Open Versus Arthroscopic Reduction for Perilunate and Lunate Dislocations: A Systematic Review and Network Meta-Analysis.

PURPOSE: Perilunate and lunate dislocations represent complex wrist injuries requiring urgent reduction and stabilization. While open reduction has been the gold standard, arthroscopic techniques have emerged as less invasive alternatives. However, comparative evidence remains limited, and the management strategies are controversial. METHODS: We conducted a systematic review and network meta-analysis following Preferred Reporting Items for Systematic Reviews and Meta-Analyses 2020 guidelines. Dedicated searches of major databases were performed up to June 16, 2025. Studies comparing open and

Outcomes of primary total hip arthroplasty using dual mobility cups across body mass index categories: a multi-centric cohort study.

INTRODUCTION: Performing a total hip arthroplasty (THA) in obese patients is known to be associated with an elevated rate of complications. The use of dual mobility cups (DMC) has already been proven for decreasing dislocation episodes in high-risk populations. This is an observational cohort study evaluating the functional and radiological outcomes of DMC across BMI categories namely healthy, overweight and obese populations. METHODOLOGY: This is a retrospective multi-centric study of 419 patients who underwent a primary THA with a DMC (THA-DMC) from 2006 to 2022. The patients were divided i

Does integrating pelvic tilt change from sitting to standing in functional 3D planning enhance early complications of robotic-assisted total hip arthroplasty?

AIMS: While robotic-assisted total hip arthroplasty (THA) improves implant positioning accuracy, it often overlooks dynamic lumbopelvic parameters, such as pelvic tilt changes between sitting and standing. These factors are increasingly recognized as critical in preventing early complications, such as impingement and dislocation. The aim was to determine whether integrating pelvic tilt variations into functional 3D planning reduces complications and improves outcomes following THA. METHODS: In this retrospective cohort study, 656 patients underwent robotic-assisted THA using either convention

Virtual Coaching and the Reduction of Radial Head Subluxation: A Case Series.

OBJECTIVES: After the establishment of the virtual pediatric emergency medicine clinic at our institution, we noted that several physicians independently began to instruct caregivers virtually on reducing a radial head subluxation. We thus conducted a case series to investigate the number, success, and follow-ups for the virtual reduction of radial head subluxation. METHODS: The electronic medical records at our institution were searched from the inception of the virtual clinic in May 2020 until August 2022 (inclusive), for visits and discharge diagnosis containing the word "elbow" or "arm."

Perilunate and Lunate Dislocations.

Lunate and perilunate dislocations, although somewhat uncommon, are serious injuries and can be easily missed. The mechanism of injury is hyperextension of the wrist, often associated with a fall on the outstretched hand (FOOSH) injury, falls from height, or motor vehicle crash with a high-energy traumatic injury to the wrist. Perilunate dislocations typically present with pain and swelling over both the dorsal and volar aspects of the wrist and limited range of motion of the wrist. Perilunate dislocations result in disruption of the relationship between the lunate and the capitate, whereas lu

The floating first metacarpal: A rare case.

A floating metacarpal is defined as the simultaneous dislocation of the metacarpophalangeal (MCP) and carpometacarpal (CMC) joints. Metacarpal dislocations are rare, and floating metacarpals with double dislocations are extremely rare. In this article, we present a very rare case of floating metacarpal in which the first MCP and CMC dislocations were found simultaneously and successfully treated with closed reduction, open reduction, and Kirschner wire fixation methods.

Carpal dislocations.

Carpal dislocations result from sequential disruption of the complex relationship between the bones and ligaments of the wrist. Injuries to the carpus occur via predictable mechanisms, an understanding of which is critical to identify and treat these frequently missed patterns of injury and to avoid the sequela of chronic instability. Lunate dislocations are by far the most common, but isolated dislocation of other carpal bones can also occur. Open reduction and internal fixation still remains the gold standard for treatment regardless of the debate around the specific approaches. These high-e

Percutaneous Reduction of Intra-Articular Posterior Wall Acetabular Rim Fragment: Surgical Technique and Case Series With an Average Follow-Up of 2 Years.

Small posterior wall rim fractures are typically stable; however, if incarcerated in the joint, they must be removed. It is possible to reduce the morbidity associated with open approaches by addressing these incarcerated fragments in a percutaneous manner. This allows the restoration of joint congruity and removal of the osteochondral fragment from the joint space. The following report details the surgical technique to accomplish this, and the results of a case series of patients who underwent this technique. The advantages include limiting the morbidity of an open approach. However, the surg

Predictive factors for the development of Gartland type IV supracondylar humerus fractures: a prospective clinical study.

