Orbitomaxillomandibular reconstruction with free osteomyocutaneous fibular flap and lower trapezius myocutaneous flap.

Several types of flaps and techniques have been used in the reconstruction of maxillomandibular defects. Myocutaneous flaps of the fibula, the scapula, the rib, the iliac bone, the radial forearm, the rectus abdominis, the anterolateral thigh, the latissimus dorsi, and the pectoralis major have been used either alone or in combination for this purpose. The aim of the current study was to discuss a 17-year-old patient with a gunshot injury who underwent orbitomaxillomandibular bone reconstruction using free fibular graft shaped as 3 pieces and soft tissue reconstruction using lower trapezius myo…

Late reconstruction of median nerve palsy.

The median nerve provides sensory innervation to the radial aspect of the hand, including the palm, thumb, index, long, and half of the ring fingers. It provides motor innervation to most of the volar forearm musculature and, importantly, to m ost of thenar musculature. The main goal of median nerve reconstructive procedures is to restore thumb opposition. There are a variety of transfers that can achieve this goal but tendon transfers must recreate thumb opposition, which involves 3 basics movements: thumb abduction, flexion, and pronation. Many tendon transfers exist and the choice of tendon…

Primary inoculation tuberculosis following a vehicular accident.

Tuberculosis is considered as a 're emerging disease', because of its resurgence and increased incidence in the 21 st century particularly in immuno-compromised patients. About one fifth of diagnosed new cases of tuberculosis have an extrapulmonary lesion, of which about one-tenth involve the musculoskeletal system. Tuberculosis involving the soft tissue from adjacent bone or joint is well recognized but cutaneous tuberculous infection is rare, accounting for 0.1% of all cases seen in a dermatology service. We report a case of primary cutaneous tuberculosis of forearm following a vehicular acc…

New workhorse flaps in hand reconstruction.

With the passage of time, certain hand surgery procedures are anecdotally dubbed "workhorse" techniques. These are procedures that are extremely reliable and have repeatedly demonstrated good results. However, with time, paradigms undergo shifts, and this is as true for hand surgery as any other field. In this article, we will describe the use of three new "workhorse" flaps that we have found to have reliable results in complex hand reconstruction: the pedicled radial forearm fascia flap for dorsal hand reconstruction, the free anterolateral thigh flap for mangled hand reconstruction, and the …

[The first Belgian hand transplantation case. Nine years follow-up].

The authors report the clinical result at 9 years followup of a unilateral hand transplantation performed on the dominant side after traumatic forearm amputation. The patient is back to work and sporting activities. The allograft has been fully incorporated into his self-image. The satisfaction index is 10/10, the DASH 5/100, and the 400-points score 68.1 %. The Lanzetta HTSS score is 91 (excellent). Good function of extrinsic and intrinsic muscles is observed. The grasping strength is markedly decreased (7.4 % as compared to the normal non-dominant side). The patient has experienced several r…

All-arthroscopic repair of peripheral triangular fibrocartilage complex tears using FasT-Fix.

Injury to the triangular fibrocartilage complex (TFCC) is a major source of ulnar-sided wrist pain that results in disability with common activities of daily living involving forearm rotation, for which operative management is indicated if conservative management fails. Past results with open repairs have been successful, but recent surgical advances have allowed the development of arthroscopic management. This article describes and reviews an all-arthroscopic technique of repair of Palmer type IB TFCC injuries with FasT-Fix suture technology (Smith and Nephew, Andover, MA, USA), which is adva…

[How to avoid operative injury to Martin-Grüber communicating branch in the transmuscular medial approach of the elbow?].

The incidence of Martin-Grüber medio-ulnar communicating branch (MUCB), from median nerve to ulnar nerve, is about 20%. To avoid operative injury to MUCB, the authors suggest some modifications to the medial approach of the elbow. They suggest medial aponeurotomy of the anterior compartment of the forearm, disinsertion of the medial epicondylar muscles with dissection between the flexor carpi ulnaris (FCU) and the other medial epicondylar muscles, not exceeding 3 cm distally. This allows a valuable exposure of the elbow joint through a medial approach, with FCU disinsertion and ulnar nerve tra…

Distal metaphyseal radius fractures in children: reduction with or without pinning.

We retrospectively reviewed 39 distal metaphyseal radius fractures in children. This study compares the results of closed reduction with or without percutaneous pinning. Twenty-four fractures were treated by closed reduction and above-elbow cast immobilisation. Re-displacement was noted after one week in three patients, for which they were revised with reduction and pinning. Fifteen patients were initially treated by closed reduction and pinning. In these patients we saw no redisplacement after six weeks immobilisation in a forearm cast. We recommend closed reduction and pinning as a predictab…

Management of brachial plexus injury in adults.

Brachial plexus injury in adults is commonly caused by motorcycle accidents. Surgical management consists of nerve repair and nerve grafting for extraforaminal nerve root or trunk injury, and of neurotization or nerve transfer for nerve roots avulsion. In general, the results regarding restoration of shoulder and elbow function are good but reinnervation of the forearm muscles is less than safisfactory in respect to restoration of hand function. Functioning free muscle transfer in combination with selective nerve transfer is a reasonable alternative surgical procedure. Key words: Brachial plex…

Reconstruction of traumatic Stensen duct defect using a vein graft as a conduit: two case reports.

