total knee arthroplasty has been advocated to preserve function of the extensor mechanism and to decrease the prevalence of lateral release. Critics have claimed that there is greater blood loss and compromised exposure in large patients who are managed with this approach. The purpose of the present study was to compare vastus-splitting and median parapatellar approaches for primary total knee arthroplasty. METHODS: Forty-two consecutive patients (fifty-one knees) undergoing primary total knee arthroplasty were randomized to treatment with a median

Electromyographical study on muscle fatigue in repetitive forearm tasks.

The purpose of this study was to examine whether repetitive muscle tasks in low weight load might influence the fatigue of forearm muscles, and to identify ergonomic risk factors of forearm muscle fatigue in these tasks. Sixteen healthy male volunteers performed eight wrist extensions in different frequency, weight and angle loads while being instructed to keep a dominant upper limb posture as constant as possible. Surface electromyograph (sEMG) was recorded from right extensors digitorium (ED), flexor carpi radialis (FCR), flexor carpi ulnaris (FCU) and extensor carpi ulnaris (ECU) during the

Diagnosis of thoracic outlet syndrome.

Thoracic outlet syndrome (TOS) is a nonspecific label. When employing it, one should define the type of TOS as arterial TOS, venous TOS, or neurogenic TOS. Each type has different symptoms and physical findings by which the three types can easily be identified. Neurogenic TOS (NTOS) is by far the most common, comprising well over 90% of all TOS patients. Arterial TOS is the least common accounting for no more than 1%. Many patients are erroneously diagnosed as "vascular" TOS, a nonspecific misnomer, whereas they really have NTOS. The Adson Test of noting a radial pulse deficit in provocative p
