[Treatment of femoral shaft fractures in children and adolescents ≥50 kg : A retrospective multicenter trial].

BACKGROUND: Operative treatment of diaphyseal fractures of the femur in older children and adolescents remains controversial due to multiple surgical options and higher complication rates in single-center studies compared to younger children. This retrospective multicenter study aimed to register early and late complications in day-by-day treatment. MATERIAL AND METHODS: Sixteen hospitals with particular expertise in pediatric orthopedic trauma participated in this study. Patients with diaphyseal femur fractures, a body weight ≥50 kg (aged 10-16 years) and treated between 2008 and 2012 were i

Functional outcomes and clinical strength assessment after infraspinatus-sparing surgical approach to scapular fracture: Does it really make a difference?

BACKGROUND: Surgical treatment of scapular fractures with posterior approach is frequently associated with postoperative infraspinatus hypotrophy and weakness. The aim of this retrospective study is to compare infraspinatus strength and functional outcomes in patients treated with the classic Judet versus modified Judet approach for scapular fracture. PATIENTS AND METHODS: 20 cases with scapular neck and body fracture treated with posterior approach for lateral border plate fixation were reviewed. In 11 of 20 cases, we used the modified Judet approach (MJ group), and in 9 cases we used the cl

Multimodal noninvasive monitoring of soft tissue wound healing.

Here we report results of non-invasive measurements of indirect markers of soft tissue healing of traumatic wounds in an observational swine study and describe the quantification of analog physiological signals. The primary purpose of the study was to measure bone healing of fractures with four different wound treatments. A second purpose was to quantify soft tissue wound healing by measuring the following indirect markers: (1) tissue oxygenation, (2) fluid content, and (3) blood flow, which were all measured by non-invasive modalities, measured with available devices. Tissue oxygenation was m

Localized deferoxamine injection augments vascularity and improves bony union in pathologic fracture healing after radiotherapy.

BACKGROUND: Medically based efforts and alternative treatment strategies to prevent or remediate the corrosive effects of radiotherapy on pathologic fracture healing have failed to produce clear and convincing evidence of success. Establishing an effective pharmacologic option to prevent or treat the development of non-unions in this setting could have immense therapeutic potential. Experimental studies have shown that deferoxamine (DFO), an iron-chelating agent, bolsters vascularity and subsequently enhances normal fracture healing when injected locally into a fracture callus in long bone ani

Post-traumatic bone and/or joint limb infections due to Clostridium spp.

BACKGROUND: Clostridium spp. are saprophytic Gram-positive bacteria found in soil and capable of generating endospores. Spore germination occurs when environmental conditions are favorable. Clostridium spp. can cause infections of compound fractures and deep wounds contaminated from soil micro-organisms. HYPOTHESIS: Clostridium spp. infections of traffic-related injuries are particularly severe events whose outcome is uncertain even with aggressive medical and surgical treatment. MATERIALS AND METHODS: We retrospectively reviewed 12 patients (median age, 45 years) with Clostridium spp. bone

Biomechanical evaluation of a low profile, anchored cervical interbody spacer device in the setting of progressive flexion-distraction injury of the cervical spine.

INTRODUCTION: Anterior cervical decompression and fusion is a well-established procedure for treatment of degenerative disc disease and cervical trauma including flexion-distraction injuries. Low-profile interbody devices incorporating fixation have been introduced to avoid potential issues associated with dissection and traditional instrumentation. While these devices have been assessed in traditional models, they have not been evaluated in the setting of traumatic spine injury. This study investigated the ability of these devices to stabilize the subaxial cervical spine in the presence of fl

Ultra-low-velocity knee dislocations.

BACKGROUND: Reports of low-velocity knee dislocations have focused primarily on dislocations occurring during athletic competition. The authors identified a subset of patients with low-velocity knee dislocations that occurred during activities of daily living, such as stepping off a curb, stepping off a stair, or simply falling while walking (ultra-low-velocity dislocations). HYPOTHESIS: Ultra-low-velocity knee dislocations are common in obese individuals and are associated with more complications than high-velocity knee dislocations. STUDY DESIGN: Case series; Level of evidence, 4. METHODS

[Posterior extensile approach to release a post-traumatic stiffness of the elbow].

PURPOSE OF THE STUDY: Restriction of joint motion is a frequent complication in the treatment of elbow injury. Grade 4 elbow stiffness up to its ankylosis is a strong limiting factor in the patient's daily activities. The surgical release of a stiff elbow has been a rare procedure. It can be done using either isolated or combined approaches. The aim of the study is to present our experience with and the results of elbow arthrolysis using a posterior extensile approach to the joint. MATERIAL AND METHODS: Seven patients aged between 17 and 38 years with post-traumatic post-operative ankylosis,

Surgical management of knee dislocations. Surgical technique.

