Global burden of chikungunya virus infections and the potential benefit of vaccination campaigns.

The first vaccine against chikungunya virus (CHIKV) has now been licensed; however, due to a limited knowledge of the underlying global burden, its potential to reduce disease burden remains unknown. We used data from seroprevalence studies, observed cases and mosquito distributions to quantify the underlying CHIKV burden in 180 countries and territories, and we explored the potential impact of vaccination campaigns. We estimate that 104 countries have experienced CHIKV transmission, covering 2.8 billion people, and that, in epidemic settings, the mean duration between outbreaks is 6.2 years,

Health Determinants in Association with Cancer Mortality from a Health Ecological Perspective.

OBJECTIVE: To explore the health determinants of all-cancer mortality from a health-ecological perspective. METHODS: Using the health-ecological model, we identified 15 health determinants across five domains: personal innate, behavioral, interpersonal network, socioeconomic, and macroenvironmental characteristics. Data on cancer outcomes and health determinants from 185 countries, classified using the Human Development Index (HDI), were extracted from GLOBOCAN 2022, the World Health Organization Global Health Observatory, the Global Burden of Disease Study 2021, the United Nations, and the W…

Global perspectives in acute and emergency general surgery in low and middle-income countries: a WSES project protocol for scoping review on global surgery.

BACKGROUND: Around five billion people globally lack access to safe, timely, and affordable surgical facilities and care in low-income and middle-income countries (LMICs). Global initiatives have been launched, including efforts led by organizations. Also, regional efforts have shed light on the unique challenges faced by different areas within LMICs. Despite these efforts, many countries still face significant challenges, including inadequate infrastructure, limited availability of trained surgical personnel, lack of essential medical equipment, and insufficient financial resources allocated

The global burden of human metapneumovirus-associated acute respiratory infections in older adults: a systematic review and meta-analysis.

BACKGROUND: The human metapneumovirus (hMPV)-associated disease burden in older adults remains under-researched. We aimed to systematically estimate the global burden of hMPV-associated disease in older adults. METHODS: We searched MEDLINE, Embase, Global Health, CINAHL, Web of Science, and Global Index Medicus in February, 2023, November, 2023, and October, 2024; and CNKI, Wanfang, and CQVip, in April, 2024, and October, 2024. We included studies conducted over at least 12 consecutive months, reporting on adults aged 60 years or older, and with laboratory-confirmed hMPV infections. Critical …

Enhancing Diagnostic Accuracy of Ophthalmological Conditions With Complex Prompts in GPT-4: Comparative Analysis of Global and Low- and Middle-Income Country (LMIC)-Specific Pathologies.

BACKGROUND: The global incidence of blindness has continued to increase, despite the enactment of a Global Eye Health Action Plan by the World Health Assembly. This can be attributed, in part, to an aging population, but also to the limited diagnostic resources within low- and middle-income countries (LMICs). The advent of generative artificial intelligence (AI) within health care could pose a novel solution to combating the prevalence of blindness globally. OBJECTIVE: The objectives of this study are to quantify the effect the addition of a complex prompt has on the diagnostic accuracy of a

Comparison of left atrial appendage occlusion with medical treatment for non-valvular atrial fibrillation: systematic review, network and reconstructed individual patient data meta-analysis.

BACKGROUND: Percutaneous left atrial appendage occlusion (LAAO) is a non-pharmacological strategy to prevent stroke and systemic emboli in patients with non-valvular atrial fibrillation (AF). However, data regarding its safety and efficacy profile compared to different oral anti-coagulant regimens remain limited. METHODS: A network meta-analysis compared LAAO, warfarin, and NOACs (standard dose [SD] and low-dose [LD]). Outcomes included all-cause mortality, stroke or systemic embolism, and bleeding risk. Bayesian models with surface under the cumulative ranking curve (SUCRA) and reconstructed

[Green dialysis and plant-based diets].

Human and planetary health are interconnected. The UN's Sustainable Development Goals aim for a healthy, peaceful world, but climate change - accelerated by human activity - is a major threat. Rising temperatures, water scarcity, pollution, and biodiversity loss contribute to worsening health crises. The healthcare sector itself is a major polluter, responsible for >5% of global CO₂ emissions. Chronic kidney disease (CKD) is increasingly linked to climate change, with extreme heat, air pollution, and water contamination exacerbating risks. Dialysis has a heavy environmental footprint, consumin

The impact of traffic legislation on maxillofacial trauma: A global retrospective review.

Maxillofacial trauma remains a critical public health concern, with road traffic accidents (RTAs) being a leading cause, particularly in low- and middle-income countries. While preventive measures like helmet laws and alcohol restrictions have shown efficacy, gaps persist in understanding their direct impact on maxillofacial injury rates globally. A retrospective, multi-database review (PubMed, WHO reports, grey literature) analyzed national traffic laws and maxillofacial trauma incidence. Data from 37 studies across five continents were evaluated using RStudio® for statistical correlation (p

Estimation and prediction on the economic burden of schistosomiasis in 25 endemic countries.