Treatment of Stensen duct defect is still controversial. The authors describe the successful use of a retrograde vein graft as a conduit for traumatic segmental Stensen duct defect reconstruction. One patient sustained facial trauma with severe duct crushing and severance, the other had multiple cutting injuries with segmental duct defect. However, primary repair was impossible. A retrograde vein graft harvested from forearm for Stensen duct defect reconstruction was performed using microsurgical technique. A silicon stent was retained for 8 weeks. The sialographic examination showed good func…

PUVA-induced lichen planus.

A 28-year-old male with histologically proven psoriasis vulgaris was administered oral 8-methoxypsoralen UVA therapy (PUVA). The plaques of psoriasis gradually responded to treatment, however numerous pruritic, violaceous papules over the right forearm and left thigh developed 5 months after starting PUVA (45 sittings with a cumulative dose of 156 J/cm(2)). Histopathologic examination of these lesions was compatible with the diagnosis of lichen planus (LP). On stopping PUVA therapy, the lesions subsided in one month. This case emphasizes the hypothesis that cell injury caused by PUVA therapy c…

Successful replantation of an amputated leg with severe crush and avulsion injury in an elderly patient: case report.

An amputated leg with severe crush and avulsion injury was replanted in a 57-year-old man. Since the skin and soft tissue around the ankle became necrotic 11 days postoperatively, a flow-through forearm free flap was transferred into the defect, preserving the blood circulation to the replanted leg. Seven weeks after the replantation, sural nerve cable grafts were interposed between both amputated edges of the tibial nerve. As the function of the ankle joint did not recover, arthrodesis of the ankle joint was performed 7 months after the replantaion. As a result, the patient could walk 1 year …

[Pediatric "floating elbow"--case reports].

Floating elbow is an injury that is characterized by simultaneous fracture of humerus and forearm of the same extremity. The injury is caused by high energy (falls, car accidents) and is often associated with extensive soft tissue contusion of affected limb and there may be present lesion of neurocircular bundle. Operative treatment (minimally invasive percutaneous pinning) should be performed as soon as possible after patient's admission and this treatment should be preferred before circular casting, which may cause compartment syndrome. In the paper we present two cases of pediatric floating…

Brachial plexus injury after transfer of free latissimus dorsi musculocutaneous flap.

Free latissimus dorsi muscle transfer is widely used for functional reconstruction of flexion of elbow and fingers after brachial plexus injury at later stage, as well as for soft tissue coverage because of its large size and long and reliable pedicle with adequate vessel diameter. Common complications recorded in literatures are hematoma and muscle atrophy due to a compartment syndrome. We treated a patient with soft tissue avulsion at forearm using free latissimus dorsi muscle transfer for soft tissue coverage in our hospital. Unfortunately during the transfer the patient's brachial plexus w…

Severe complex regional pain syndrome type II after radial artery harvesting.

The radial artery is increasingly used as a coronary bypass graft. We report on a patient who developed a severe complex regional pain syndrome type II--also called causalgia--after radial artery harvesting. After an odyssey of diagnostic and presumed therapeutic procedures, she underwent surgical revision of the left forearm. Intraoperatively, a titanium clip in close neighborhood to the superficial radial nerve affecting the perineurium was found and removed, and more distally, a small neurinoma (3 by 4 mm) was resected. Despite presumed successful operation, symptoms did not improve. The pa…

[A case of familial hypokalemic periodic paralysis with hyperuricemia during paralytic attack and genetic analysis of the pedigree].

We reported a 13-year-old boy and his family with hypokalemic periodic paralysis. He showed marked hyperuricemia during his paralytic attack, although neither ischemic forearm exercise test nor bicycle-ergometer exercise test revealed myogenic hyperuricemia when he was free from paralysis. Genetic analysis was performed to the proband and his affected elder brother, mother, and, maternal grand mother. We found the Arg528His mutation of CACNL1A3 gene in all the patients examined. The severity of the attacks and the age of onset did not vary in the different generations, and male predominancy wa…

Osteosynthesis of diaphyseal fractures of the radius and ulna using an internal fixator (PC-Fix). A prospective study.

Over a five year period, 121 forearm shaft fractures in 82 patients including nine non unions were fixed using the PC-Fix. Two patients died early; of the remaining 80 patients (119 bones) 96% were followed until bone healing. No non unions or late infections were observed in spite of the fact that no primary bone grafts were used except when treating non unions. In this clinical study with a high follow up rate using a newly developed internal fixator, encouraging overall results were achieved and the technology of monocortical locked screws was proven to be valid.

[Kickboards - Cause of injury, patterns of injury and prevention].

69 patients with kickboard injuries were treated over a 5-month period. 6 patients were admitted to hospital. Fractures (48%), contusions/ lacerations (45%) and capsular/ ligamentuous injuries (27%) were the most prevalent types of injury. Injuries of the elbow, forearm, wrist and hand accounted for 54% of all cases. Ankle (23%), head (6%) and knee were other regions frequently involved. It is concluded that kickboard-driving imposes a severe risk of injuries. The typical kickboard injuries was the fracture of the wrist and the ankle sprain. Protective equipment for the injured regions is very…
