INTRODUCTION: The aim of this study was to evaluate the subjective and objective outcome of the percutaneous minimal osteosynthesis in elderly patients. Untreated, unstable and dislocated proximal humeral fractures show poor functional and subjective results. Reduction and fixation of the fragments is essential to achieve a good clinical outcome. Especially noted in elderly patients, the osteosynthesis is concurrent with the implantation of a prosthesis.
PATIENTS AND METHODS: We examined 31 patients with a mean age of 72 years (51-87) after an average follow-up period of 15.8 months (9-31). T

STUDY DESIGN: A mixed cross-sectional survey and cohort study using a prospectively gathered database of persons with traumatic spine injury.
OBJECTIVES: To identify demographic and injury mechanism factors that predict greater injury severity, and to determine the effect of injury severity on outcomes in traumatic spine fracture.
SUMMARY OF BACKGROUND DATA: Traumatic spine fracture outcome studies have focused on defining type and level of vertebral fracture without considering the severity of associated injuries. In the trauma population, greater injury severity has been shown to be relate

INTRODUCTION: Fracture of the atlas was first described by Jefferson (1920). He theorised a bursting mechanism of fracture as the occipital condyles were driven into the atlas. Experimental studies by Hays and Alker (1988) and Panjabi et al (1991) were also conducted to explain the injury mechanisms. Injury mechanisms and fracture patterns are important in the clinical evaluation of spinal injuries. Recognition and interpretation of the fracture patterns help to determine the spinal instability and consequently the choice of treatment. Although the fracture mechanics of the atlas have received

INTRODUCTION: Kirschner wires (K-wires) are used in fracture fixations but are often associated with pin tract infections. This prospective study compared the infection rate between buried and exposed K-wires in closed injuries of the wrist and hands in individuals with no comorbidities.
METHODS: Fifteen patients were recruited with a total of 41 K-wires (21 buried K-wires; 20 exposed K-wires). Clinical and radiographic evidence of infection was assessed at three months based on the Modified Oppenheim classification.
RESULTS: Two out of 21 wires in the buried group developed grade 4 infectio

OBJECTIVE: Anatomic reduction and stabilization in case of dislocated injuries.
INDICATIONS: Dislocated epiphysiolyses types Harris-Salter I and II of the distal femur and the proximal tibia.
CONTRAINDICATIONS: General contraindications to anesthesia and surgical treatment as well as nondislocated injuries.
SURGICAL TECHNIQUE: After initial closed anatomic reduction, the fracture is stabilized with crossing Kirschner wires. If closed reduction is not successful, open reduction via a small medial or lateral incision becomes necessary. Selected injuries with a big metaphyseal wedge should be

Closed ankle injury without fracture is a common finding in the emergency room. Outcome is generally spontaneously favorable, the injury having no long-term clinical impact. Exceptionally, these injuries can be associated with arterial damage. We report a case of an apparently benign closed ankle injury which was found to be associated with serious arterial damage. Forced dorsal flexion of the ankle joint during a traffic accident caused an arterial lesion without any apparent damage to the bone and joints. The posterior tibial artery was interrupted leading to subacute ischemia of the foot. T

OBJECTIVE: To quantify the three-dimensional (3D) displacement characteristics of the posterior facet fragment in Sanders type II and III intra-articular calcaneal fractures (IACFs) caused by falls from height, thereby providing a quantitative anatomical reference for reduction planning.
METHODS: We retrospectively analyzed CT data from 71 patients with closed Sanders type II/III IACFs. Mimics and 3-matic software were utilized for 3D reconstruction and simulated reduction. The translational and rotational changes of the fragment's geometric center were measured before and after reduction.

The treatment of mandibular fractures affecting the condylar head (CHF) can be either closed or open. In the case of an open approach, the headless bone screw (HBS) is an option. This study was performed to investigate the efficacy of osteosynthesis with HBS using three-dimensional radiographic imaging and clinical evaluation over long-term follow-up. This was a single-centre retrospective study. Clinical parameters and three-dimensional radiographic scans were collected during follow-up, DICOM datasets were segmented, and model analysis was conducted. Forty-five patients who received a HBS an

BACKGROUND: Bilateral facet dislocation results from flexion and distraction forces that tear the posterior ligamentous complex, disrupt the facet joint capsules, and can cause fracture of the superior articular processes of the facet joints, leading to vertebral subluxation and potentially serious spinal cord injury. When clinically feasible, the typical management is rapid closed reduction followed by fusion, typically by means of anterior, posterior, or circumferential fixation. However, the risk of construct failure in the event of anterior-only fixation is not fully understood. Since cons

Diaphyseal fractures of the tibia is a common injury with an incidence of 16.9/100,000 population. Surgical treatment with intramedullary interlocking nail is the treatment of choice in closed diaphyseal tibial injuries. Removal of the nail is required in non-union, infected implant, implant failure or during exchange nailing procedure. Implant removal surgery presents some unique challenges to the Orthopaedic surgeons like broken nails, broken screws and failure of the extraction devices to engage with the threads of the nail due to stripping of the threads in the proximal part of the nail. While it is well-established that bone responds dynamically to mechanical loading, the effects of mild traumatic brain injury (mTBI) on cranial bone composition are unclear. We hypothesized that repeated mTBI (rmTBI) would change the microstructure of cranial bones, without gross skull fractures. To address this, young adult female Piebald Viral Glaxo rats received sham, 1×, 2× or 3× closed-head mTBIs delivered at 24 h intervals, using a weight-drop device custom-built for reproducible impact. Skull bones were collected at 2 or 10 weeks after the final injury/sham procedure, imaged by micro co

Open anterior hip dislocation is a rare condition and results from high-energy trauma. Ten cases of open anterior dislocation have been described in the literature so far. Its rarity is due to the inherent stability of the joint, its deep position in the pelvis, with strong ligaments and bulky muscles around the articulation. Several factors influence the prognosis, such as the degree of compounding, the associated soft tissue injuries, the age of the patient and, mainly, the delay in reduction. The main complications are: arthrosis of the hip, with incidence of 50% of cases, when associated w
