Analysis of the bleeding manifestations of 130 cases of dengue haemorrhagic fever admitted into the Children's ward of the General Hospital, Kuala Lumpur from May 1973 to September 1978 has been done. Petechial skin rash, epistaxis and gum bleeding were seen most commonly in mild and moderately severe cases. However, blood stained gastric aspirates, and severe haematemesis were seen in severe or very severe cases. Though with better vector control and preventive measures, a marked reduction in the incidence of the cases has been noted, severe cases were seen with symptoms of shock and gastroin…

We describe five children in whom symptoms and signs fully consistent with a diagnosis of acute rheumatic fever arose in association with infection by Mycoplasma pneumoniae. With the decline of streptococcal disease as a causative agent of acute rheumatic fever, other etiologic possibilities capable of reproducing the clinical syndrome - especially Mycoplasma pneumoniae - should be considered, as this may have therapeutic and prognostic implications.

Athymic nude mice and euthymic littermate controls were exposed to 10(4) Coxiella burnetii organisms by small-particle aerosol. Antibody res … in nude and euthymic mice on days 7 and 14. Later, the number and size of lesions attributable to Q fever diminished in euthymic mice. Infection was progressive in nude mice, with macrophage infiltration of most tissues, especially spleen and liver. Numerous rickettsiae were seen by immunofluorescence and electron microscopy in phagocytic vesicles of ma …

In the case of Legionnaires' disease described, severe respiratory problems necessitated mechanical ventilatory support, tracheal intubation, and positive end-expiratory pressure. Fever was eliminated with erythromycin therapy but returned after five days, and lung infiltrates spread. After supplementary treatment with other antibiotics and methylprednisolone sodium succinate, both the fever and the infiltrates disappeared. We feel that the multisystem involvement and the recrudescence of fever in our patient emphasize the wide spectrum of characteristics of Legionnaires' disease and the impor…

Previous reports have well documented the radiographic appearance of typhoid fever as seen on barium meal antegrade studies. A case is presented whereby barium enema, with small bowel reflux, presents similar radiographic findings. Because the enema is more likely to be employed in emergency situations to exclude appendicitis prior to definitive diagnosis, knowledge of such an appearance of typhoid fever is important.

We discuss the case of a 24-year-old black woman at 33--34 weeks gestation, who after intravenous injection of Talwin presented with the fol … red through cotton ball. History plus exclusion of other etiologies led to the diagnosis of "cotton fever." The available literature is reviewed, and the importance of recognizing this entity when servicing a pregnant population with a high rate of drug abuse is discussed.

Under continuous observation of several months, 42 patients from the eastern province of Sierra Leone, Liberia (Lofa County), and neighbouring Guinea were identified as Lassa fever cases by indirect immunofluorescent antibody technique, indicating that the disease is endemic in these areas. The clinical course varied from mild disease to severe illness with haemorrhagic disorders. The fatality rate was 14%. The occurrence of only two possible secondary cases suggests that person-to-person spread of the disease is unimportant epidemiologically. There was a wide range of patients' ages, tribes, …

Spleen and lymph nodes from seven fatal cases of Argentine hemorrhagic fever were studied by light and electron microscopy and by immunofluorescent techniques. In addition, Junin virus infectivity was determined. Destruction of splenic white pulp and of lymph node cortex was observed. The ultrastructural study showed a characteristic cytopathic effect on the macrophages with presence of intracisternal virus-like particles and occasional virions budding from the plasma membrane. The immunofluorescent techniques indicated the presence of Junin virus antigenic determinants in the cytoplasm of mac…

A subhuman primate model was developed for study of the pathogenesis of infection with Coxiella burnetii. Cynomolgus monkeys (Macaca fascicularis) that were exposed to 10(5) mouse median infectious intraperitoneal doses of C. burnetii in a small-particle aerosol developed clinical signs of illness and pathologic changes characteristic of Q fever infection in humans. All monkeys had radiologic evidence of pneumonia by day 9. Antibodies to C. burnetii were detectable by the indirect fluorescent antibody test by day 7. These data indicate that the cynomolgus monkey is a suitable model for study o…

Evidence of chronic Coxiella burneti infection of the heart, a disease previously considered peculiar to patients with valvular heart-disease, was found in a patient during routine serological tests before resection of a ventricular aneurysm and also by isolation of the rickettsia from the resected tissue. The patient had no symptoms or signs of Q-fever endocarditis and none of the laboratory evidence usually associated with it.

Review of clinical and pathologic data from ten fatal cases of Rocky Mountain spotted fever (RMSF) revealed the importance of acute renal failure in the clinical course and of multifocal perivascular interstitial nephritis as the principal pathologic lesion. In nine cases, Rickettsia rickettsii were demonstrated by immunofluorescence in the areas of vasculitis. Evidence was lacking for the role of disseminated intravascular coagulation, glomerulonephritis, or myoglobinuria in the pathogenesis of acute renal failure in these cases. Rickettsia-induced vascular injury led to acute renal failure b…