OBJECTIVE: This study aimed to identify the preoperative predictive factors for the development of Gartland type IV supracondylar humerus fracture based on the patient characteristic, fracture mechanism, and preoperative radiographic fracture characteristics. METHODS: This prospective study included the data of 120 patients with Gartland type III and IV supracondylar humerus fractures treated in a single center from 2020 to 2021. Patients' age, gender, height/weight percentile values, injury mechanisms, the proximity of fracture fragment to the skin (i.e., dimple sign), and time from trauma t

Subtalar dislocation: a narrative review.

BACKGROUND: Subtalar joint dislocation (1% of all dislocations) is the permanent loss of articular relationships in the talonavicular and talocalcaneal joints, without other involvement of the foot. Dislocation can occur medially (85%), laterally (15%), posteriorly (2.5%) and anteriorly (1%). Reduction can be performed by closed or open technique; lateral dislocations often require open reduction because of inclusion of soft tissues or bone fragments. Lateral dislocations are frequently complicated by bone exposure, risk of infection and associated soft tissues injuries. AIM OF THE STUDY: The

Bipolar clavicular dislocation treated with biological anatomical repair and reconstruction: A case report with a literature review of management options.

Bipolar clavicular dislocation is rare, and therefore its management is contentious. With an increase of patient's physical demand and a near-normal shoulder function, there has been a shift in the paradigm of its management from a conservative one to a stabilized one of anatomical open reduction. Proposed methods of fixation have also evolved with the advent of more biological fixation devices, which elude implant or fixation related complications. This case report highlights the salient features of this rare case and details the management options along with the benefits of biological anatom

Management of proximal interphalangeal joint injuries.

Injuries about the PIP joint of the finger are commonly encountered by primary care physicians and are associated with significant morbidity, including pain, stiffness, instability, premature degenerative arthritis, and residual deformities. An accurate understanding of the regional anatomy and appreciation of the mechanism of injury allows for classification so that a treatment protocol can be formulated for each injury pattern. Emphasis on careful consideration of the implications of open reduction, rational splinting, and early, active, protected motion provides for the most favorable outco

Dislocation of the elbow with intra-articular entrapment of the medial epicondyle in adults. Report of two cases.

Intra-articular entrapment of the medial epicondyle with dislocation of the elbow is a common injury pattern in childhood. The occurrence of this injury is extremely rare in adults. We present two cases of dislocation of the elbow with intra-articular inclusion of the medial epicondyle in adult patients. One case was treated by closed reduction and the other required open reduction. A brief review of the scanty literature on the subject is also presented. The case reports highlight the need for early diagnosis and management of these atypical cases in adults.

Avulsion fracture of the flexor digitorum profundus tendon ('Jersey finger') type III.

Avulsion fracture of the flexor digitorum profundus tendon (Jersey finger) is an uncommon injury. This injury involves mainly young and active patients, especially during sports activities. It is classified into three types according to the extent of the tendon retraction. Type III Jersey finger is very rare, and only a few cases have been described so far.In this article, we report a case of a type III Jersey finger. Rapid diagnosis accompanied by surgery which included open reduction and internal fixation with 2 mini screws, followed by physiotherapy, led to restoration of a full range of mo

Injuries of the shoulder girdle.

Shoulder girdle injuries occur infrequently, but can lead to high rates of morbidity and mortality if these high energy injuries are underdiagnosed or mistreated. Most injuries can be treated conservatively, but a high index of suspicion and aggressive physical and radiographic evaluations should be used to evaluate patients with polytrauma. This article discusses these injuries, their associated pathoanatomy, and current management.

[Injury of the hamatometacarpal joint--treatment guidelines based on a new classification scheme].

Using a scheme in accordance with the AO-fracture classification, principles of conservative and operative treatment of lesions of the hamatometacarpal joint are given. Because of mild radiological signs, diagnosis may be difficult; if clinical symptoms are indicative for a lesion of the hamatometacarpal joint, a careful search is mandatory.

[Three cases of thoracic spina fracture with displacement without spinal cord injury].

Three cases of extremely rare, thoracic spine displaced fractures without injury of the spinal cord are reported. An attempt to reconstruct injury mechanism and pathology of the fracture has been undertaken on the basis of radiographs analysis. Immobilisation treatment with no reduction yielded good results. The authors are convinced that this paper, if studied, will indicate the management of this type of injury to those come across it for the first time.

Traumatic dislocations of the patella: problems related to treatment.

Traumatic dislocation of the patella is an infrequent occurrence favored by dysplasia of the extensor apparatus of the knee. The problem of diagnosis is at the basis of a choice of treatment, as any pre-existing dysplasia may be worsened by traumatic dislocation. Furthermore, the presence of a patellar or femoral osteochondral fragment, a more frequent finding than is generally believed as demonstrated by arthroscopy, influences surgical treatment. The results of 37 cases are reported, and several details related to surgical method discussed.