BACKGROUND: The evaluation and management of knee dislocations remain variable and controversial. The purpose of this study was to describe our method of surgical treatment of knee dislocations with use of a standardized protocol and to report the clinical results. METHODS: Forty-seven consecutive patients presented with an occult (reduced) or grossly dislocated knee. Fourteen of these patients were not included in this series because of confounding variables: four had an open knee dislocation, five had vascular injury requiring repair, three were treated with external fixation, and two had a

Abdominoplasty and abdominal wall rehabilitation: a comprehensive approach.

Standard abdominoplasty techniques involve a low horizontal or W skin excision, muscle plication, and umbilical transposition. Newer techniques include suction-assisted lipectomy, the use of high lateral tension with fascial suspension, and external oblique muscle advancement. The author has modified these traditional procedures and added new techniques to improve the aesthetic and functional results of the abdominoplasty procedure. This modification provides a comprehensive approach to abdominal wall aesthetic improvement and rehabilitation. The comprehensive approach described includes four

[Results of surgical repositioning of congenital hip dislocation].

The authors submit an analysis of the results of surgical treatment of inborn dislocation of the hip joint in children from the age of one year to completed growth. Decisive for inclusion into this group was surgical reposition. The operated patients were divided into the following three groups by age at the time of operation: 1. first year up to 17 months; 2. 18 to 48 months; 3. patients older than 4 years up to 15 years. Between 1978 and 1992 96 children with inborn dislocation of the hip joint were operated (103 hip joints). Complete documentation was available for 59 hip joints of 54 patie

[Diagnosis, Treatment and Complications of Knee Dislocation: a Retrospective Study].

PURPOSE OF THE STUDY Acute knee dislocation is a less common injury of the knee joint. It is, however, a serious injury with a high rate of nerve and vascular damage and it is considered a limb threatening injury with long-term functional disability, which can ultimately lead to amputation. Knee dislocations constitute less than 0.5% of all joint dislocations. Most of these injuries occur in highenergy traumas and careful diagnosis can identify the patient at risk of this injury. MATERIAL AND METHODS The total number of patients with knee dislocation was 37. The incidence of knee dislocation w

Acute elbow dislocation with arterial rupture. Analysis of nine cases.

UNLABELLED: Elbow dislocations are the most frequently encountered dislocations after shoulder dislocations. In their vast majority these injuries involve only the joint and carry a good prognosis. Close anatomic proximity to the joint of neurovascular structures put them at risk of concomitant injury but this occurrence remains, actually very rare. The objective of this study is to retrospectively analyze the results of nine cases of elbow dislocations with brachial artery complications and to propose coherent therapeutic guidelines derived from this experience. MATERIALS AND METHODS: From 1

Incisional hernia repair trends in the last decade (2013-2023): an ACHQC analysis.

AIM: This study evaluates trends in incisional hernia repair techniques and their associated outcomes over 10 years. It hypothesizes that advancements in surgical techniques have led to an increase in minimally invasive procedures, resulting in a subsequent decrease in 30-day postoperative complications. MATERIAL & METHODS: A retrospective review of prospectively collected data from the Abdominal Core Health Quality Collaborative (ACHQC) database was conducted from January 2013 to December 2023. Adult patients who underwent incisional ventral hernia repair via open, laparoscopic, or robotic ap

Directed acoustic shadow enhancement for pre-incision ultrasound localization and precise incision planning in multiple rib fractures undergoing SSRF: a retrospective comparative feasibility study.

BACKGROUND: Accurate pre-incision localization of fracture segments is essential for targeted exposure during surgical stabilization of rib fractures (SSRF). We evaluated Directed Acoustic Shadow Enhancement (DASE), a simple ultrasound-assisted localization technique using an external cotton swab as an acoustic reference marker to translate sonographic fracture findings into skin surface marking. METHODS: This single-center retrospective comparative study included 91 adult patients with multiple (≥3) rib fractures who underwent SSRF, including 22 consecutive DASE-assisted cases and 69 histori

Magnetic Intramedullary Lengthening Nails for Complex Lower Extremity Reconstruction: Functional Outcomes, Bone Healing, and Complications.

Background: Magnetically controlled intramedullary lengthening nails have become increasingly popular for lower extremity reconstruction because they reduce complications associated with external fixation and facilitate earlier rehabilitation. However, comprehensive data integrating deformity correction, bone healing, functional recovery, psychosocial outcomes, and complication-related analyses remain limited. This study aimed to evaluate the radiological, functional, psychosocial, and complication-related outcomes of magnetic intramedullary lengthening nails in patients undergoing complex low

Outcomes at mean 8-year follow-up after reverse total shoulder arthroplasty for failed fracture treatment.

BACKGROUND: Proximal humerus fractures are a common challenge in patients with initial treatment ranging from nonoperative management, to open reduction and internal fixation, to arthroplasty. Initial treatment failure is not uncommon, and revision to reverse total shoulder arthroplasty (rTSA) has emerged as a viable salvage option, though long-term outcomes in these patients are variable and remain underreported. This study aimed to evaluate long-term outcomes and failure rates of rTSA after failed initial proximal humerus management (nonoperative and operative). We hypothesized that rTSA for
