Designing Well-Being Intervention Trials for People With Progressive Multiple Sclerosis: The Importance of Understanding Usual Care Comparators.

Well-being captures an individual's capacity to lead a meaningful and engaged life. Interventions to address well-being go beyond managing symptoms or supporting daily activity performance. Designing trials to evaluate the effectiveness of such well-being interventions can be challenging. One such challenge is determining what the comparator or control arm should be. Several trials of complex interventions use usual care as a comparator, but this can be particularly challenging when planning trials across multiple countries and health care systems. Therefore, trial design decisions must be inf

Gene Discovery and Biological Insights into Anxiety Disorders from a Multi-Ancestry Genome-wide Association Study of >1.2 Million Participants.

We leveraged information from more than 1.2 million participants to investigate the genetics of anxiety disorders across five continental ancestral groups. Ancestry-specific and cross-ancestry genome-wide association studies identified 51 anxiety-associated loci, 39 of which are novel. Additionally, polygenic risk scores derived from individuals of European descent were associated with anxiety in African, Admixed-American, and East Asian groups. The heritability of anxiety was enriched for genes expressed in the limbic system, the cerebral cortex, the cerebellum, the metencephalon, the entorhi

Implementation of collaborative psychiatric care in a family medicine clinic: A quality assurance study.

In recent decades, numerous primary care clinics throughout the United States have implemented a collaborative care model of psychiatry in their practice. In this care model, patients with a psychiatric diagnosis meet with a team commonly composed of a primary care provider, behavioral health provider, and psychiatric consultants to develop a well-informed treatment plan. The St. Mary's Family Medicine Center in Grand Junction, Colorado implemented this care model in March 2020. Here, we evaluated its implementation and assessed its efficacy in producing favorable patient outcomes. We performe

Prostatic irradiation-induced sexual dysfunction: A review and multidisciplinary guide to management in the radical radiotherapy era (Part III on Psychosexual Therapy and the Masculine Self-Esteem).

Psychological morbidity, sexuality, and health/system information have been identified as the highest areas of support needs in patients undergoing management of their prostate cancer (PCa). Management of a patient's sexual function prior to, during and after PCa radiotherapy requires multidisciplinary coordination of care between radiation oncologists, urologists, dermatologists, pharmacists, and psychiatrists. The finale of this three-part review provides a framework for clinicians to better understand the role of mental healthcare providers in the management of sexual toxicities associated

Bayesian evidence synthesis for a transmission dynamic model for HIV among men who have sex with men.

Understanding infectious disease dynamics and the effect on prevalence and incidence is crucial for public health policies. Disease incidence and prevalence are typically not observed directly and increasingly are estimated through the synthesis of indirect information from multiple data sources. We demonstrate how an evidence synthesis approach to the estimation of human immunodeficiency virus (HIV) prevalence in England and Wales can be extended to infer the underlying HIV incidence. Diverse time series of data can be used to obtain yearly "snapshots" (with associated uncertainty) of the pro

[Actual state of knowledge of the german population about the early detection of colorectal cancer--a study by the "Stiftung LebensBlicke" in cooperation with the institute for demoscopy in Allensbach].

Colorectal cancer is the most common cancer in Germany. The LebensBlicke foundation and it's supporting alliance have or many years, like others, been pushing information campaigns abour early detection measures and their chances for the general population. With the aid of the institute for public opinion research in Allensbach a representative cross-sectional investigation about the state of knowledge, opinions and attitudes concerning this topic was carried out. Surprisingly, it was demonstrated that an information deficit no longer exists and that especially people of older age are concerne

From attending a class on breast self-examination (BSE) to coping with breast cancer: the experiences of eight women.

This study compared the effectiveness of three methods of teaching breast self-examination (BSE). It involved 587 women who were followed for six years. During the follow-up period, eight women developed breast cancer. These women were interviewed to determine their care-seeking behaviours. Generally speaking, they identified the first sign of the problem themselves; most sought attention fairly soon after detection; some received a diagnosis immediately, while others experienced considerable delay. They were not particularly well-informed about their treatment options but were fairly well-inf

Incidence of acute care adverse events and long-term health-related quality of life in patients with TSCI.

BACKGROUND CONTEXT: Adverse events (AEs) with significant resultant morbidity are common during the acute hospital care of patients with traumatic spinal cord injury (TSCI). The Rick Hansen SCI Registry (RHSCIR) collects Canada-wide data on patients with TSCI, such as sociodemographic, injury, diagnosis, intervention, and health outcome details. These data contribute to an evidence base for informing best practice and improving SCI care. As the RHSCIR captures data on patients from prehospital to community phases of care, it is an invaluable resource for providing information on health outcome

Racialized vulnerability and socioeconomic determinants of health among Afghan refugees in Pakistan.

Afghan refugees face persistent poverty, social marginalization, and restricted access to healthcare in Pakistan, making them one of the world's largest and longest-displaced groups. Health disparities have been made worse by decades of instability, especially among marginalized groups living in informal urban settlements and refugee camps. With an emphasis on how income, education, and legal status affect health outcomes and healthcare access, this study explores the socioeconomic determinants and health burdens among Afghan refugees residing in Pakistan. Between January and June 2025, 250 Af

HIV and drug overdose: perspectives on risk of overdose and access to health care among people living with HIV in Vancouver, British Columbia.

Drug toxicity and overdose remain major public health concerns in British Columbia (BC), with People Living With HIV (PLWH) experiencing disproportionately high rates of nonfatal and fatal overdose due to intersecting social and structural inequities. The COVID-19 pandemic exacerbated these vulnerabilities and introduced challenges that impacted health care delivery and prevention efforts, with lasting repercussions. This qualitative study explored perceptions of overdose risk and access to post-overdose care among PLWH who use drugs and the Health Care Providers (HCPs) who support them in Van

Addressing Public Health Risks: Strategies to Combat Infectious Diseases After the August 2024 Floods in Bangladesh.

The August 2024 floods in Bangladesh have precipitated a major public health crisis, significantly elevating the risk of waterborne and vector-borne diseases and exacerbating existing health vulnerabilities. This disaster has impacted over 5 million people, causing widespread environmental disruption, population displacement, and strained healthcare resources. The flooding of latrines, sewage systems, and agricultural land has led to the contamination of drinking water sources, increasing the risk of cholera, enterotoxigenic Escherichia coli diarrhoea, shigellosis, and hepatitis. Additionally,

Clinicians', patients' and carers' perspectives on borderline personality disorder in Pakistan: A mixed methods study protocol.

Borderline Personality Disorder (BPD) is a condition characterised by significant social and occupational impairment and high rates of suicide. In high income countries, mental health professionals carry negative attitudes towards patients with BPD, find it difficult to work with patients with BPD, and even avoid seeing these patients. Negative attitudes and stigma can cause patients to fear mistreatment by health care providers and create additional barriers to care. Patients' self-stigma and illness understanding BPD also affects treatment engagement and outcomes; better knowledge about ment

Detention of People Lost to Follow-Up on TB Treatment in Kenya: The Need for Human Rights-Based Alternatives.

Adherence to treatment is a key element for global TB control. Public health laws can be used to enforce isolation, adherence, and completion of TB treatment. However, the practical application of public health laws can potentially range from voluntary measures to involuntary detention approaches. This paper explores the potential risks and impacts of using detention approaches to enforce TB treatment adherence. In August 2015, we conducted a literature search regarding the application of public health laws to enforce adherence to TB treatment globally, and specifically in Kenya. Texts were an