BACKGROUND: Schistosomiasis is a neglected tropical disease, primarily prevalent in tropical and subtropical regions. It imposes a significant health and economic burden in low- and middle-income countries, but a study of its comprehensive economic impact of the disease at the global level has not been carried out. As this is essential for evidence-based decision-making, this study aims to estimate the macroeconomic burden of schistosomiasis in 25 endemic countries. METHODS: We used a health-augmented macroeconomic (HAM) model, as well as observed data from 2010 to 2021 and projected data fro

Seroprevalence of Toxoplasma gondii in Equids: A Global Systematic Review and Meta-analysis.

BACKGROUND: Toxoplasmosis is a zoonotic infectious disease caused by T. gondii, a widely distributed obligate intracellular parasite. It has received widespread attention because it can infect a large number of domestic and wild animals, thereby posing a threat to public health and the economy on a global scale. Among them, equine animals are critical intermediate hosts. However, studies on T. gondii infection in equine animals are currently not systematic on a global scale. For this purpose, we conducted a systematic review and meta-analysis. METHODS: This study searched for relevant studies

Epidemiology of Chronic Suppurative Otitis Media: Systematic Review To Estimate Global Prevalence.

BACKGROUND: Chronic Suppurative Otitis Media (CSOM) is a disorder characterised by a perforation of the tympanic membrane leading to ear discharge and hearing loss, a disability causing marginalisation in employment, education, social stigma, and reduced wellbeing and quality of life. Previous studies on the global epidemiology of this condition, despite methodological limitations, have estimated global prevalence at 200 million. METHODS: Adhering to PRISMA guidelines, published literature was systematically reviewed across Ovid and Embase databases, with original community-based studies on C

Use of artificial intelligence for detecting left ventricular dysfunction and predicting incident heart failure risk.

Effective medications are available for the prevention of heart failure (HF). While their use is indicated in patients with risk factors, engagement and adherence among 'at risk' individuals is challenging, as it is with atherosclerotic heart disease prevention. The detection of patients with subclinical cardiac dysfunction could provide a subgroup at heightened risk, warranting more intensive disease management programmes. The process of screening the aging population is a huge task that could be facilitated using artificial intelligence (AI) to identify clinical risk, select 'at risk' indivi

From treatment to prevention: reimagining schistosomiasis control through WASH and environmental management.

Schistosomiasis remains a persistent global health challenge, primarily controlled through Mass Drug Administration (MDA) of praziquantel. While MDA has reduced prevalence and morbidity in endemic regions like China and Sierra Leone, its limitations, including rapid reinfection, logistical barriers to coverage, emerging drug resistance, and failure to interrupt transmission, highlight the inadequacy of a purely pharmacological approach. This review advocates for a paradigm shift from treatment-focused to prevention-oriented strategies, integrating Water, Sanitation, and Hygiene (WASH) interven…

The Key Importance of Screening Underprivileged People in Order to Achieve Global Hepatitis Virus Elimination Targets.

Chronic hepatitis B (HBV), alongside hepatitis D virus (HDV) super-/co-infection and chronic hepatitis C (HCV), are major contributors to cirrhosis, end-stage liver disease, hepatocellular carcinoma (HCC), and liver-related mortality. Despite significant progress in antiviral treatments and HBV vaccination, viral hepatitis remains a global health burden. Vulnerable populations, such as those experiencing homelessness, migrants, and economically disadvantaged groups, are disproportionately impacted by these infections, often facing barriers to care and exclusion from traditional health systems.…

Modelling international spread of clade IIb mpox on the Asian continent.

OBJECTIVE: To understand and simulate international spread of the disease mpox, considering variations in sexual activity levels and international travel among men who have sex with men. METHODS: We developed a mathematical model that considers differing sexual networks and the volume of international travel among men who have sex with men, calibrated to disease incidence data in Japan. We then used our model to simulate the potential international spread of mpox across 42 countries and territories on the Asian continent, assuming Japan as the origin of spread. FINDINGS: Our simulations iden

Epidemiology of the Crimean-Congo hemorrhagic fever virus.

Crimean-Congo hemorrhagic fever virus (CCHFV) is endemic in >30 countries across Asia, Europe, the Middle East, and Africa. CCHFV causes Crimean-Congo hemorrhagic fever (CCHF), a disease characterized by bleeding and haemoptysis. Since the first diagnosis in 1944, 10,000-15,000 CCHF cases are reported worldwide annually. Around three billion people are at risk of CCHFV infection. The World Health Organization (WHO) announced CCHFV as a priority pathogen in December 2015 due to its widespread and significant healthcare threat. In this review, we track the outbreaks, discuss their origin, and an
